Among 481 patients undergoing various mitral valve operations, Aschoff bodies were found in 40 (21%) of 191 operatively excised left atrial appendages, in four (2%) of 273 operatively excised left ventricular papillary muscles (1 per patient), and in one (6%) of 17 patients with both left atrial appendage and papillary muscle operatively excised. Of the total of 45 patients with Aschoff bodies, 44 preoperatively had mitral stenosis, and only one, a 10-year-old boy, had pure mitral regurgitation. Sinus rhythm was present in 38 (84%), and atrial fibrillation in seven (16%). Perioperatively, only…

Cardiac computed tomography (CCT) has emerged as a central imaging modality in the workflow for left atrial appendage occlusion (LAAO), as accurate anatomical assessment is vital for successful intervention. CCT provides superior three-dimensional (3D) anatomical detail and isotropic spatial resolution compared to traditional trans-esophageal echocardiography, enabling precise pre-procedural planning, device sizing, and identification of optimal fluoroscopic projections. Furthermore, CCT serves as a highly accurate, non-invasive tool for detecting post-procedural complications such as peri-dev…

BACKGROUND: Selecting the optimal WATCHMAN FLX (a second-generation left atrial appendage [LAA] closure device) size for a borderline-sized LAA, for which 2 device sizes are acceptable, remains challenging.
OBJECTIVES: The aim of this study was to evaluate how device size affects procedural and clinical outcomes in borderline-sized LAAs undergoing LAA closure.
METHODS: From the Japanese multicenter TERMINATOR (Transcatheter Modification of Left Atrial Appendage by Obliteration with Device in Patients With NVAF) registry, 1,190 patients who underwent LAA closure with the second-generation dev…

BACKGROUND: We sought to investigate the safety of using left atrial appendage (LAA) closure with clip devices in patients who underwent off-pump coronary artery bypass grafting and who experienced atrial fibrillation.
METHODS: We applied LAA clips in 25 patients between November 2018 and May 2025. The mean age was 74 years, and 5 patients were female. All patients underwent off-pump CABG via a heart positioner and stabilizer and an LAA clip (21 with AtriClip and 4 Penditure). The mean number of coronary anastomoses was 4.0. The mean follow-up period was 28.6 ± 17.5 months.
RESULTS: There we…

BACKGROUND: Left atrial appendage closure (LAAC) is a proven stroke prevention method for patients with non-valvular atrial fibrillation (AF) who are not suitable for long-term anticoagulation. However, the use of iodinated contrast agents during the procedure can increase the risk of acute kidney injury (AKI), especially in patients with chronic kidney disease (CKD). This study evaluates the safety and effectiveness of a zero-contrast LAAC technique using transesophageal echocardiography (TEE) and fluoroscopy.
METHODS: In this retrospective study, 324 consecutive patients underwent zero-cont…

BACKGROUND: Transesophageal echocardiography (TEE) is routinely used to exclude left atrial appendage thrombus before direct current cardioversion (DCCV) in atrial fibrillation/atrial flutter but is invasive and limited by procedural availability. Cardiac computed tomography (CCT) offers a noninvasive alternative.
PROJECT RATIONALE: Limited access to TEE may delay DCCV and prolong hospital length of stay (LOS). We hypothesized that an early CCT-guided pathway would reduce time to cardioversion and LOS.
PROJECT SUMMARY: Adult patients hospitalized with atrial fibrillation/atrial flutter under…

BACKGROUND: The left atrial appendage (LAA) is the predominant site of thrombus formation in atrial fibrillation. Current percutaneous LAA occlusion devices require permanent implants. A catheter-based, non-implant mechanical inversion strategy may offer an alternative approach to stroke prevention.
OBJECTIVES: To assess the feasibility of vacuum-assisted catheter-based inversion of the LAA using transseptal aspiration in a swine model.
METHODS: A 59-kg domestic swine underwent transseptal access via the right femoral vein under fluoroscopy, transesophageal echocardiography (TEE), and intrac…

BACKGROUND: Mounting evidence suggests surgical left atrial appendage (LAA) exclusion reduces stroke risk in patients with atrial fibrillation. Prior older research suggests that LAA exclusion is often incomplete, but few transesophageal echocardiogram (TEE) data exist evaluating LAA remnants.
METHODS: We analyzed 121 patients with an available postoperative TEE who underwent LAA exclusion by surgical excision (SE), AtriClip occlusion (AO), or Tiger Paw occlusion (TO). TEE images were assessed for LAA remnant depths, presence of flow into remnant, and visible suture, thrombus, or pectinate. S…

