Cardiovascular implications in the treatment of obstructive sleep apnea.

Epidemiological studies provide strong evidence that obstructive sleep apnea (OSA) is associated with cardiovascular complications such as systemic hypertension, congestive heart failure, and atrial fibrillation. Successful OSA treatment with continuous positive airway pressure (CPAP) has resulted in coincident reductions in systemic hypertension, improvements in left ventricular systolic function, and reductions in sympathetic nervous activity. These data suggest that successful treatment of OSA may reduce cardiovascular morbidity in such patients. Although CPAP is the more successful treatme…

[Physiological role of ITR in reflex and sleep apneas].

This study demonstrates that unilateral excitotoxic lesionin of the pontine ITR increases the frequency of spontaneous central sleep apneas in freely behaving rats without alteration in average respiratory rate, minute ventilation, or sleep architecture. Thus, our findings suggest that the ITR may function specifically to dampen respiratory perturbations and that bilateral integrity of this structure is important. We may also speculate that the ITR functions to maintain stable breathing during sleep in man.

Is obstructive sleep apnea a problem in Parkinson's disease?

BACKGROUND: Parkinson's disease (PD) is associated with sleep disorders and daytime sleepiness. Upper airway dysfunction in PD may promote obstructive sleep apnea. However, the frequency and clinical relevance of sleep-disordered breathing in PD remains unclear. METHODS: Sleep apnea symptoms, cardiovascular events and treatment were collected in 100 patients with PD (50 unselected, consecutive patients matched for age, sex and body mass index with 50 patients referred for sleepiness) and 50 in-hospital controls. The motor and cognitive status was evaluated in patients with PD. The 150 subject…

Success and failure in treatment of sleep apnea patients.

A variety of surgical procedures exist to treat obstructive sleep apnea in adults. Some, such as tracheotomy and maxillomandibular advancement, have very high cure rates, over 90%. These procedures have significant disadvantages, however, and there is a need to define the best combination of low morbidity procedures to provide similar success. To do this, better means to diagnose the sites of obstruction must be used and new procedures must be developed. This article reviews where we are in reaching these goals.

Diagnosis and treatment of sleep apnea in heart disease.

One of the most common yet unidentified conditions in heart disease is sleep-disordered breathing (SDB). Although it is most prevalent in patients with heart failure, it has been epidemiologically and pathophysiologically linked to ischemic heart disease, hypertension, sudden cardiac death, atrial fibrillation, and stroke. There are two primary SDB syndromes: obstructive sleep apnea (OSA) and central sleep apnea (CSA; also known as Cheyne-Stokes respiration). The pathophysiologic mechanisms that underlie these disorders appear to be distinct but both involve recurrent cycles of excessive sympa…

The effect of altitude descent on obstructive sleep apnea.

BACKGROUND: The present requirement for "at facility" polysomnograms requires many residents in mountain communities to descend in elevation for sleep testing, which may cause misleading results regarding the severity of obstructive sleep apnea (OSA). DESIGN: Eleven patients with previously undiagnosed sleep apnea living at an altitude > 2,400 m (7,900 feet) in Colorado underwent diagnostic sleep studies at their home elevation and at 1,370 m (4,500 feet), and 5 of the 11 patients were also studied at sea level. RESULTS: The mean (SE) apnea-hypopnea index (AHI) fell from 49.1 (10.5)/h to 37.…

[Revision of obstructive sleep apnea syndrome in the child].

The obstructive sleep apnea syndrome (OSAS) is frequent in infancy and childhood. It is caused by a prolonged upper respiratory airway obstructioon during sleep, and adenotonsillar hypertrophy is the most important cause. OSAS may have an impact on physical and cognitive development, but syntoms in children are subtle and may pass unrecognised. Polysomnography is the gold standard technique for OSAS diagnosis and surgical approach with adenotonsillectomy is the most frequently treatment indicated. Early diagnosis and treatment and adequate follow up are important to prevent physical disturbanc…

[Treatment of the obstructive sleep-apnea syndrome in adults].

When treating the obstructive sleep-apnoea syndrome (OSAS), conservative management and the correction of treatable stenoses in the upper airway should be considered first. If these measures are neither effective nor applicable, then continuous positive airway pressure (CPAP) is the preferred treatment. Surgical interventions should only be considered after failure of non-surgical treatment modalities. Pharmacological management of OSAS is usually only indicated as a form of supplementary treatment in specific patients. Oral-appliance therapy appears to be of value in the management of OSAS an

Taft and Pickwick: sleep apnea in the White House.

