Pediatric hip effusions: evaluation with power Doppler sonography.

PURPOSE: To determine if power Doppler sonography allows differentiation of infectious from noninfectious hip effusions and thereby obviates joint aspiration. MATERIALS AND METHODS: Twenty-nine consecutive children (30 hips) with sonographically identified hip effusion were prospectively evaluated with power Doppler sonography. Both hips were evaluated in each patient by using identical imaging parameters and were then compared. Medical charts were reviewed to determine the eventual diagnosis. RESULTS: At power Doppler sonography, none of 16 patients with transient synovitis had increased fl

Transient osteoporosis of the hip: successful treatment with teriparatide.

A 62-year-old man presented with a 2-month history of increasing pain in the left hip. Magnetic resonance imaging (MRI) showed bone marrow edema (BME) of the left femur, dual energy X-ray absorptiometry (DXA) showed osteopenia at the same level, whereas pelvis X-rays failed to show any objective findings. After ruling out other possible causes of BME such as aseptic osteonecrosis, infectious arthritis, primary or metastatic malignancy, tuberculosis, osteomyelitis, rheumatoid arthritis, and seronegative spondyloarthropathies, a diagnosis of transient osteoporosis of the hip (TOH) was made, and

Hanganutziu-Deicher antigen and antibody in pathologic sera and tissues.

Heterophile, Hanganutziu-Deicher (H-D) antigen was studied in pathologic sera by means of inhibition of agglutination of bovine erythrocytes by H-D antibodies. H-D antigen was demonstrated in 38% of random cancer sera, 25% of lymphoma or leukemia sera, 25% of leprosy sera, 8% of infectious mononucleosis sera, 6% of rheumatoid arthritis sera, and 27% of synovial fluids of rheumatoid arthritis patients. None of 134 normal human sera gave positive results. Some of the inhibition-positive cancer sera formed precipitation lines with H-D antibody-containing sera. Over 50% of various extracts of canc

Intra-articular organisms are not always cytologically detectable in dogs with blastomycosis-associated inflammatory arthritis.

OBJECTIVE: To describe the clinical, cytologic, and radiographic features of inflammatory arthritis associated with blastomycosis in dogs. METHODS: This study included 12 dogs with blastomycosis and cytologic evidence of inflammation in at least 1 joint identified at a tertiary referral center over a 25-year period. Median age of presentation was 5.0 (1.1 to 9.0) years, and 4 of 12 cases were Labrador Retrievers. Musculoskeletal signs were the primary complaint for 10 dogs, while the other 2 presented for respiratory concerns. All dogs were lame upon evaluation, and 10 of 12 had palpable join

Coccidiomycosis septic arthritis presenting as ankle monoarthritis in a patient with presumed psoriatic arthritis.

A man in his 50s with a history of psoriasis was evaluated for acute on chronic left ankle pain. His symptoms were attributed to psoriatic arthritis, and he tried several immunosuppressive regimens without improvement. Further diagnostic workup confirmed Coccidioides immitis/posadasii septic monarthritis thought secondary to a known remote history of Valley fever while residing in Arizona and subsequent reactivation in the setting of immunosuppression. The patient ultimately required prolonged anti-fungal therapy and multiple surgical debridements.Although psoriatic arthritis can present as mo

Risk factors for septic arthritis and multiple arthroscopic washouts: minimum 2-year follow-up at a major trauma centre.

BACKGROUND: Septic arthritis (SA) is a dangerous condition that requires emergency treatment. Managed by culture-specific antibiotics, irrigation, and debridement (I&D), some patients require repeat surgical treatment. The objectives were to determine the risk factors for SA and risk factors for repeat arthroscopic I&D in SA patients. We hypothesized that variables which directly or indirectly contributed to a larger infection burden would be associated with the development of SA and the need for repeat arthroscopic I&D. METHODS: All patients ≥ 18 years old presenting to the emergency departm

Chondroprotective effect of salicylate and chloroquine in pyogenic septic arthritis.

INTRODUCTION: This study aims to investigate the effectiveness of salicylate (Aspisol; Bayer AG, Leverkusen, Germany) and chloroquine on preventing cartilage damage in septic arthritis. METHODS: A septic arthritis model was created by inoculating the knee joint of young rabbits (n=21) with Staphylococcus aureus. Some rabbits with inflicted septic arthritis did not undergo any treatment and served as the control group (n=7). The second group (n=7) was started on salicylate on day 2 to investigate the effects of salicylate on joint cartilage. The third group (n=7) was started on chloroquine on

Poststreptococcal reactive arthritis mimicking acute septic arthritis: a hospital-based study.

Poststreptococcal reactive arthritis (PSRA) is an inflammatory arthritis that follows group A streptococcal pharyngitis. The clinical presentation of PSRA can mimic acute septic arthritis. We present 12 children with PSRA seen between December 1991 and September 1993. Most children had only mild pharyngitis by history. The pattern of arthritis was variable and included migratory polyarthritis, additive polyarthritis, and monoarthritis, and was accompanied by fever in seven children. In four patients, the presentation of fever and acute onset of painful monoarthritis mimicked septic arthritis,

Septic spondylodiscitis in ankylosing spondylitis.

A patient with ankylosing spondylitis (AS) for 10 years suddenly developed localized midback pain after minimal activity. Although he sought immediate medical assistance, recognition of a septic spondylodiscitis was delayed 3 weeks. One day after admission, he developed fever and admitted to intravenous drug use. Staphylococcus aureus empyema and spondylodiscitis were subsequently diagnosed. Clinical differentiation of aseptic from septic spondylodiscitis cannot be ignored in patients with AS.

