Right atrial function by speckle tracking echocardiography in atrial septal defect: Prediction of atrial fibrillation.

BACKGROUND: Most antiarrhythmic interventional therapies for atrial fibrillation (AF) have been provided with special focus on the treatment of left-sided valvular disease and enlarged left atrium but few studies have assessed AF associated with congenital heart disease and dilated right atrium. HYPOTHESIS: We hypothesized that right atrial (RA) function assessed by two-dimensional (2DSTE) and three-dimensional (3DSTE) speckle-tracking echocardiography in patients with atrial septal defect (ASD) before and after percutaneous trancatheter closure could predict paroxysmal atrial fibrillation (P…

[Atrial tachycardia, atrial flutter, atrial fibrillation: curative therapy by focal ablation in a pulmonary vein].

We describe the case of a 50-year-old woman with the clinical diagnosis of cardiomyopathy associated with supraventricular tachycardia refractory to pharmacological treatment. The totally irregular tachyarrhythmia was the result of different episodes of atrial tachycardia, atrial flutter and atrial fibrillation that could be identified in the surface ECG. These findings and the patient's symptoms were all caused by a single focal tachycardia originating from the left upper pulmonary vein. Ablation of this focus represented a curative antiarrhythmic therapy also restoring a normalized ventricul…

Right atrial isolation associated with atrial septal closure in patients with atrial septal defect and chronic atrial fibrillation.

To restore sinus rhythm in the remaining heart chambers of six adult patients with atrial septal defect and chronic or paroxysmal atrial fibrillation, electrical, right atrial isolation associated with surgical correction of the defect was performed. All but one patient was free from atrial fibrillation without medication 2-25 months after operation. The isolated right atrial appendages showed intrinsic rhythmical activity in five patients and no electrical activity in one. Right atrial isolation is a safe and effective procedure that abolishes atrial fibrillation in patients with arrhythmia a…

Atrial and Ventricular Dyssynchrony, Cardiomyopathy, and Atrial Fibrillation: To Synchronize or to Ablate First?

BACKGROUND: Optimal management in patients presenting with persistent atrial fibrillation, cardiomyopathy, and atrial as well as ventricular dyssynchrony with left bundle branch block is unclear. RESULTS AND METHODS: A 67-year-old female patient presented with persistent atrial fibrillation, cardiomyopathy with a newly reduced ejection fraction (EF), increased P wave duration on electrocardiogram (ECG) and left bundle branch block. After shared decision making, the patient elected to proceed with Bachman bundle/left bundle branch area pacing plus atrioventricular (AV) node ablation. At 6-mont…

The relation between the atrial blood supply and the complexity of acute atrial fibrillation.

BACKGROUND: Patients with a history of myocardial infarction and coronary artery disease (CAD) have a higher risk of developing AF. Conversely, patients with atrial fibrillation (AF) have a higher risk of developing myocardial infarction, suggesting a link in underlying pathophysiology. The aim of this study was to assess whether coronary angiographic parameters are associated with a substrate for AF in patients without a history of AF. METHODS: During cardiac surgery in 62 patients (coronary artery bypass grafting (CABG;n = 47), aortic valve replacement (AVR;n = 9) or CABG + AVR (n = 6)) wit…

Percutaneous Left Atrial Appendage Closure: is there a Role in Valvular Atrial Fibrillation.

Atrial fibrillation, a chronic and highly morbid cardiovascular condition which affects over 33 million people worldwide, can be broadly categorized as valvular vs non-valvular in etiology. However, definitions of valvular atrial fibrillation have varied widely in the literature, and there is no clear consensus definition to date. Historically, patients with atrial fibrillation in the setting of rheumatic mitral valve disease have constituted a particularly high risk group for cardioembolic stroke, and for this reason many contemporary trials of pharmaceutical and device therapies for atrial f…

CTA Detection of Left Atrial Stasis and Thrombus in Patients with Atrial Fibrillation.

