Histocompatibility antigens in polymyalgia rheumatica and giant cell arteritis.

Fifty-five Caucasoid patients with polymyalgia rheumatica or giant cell arteritis were typed for HLA, A, B, C and DR locus antigens. In the total patient group, DR4 had a significantly increased frequency. The prevalence of CW3 and CW6 was also significantly increased, the rise of CW3 possibly being attributable to its well-documented linkage disequilibrium with DR4. The rise in CW6 cannot be attributed to its known linkage disequilibrium with DR7.

Polymyalgia rheumatica and giant cell arteritis: circulating immune complexes.

Circulating immune complexes (CIC) were detected in a large population of patients with polymyalgia rheumatica (PMR) alone, PMR-giant cell arteritis (GCA), and GCA alone. CIC were found in the majority of patients studied. A direct correlation was observed between levels of CIC material and the erythrocyte sedimentation rate. The presence of immune complexes in the sera of patients with these disorders do not have diagnostic significance but may play an important role in their pathogenesis.

Acute ocular hypotony. A rare complication of temporal arteritis.

Acute ocular hypotony is a serious, but rare complication of temporal arteritis. It occurs equally often in men and women, and appears mostly in patients older than 70 years. This complication may occur in one or in both eyes. In the literature 7 cases of temporal arteritis accompanied by ocular hypotony have been reported. Hypotony probably results from a restricted production of aqueous humor caused by involvement of the arteries supplying the ciliary body. A high-dose corticoid therapy may normalize completely the intraocular pressure, and a timely treatment may preserve a satisfactory visi…

Polymyalgia rheumatica and giant cell arteritis--a difficult diagnosis.

This five-year study of 108 patients with giant cell arteritis and/or polymyalgia rheumatica drawn from all departments of a district general hospital emphasizes the difficulties of diagnosis. A correct diagnosis was made by the referring doctor in 33 per cent of patients and on initial attendance at hospital in 67 per cent of patients. Symptoms were present for more than three months before referral to hospital in 39 per cent of patients, and the delay before diagnosis at hospital was greater than one month in 20 per cent. Systemic illness (present in 83 per cent of cases), anaemia (33 per ce…

Coronary ischaemia and occlusion in giant cell (temporal) arteritis.

Reports of coronary artery involvement in giant cell (temporal) arteritis--polymyalgia rheumatica (GC(T)A--PMR) together with other large arteries arising from the aorta have been numerous over the past 40 years, but on this the specialist cardiac literature has been virtually silent. This article summarises that evidence, and records nine additional patients from a large group of cases with both GC(T)A--PMR and ischaemic heart disease (IHD) observed since a previous report in 1960. The case histories illustrate the benefit from corticosteroids and the hazards of non-diagnosis and premature ce…

Temporal (granulomatous) arteritis: a histopathological study of 32 cases.

Thirty-two cases of temporal arteritis were reviewed. All patients were adults with a mean age of 69.6 years, and no sex predominance. The erythrocyte sedimentation rate was raised and there was a good response to steroid therapy in the 29 patients who were adequately documented and followed up. Detailed histopathological study of the temporal artery biopsies showed three main histopathological variants; 12 cases (37.5%) had predominantly intimal proliferative changes, four cases (12.5%) had granulomatous inflammation without giant cells and 16 (50%) had granulomatous inflammation with giant c…

[Syphilitic meningo-encephalitis with probable associated arteritis (author's transl)].

A 21-year-old woman was referred for consultation because of sensory and motor disorders of sudden onset but on lung duration. Neurological examination was normal. Lumbar puncture confirmed the presence of a clear liquid meningitis. Arteriography demonstrated segmental arteritis of the anterior cerebral vessels. Systematic serological tests were positive for syphilis. The authors discuss the relation between arteritis and syphilitic meningitis in this young woman taking oral contraceptives.

[Nonspecific aorto-arteritis. Anatomo-pathological study of 18 cases].

The present study comprises a pathological review of 18 necropsy cases with nonspecific aorto-arteritis (Takayasu's disease), autopsied at the Instituto Nacional de Cardiología of Mexico up to 1976. There were 11 females and 7 males with an average age at the time of death of 21.5 years. A comparison according to the distribution and extention of arterial lesions disclosed that our cases are often less classical than those described from Japan. The principal arterial changes were stenosis, occlusion, dilatation and aneurysm formation in the aorta, its branches and main pulmonary artery. More o…

Unexplained pulmonary hypertension with pulmonary arteritis in rheumatoid disease.

The case is described of a 24-year-old woman who died after suffering from rheumatoid arthritis for eight years. During the last four months … es. Some of the muscular pulmonary arteries also showed fibrinoid necrosis of their media and acute arteritis. It was concluded that the fibrinoid necrosis was a minifestation of severe hypertensive pulmonary vascular disease rather than being a primary rheumatoid arteritis of the lung.

The combination of pericarditis, valvular disease of the heart, coronary and systemic bacterial arteritis and focal myocardial degeneration and myocarditis is unusual for Whippl'e disease. Non-specific arteritis ofhe aorta and its main branches.

Four cases of non-specific arteritis involving the aorta and its main branches are described. Three of the cases were hypertensive and one of these had evidence of aortic incompetence. Cases 1, 2, and 3 had involvement of the aortic arch vessels and the descending aorta, whereas Case 4 presented as a coarctation of the abdominal aorta. There was a significant association with systemic disturbance such as polyarthritis, fever, weight loss, raised erythrocyte sedimentation rate, and hyperglobulinaemia. A detailed necropsy in Case 2 showed two large dissecting aneurysms. The nomenclature, the dia…

Necrotizing arteritis with giant cells associated with haemolytic anaemia.

Necrotizing arteritis with giant cells, involving the aorta, pulmonary and coronary arteries, and coronary, splenic, and renal arterioles was found at necropsy in a 74-year-old male who had died with severe haemolytic anaemia associated with cold haemagglutinins. The clinical and pathological features of this case are shared by well-recognized forms of necrotizing angiitis, in particular giant cell arteritis, but do not conform satisfactorily with any of these disorders. Cold agglutinin haemolytic anaemia in this instance suggests the possibility of an autoimmune aetiology.