As President of the United States from 1909 to 1913, William Howard Taft's minimum body mass index was 42 kg/m(2). This article presents evidence that he suffered from obstructive sleep apnea, manifested by excessive daytime somnolence, snoring, systemic hypertension and, perhaps, cognitive and psychosocial impairment. As president, Taft's hypersomnolence was severe and obvious, but never prompted official discussion of his fitness to govern. Within 12 months of leaving office, Taft permanently lost over 60 pounds. His somnolence resolved. As Chief Justice of the United States from 1921 to 193…

Blood transfusion in anemic infants with apnea of prematurity.

We found recently that blood transfusions had no effect on bradycardia and hypoxemia, the clinically important components of apnea of prematurity, in mildly anemic infants. Here, we wanted to know whether this also holds true for more severely anemic patients. Nineteen preterm infants, median gestational age at birth 25 (range 22-30) weeks, age at the time of study 5.5 (range 1-13) weeks, for whom a blood transfusion was ordered because of recurrent episodes of bradycardia and/or hypoxemia in conjunction with anemia (median hemoglobin level 78 g/l, range 63-98 g/l) were investigated. One infan…

Gastroesophageal reflux and apnea of prematurity: no temporal relationship.

OBJECTIVE: A relationship between gastroesophageal reflux (GER) and apnea of prematurity (AOP) has long been suspected but is difficult to prove because most GER in this age group is nonacidic and thus undetectable by pH monitoring, the current standard for GER detection. The new multiple intraluminal impedance (MII) technique allows pH-independent reflux detection via changes in impedance caused by a liquid bolus inside the esophagus. We used this technique to investigate whether there is a temporal relationship between GER and AOP and whether GER occurs predominantly before a cardiorespirato…

Theophylline or caffeine: which is best for apnea of prematurity?

Apnea of prematurity is a common problem of the premature infant under 30 weeks gestation. Theophylline and caffeine, two methylxanthines, are widely used to treat this condition. The drugs are equally effective in preventing apnea in the premature infant. Caffeine citrate has many advantages over theophylline, however, including once-a-day dosing, more predictable plasma concentrations, earlier onset of action, and minimal side effects. Caffeine is therefore the initial drug of choice for apnea of prematurity.

The relationship between gastroesophageal reflux and apnea in infants.

OBJECTIVE: To determine the prevalence and relationship between apnea and gastroesophageal reflux (GER) among infants with apparent life-threatening events. METHODS: We evaluated 67 infants of ages up to 6 months who had an idiopathic apparent life-threatening event as determined by polysomnography and pH monitoring. RESULTS: No reflux (pH < 4 for more than 6 seconds) was evident in 32 infants. Fourteen others had such prolonged reflux that the relationship between apnea and reflux could not be evaluated. The 21 remaining infants who had episodes of apnea and reflux constituted the study gro…

[Surgical options for treatment of snoring and sleep apnea].

Simple snoring and obstructive sleep apnea are part of a complex disease process with a high prevalence. Surgical treatment options have been developed by otolaryngologists over the last decades. Knowledge of different surgical procedures is required to address the various findings during preoperative assessment. Patients with nasal airway blockage due to septal deviation, large turbinates or polyps may benefit from endonasal surgery. Obstructive tonsils, palatal webbing, and an enlarged uvula require surgical approaches including the soft palate and tonsils. The controversy on indications and…

From snoring to sleep apnea in a Singapore population.

We have no information on snoring and obstructive sleep apnea (OSA) in our population, which is predominantly Chinese. Our perception is that sleep apnea syndrome is more common than the 2-4% prevalence (Young et al., 1993) often quoted, judging from the experience in our sleep disorder unit. We studied the snorers in an adult population in Singapore and then went on to see how many snorers suffer pathological apnea and sleep apnea syndrome. Room partners, 220 of them aged 30-60 years, were interviewed for their observation of snoring among each other. 106 consecutive habitual loud snorers of …

[Autonomic nervous system and obstructive sleep apneas].

Understanding of the pathophysiology of obstructive sleep apnoea, a common yet relatively newly recognised condition, has advanced rapidly in recent years. This condition produces major acute haemodynamic changes and causal relationships with arterial hypertension and cardiovascular morbidity have been proposed. The role that the autonomic nervous system plays in mediating these cardiovascular changes has been the focus of intensive research activity and the development of new techniques in physiological monitoring, such as spectral analysis of heart rate variability, Finapres blood pressure m