Knee arthrodesis following total knee arthroplasty in rheumatoid arthritis.

Twenty-seven knees in 23 patients, all with seropositive rheumatoid arthritis and failed total knee arthroplasty, were treated by arthrodesis. Twenty of the 27 knees were solidly fused. A fusion aligned in 7 degrees +/- 5 degrees of valgus and knee flexion from zero to 30 degrees was associated with the highest rate of arthrodesis, the lowest rate of progression of disease in other joints, and the highest functional scores. Stable fixation using either internal or external fixation gave the most predictable rate of arthrodesis. Persistent sepsis and bone stock losses were associated with failu

Use of technetium-99m methylene diphosphonate and gallium-67 citrate scans after intraarticular injection of Staphylococcus aureus into knee joints of rabbits with chronic antigen-induced arthritis.

Numerous clinical studies have questioned the ability of radionuclide scans to differentiate septic from aseptic joint inflammation. A clinical study may not be able to document an underlying disease process or duration of infection and, thus, may make conclusions about the accuracy of scan interpretations open to debate. In this study, the Dumonde-Glynn model of antigen-induced arthritis in rabbits was used as the experimental model to study technetium and gallium scans in Staphylococcus aureus infection of arthritic and normal joints. Gallium scans were negative in normal rabbits, usually ne

[Differential diagnosis of knee joint fluid. Enzymatic analysis (author's transl)].

Between 1971 and 1976 clinical and chemical examinations of 165 joint fluids in 149 patients were made. The comparison of clinical findings and results of laboratory tests lead us to distinguish between inflammatory and non-inflammatory joint fluids. The reported tests are simple and results can be given quickly. We do not know of any literature on the incidence of CHE in joint fluids. It seems to be of considerable importance that there is a correlation between the degree of CHE-activity and the degree of cartilagenous lesions. As far as we know no comparative tests on Caeruloplasmin in the s

Family study of Reiter's disease and HLA B27 distribution.

Starting from index patients with confirmed Reiter's disease, a clinical and immunogenetic study was performed on 12 families in which there were further cases of arthritis. Altogether 51 family members were investigated and some information was available on 15 additional members. In most families there were two or three affected members in addition to the proband. The manifestations included acute polyarthritis (16 cases), which frequently followed urethritis or occurred as a complication of Yersinia or Shigella infection, and chronic arthritis (9 cases), either ankylosing spondylitis or peri

Rapidly progressing polyarticular septic arthritis in a patient with rheumatoid arthritis.

Septic arthritis is an orthopedic emergency that can lead to significant morbidity and mortality. Polyarticular involvement is a relatively rare phenomenon occurring primarily in high-risk patients. In this article, we report the rare case of a patient with rheumatoid arthritis presenting with an acute episode of septic arthritis involving most of the joints of the body. Surprisingly, his bilateral total hip arthroplasties were completely unaffected. Unusual polyarticular presentations of septic arthritis, though rare, must still be considered within the differential diagnosis by all healthcar

The importance of the disease process and disease-modifying antirheumatic drug treatment in the development of septic arthritis in patients with rheumatoid arthritis.

OBJECTIVE: To evaluate the effect of disease-modifying antirheumatic drugs (DMARDs) on the likelihood of patients with rheumatoid arthritis (RA) developing septic arthritis (SA). METHODS: The United Kingdom General Practice Research Database (GPRD) was used to identify adults with RA, and age-, sex-, and practice-matched control subjects. Subjects were studied between 1987 and 2002. The risk of developing SA (excluding infected joint replacements) for individuals with RA was calculated and the effect of DMARD use determined. RESULTS: A total of 136,977 subjects (34,250 patients with RA, 102,

Chronic arthritis in adolescence.

Musculoskeletal complaints, and joint pain in particular, are common symptoms among adolescents presenting to primary care providers. In this article, the distinction between arthralgia and arthritis is described, and important inflammatory arthropathies seen in teenagers are addressed. The current nomenclature of juvenile idiopathic arthritis is used to address each of the major clinical entities that account for chronic arthritis in teens, including oligoarthritis, polyarthritis, systemic arthritis, psoriatic arthritis, and spondyloarthropathy. Diagnostic criteria, important comorbidities, a

Selective induction of CCL18/PARC by staphylococcal enterotoxins in mononuclear cells and enhanced levels in septic and rheumatoid arthritis.

Chemokines are mediators of innate and acquired immunity. CCL18, also designated pulmonary and activation-regulated chemokine (PARC), dendritic cell-derived CC chemokine-1 (DC-CK1), alternative macrophage activation-associated CC chemokine-1 (AMAC-1) and macrophage inflammatory protein-4 (MIP-4), was for the first time isolated from peripheral blood mononuclear cells (PBMC) and biochemically characterized. We found that CCL18/PARC protein is spontaneously secreted by PBMC and is selectively induced in PBMC by staphylococcal enterotoxins (SEA, SEB) and IL-4, but not by IFN-gamma and the CXCL8/I

The importance of ileocaecal integrity in the arthritic complications of Crohn's disease.

Experiments in animal models have suggested a role for bacterial overgrowth in the caecum in the pathogenesis of extracolonic inflammation in IBD. The aim of this study was to identify patients with Crohn's disease who have undergone ileocaecal resection and to compare the incidence of new arthritic complications in these patients with those who have never undergone surgery. Patients who had undergone surgery were identified by case note review. The date and nature of surgery were noted. The occurrence of new joint complications (Type 1 and 2 peripheral arthropathy and AS) was noted in patient