BACKGROUND: Evidence supporting the clinical superiority of the Watchman FLX with respect to its previous iteration, the Watchman 2.5, is still sparse.
AIMS: We aimed to compare the Watchman FLX and Watchman 2.5 in terms of device-related complications and clinical outcomes.
METHODS: All consecutive left atrial appendage closures (LAACs) completed with implantation of a Watchman device at two high-volume centres between July 2018 and January 2023 were considered. Based on the type of implanted device, patients were assigned to either the Watchman FLX or Watchman 2.5 group. The study endpoint…

OBJECTIVES: This study sought to compare intracardiac echocardiography (ICE) probe via the esophageal route (TE-ICE) and transesophageal echocardiography (TEE) guidance for the LAAO procedure.
BACKGROUND: Intraprocedural imaging guidance is recommended for all LAAO procedures. Currently, both femoral-ICE-guided and TEE-guided LAAO demonstrated similar outcomes. TE-ICE may serve as a potential alternative imaging modality in LAAO. This approach avoids general anesthesia and its potential complications, while maintaining a favorable learning curve, as it is highly similar to standard transesoph

Percutaneous left atrial appendage occlusion (LAAO) is used to prevent stroke in atrial fibrillation. We present a national registry analysis of peri‑procedural outcomes of LAAO among patients with lower versus higher CHA2DS2-VASc score. The National Readmissions Database was used to perform a retrospective review of all hospitalizations for percutaneous endocardial LAAO identified between September 2015 and November 2019. ICD codes for congestive heart failure, hypertension, type 2 diabetes, stroke, transient ischemic attack, thromboembolism, and vascular disease were identified. CHA2DS2-VASc…

BACKGROUND: Left atrial appendage closure (LAAC) has become the treatment of choice for stroke prevention in patients with nonvalvular atrial fibrillation who are at high risk of bleeding or with contraindications for anticoagulation. However, the optimal duration of anticoagulation after LAAC remains uncertain. The aim of this study was to evaluate the optimal duration of treatment with novel oral anticoagulants (NOACs) after LAAC.
METHOD: We searched the PubMed, Embase, Cochrane Library, and Web of Science databases for studies related to LAAC published from inception to 20 December 2023, a…

BACKGROUND: Patients with non-valvular atrial fibrillation (AF) who experience an ischemic stroke despite oral anticoagulation (OAC) are at particularly high risk of recurrence, with a reported annualized ischemic stroke rate of 5.3%-8.9%. The optimal strategy for secondary prevention in these patients remains unknown.
METHODS: We reviewed all percutaneous left atrial appendage closures (LAAC) attempted in AF patients experiencing an ischemic stroke under OAC and who were prospectively collected in four European centers. All index strokes were categorized by an experienced neurologist to excl…

Catheter-based ventricular tachycardia (VT) ablation is widely recognized as a first-line treatment for patients with VT and structural hear … d risk of recurrent VT episodes. In such scenarios, a combined strategy involving percutaneous left atrial appendage closure followed by VT ablation may present a feasible alternative, potentially allowing for the safe and effective treatment of both conditions.

BACKGROUND: Atrial tachycardia (AT) originating from the distal left atrial appendage (LAA) is rare, especially in older adult patients.
CASE SUMMARY: A 64-year-old man was admitted for heart failure due to drug- and ablation-resistant AT. The arrhythmia was traced to the distal LAA. Owing to high risk of recurrence and complications with repeat catheter ablation, the patient underwent total thoracoscopic en bloc isolation of the left pulmonary veins and LAA, followed by LAA resection.
DISCUSSION: Sinus rhythm was restored intraoperatively and was maintained postoperatively without antiarrhy…

BACKGROUND: Dual antiplatelet therapy and oral anticoagulation in combination with aspirin represent recommended treatment regimens after left atrial appendage closure (LAAC). As most patients receiving LAAC have high bleeding risk, less aggressive antithrombotic treatments are needed, such as single antiplatelet therapy.
OBJECTIVE: We sought to compare both ischemic and bleeding outcomes in patients receiving single antiplatelet therapy (SAPT) or dual antiplatelet therapy (DAPT) after successful LAAC.
METHODS: Data on consecutive patients undergoing percutaneous LAAC between 2009 and 2023 w…