BACKGROUND: We investigated computed tomography (CT) angiography (CTA) in assessment of left atrial appendage (LAA) stasis and thrombus in preprocedural evaluation for atrial fibrillation (AF) ablation in a large community cohort. METHODS AND RESULTS: We reviewed CTA and transesophageal echocardiographic images obtained in 861 consecutive patients with a history of AF undergoing same-day CTA and transesophageal echocardiogram (TEE) before AF ablation at a single hospital (2006-2013). CTA findings of LAA filling defects from acquisitions without electrocardiogram gating were compared to TEE fe…

Diffuse FDG uptake of the bilateral atrial walls in a patient with atrial fibrillation.

A 59-year-old man, who had a 3-year history of paroxysmal atrial fibrillation, was referred to our hospital because of palpitation for 12 hours. Atrial fibrillation was confirmed by ECG. F-FDG PET/CT scan was performed showing diffuse FDG uptake of the bilateral atrial walls. Enhanced chest CT showed that the bilateral atria and ventricles were normal. This case highlights that atrial fibrillation should be included in the differential diagnosis of abnormal atrial FDG accumulation along with tumor and inflammatory processes.

The influence of atrial and ventricular pacing on the incidence of atrial fibrillation: a meta-analysis.

BACKGROUND: The effect of atrial pacing on the incidence of atrial fibrillation (AF) is unknown. Furthermore, the threshold of ventricular pacing that is associated with a higher incidence of AF has yet to be determined. Thus, we set out to determine the optimal pacing modality in patients with sinus node dysfunction (SND) for the prevention of AF. METHODS: Individual patient data from four contemporary pacemaker studies were gathered and analyzed. Since AF would inherently lead to a reduction in atrial pacing, percent atrial and ventricular pacing (%AP and %VP) were determined at the first f…

Atrial fibrosis and atrial fibrillation: the role of the TGF-β1 signaling pathway.

BACKGROUND: Atrial fibrosis concurs with chronic atrial fibrillation (AF), a phenomenon that contributes to the resistance to restore and maintain sinus rhythm (SR). Fibrogenesis represents a complex process in which the transforming growth factor-β1 (TGF-β1) pathway may play a major role, e.g. in the setting of myocardial infarction. The present study addresses the potential contribution of the TGF-β1 signaling pathway to atrial fibrosis in patients with AF. METHODS AND RESULTS: Right atrial appendages of 163 patients were excised during heart surgery and grouped according to rhythm (SR vs. …

Increased inflammatory cell infiltration in the atrial myocardium of patients with atrial fibrillation.

Atrial fibrillation (AF) may be caused by structural and electrophysiological changes in the atria induced by inflammation. This study analyzed 35 adult patients with symptomatic severe mitral valve disease and moderate-to-severe tricuspid valve disease and without coronary artery disease who underwent valve operations for congestive heart failure; 18 patients had persistent AF and 17 patients had no history or electrocardiogram examination of AF before surgery. Atrial appendageal tissues obtained during surgery were evaluated for histopathological changes. The number of inflammatory cells ide…

High prevalence of paroxysmal atrial fibrillation and/or atrial flutter in metabolic syndrome.

BACKGROUND: Because metabolic syndrome is associated with cardiovascular diseases, its association with the risk of paroxysmal atrial fibrillation (PAF) and/or atrial flutter (PAFL) was examined in the present study. METHODS AND RESULTS: A prospective analysis was performed in 592 consecutive hospitalized patients without obvious structural heart diseases. Sinus rhythm was confirmed by electrocardiography in all patients. PAF/PAFL occurred in 32 (5%) and metabolic syndrome was present in 127 (21%) of the patients enrolled. PAF/PAFL occurred in 12 (9%) of the patients with metabolic syndrome, …

The role of left atrial muscular bundles in catheter ablation of atrial fibrillation.

OBJECTIVES: We sought to investigate the imaging of the left atrial (LA) muscular bundle and the relationship between the bundle and inducibility of tachyarrhythmia after pulmonary vein isolation (PVI). BACKGROUND: Noninducibility is used as a clinical end point of atrial substrate ablation after PVI. However, little is known about the role of the LA muscular bundles in tachyarrhythmia after PVI. METHODS: Forty-three consecutive patients with paroxysmal atrial fibrillation who underwent catheter ablation were included. Bi-atrial isochronal mapping was performed with the NavX system (St. Jude…

Left atrial enlargement as a predictor of recurrences in lone paroxysmal atrial fibrillation.

BACKGROUND: A mild increase in left atrial (LA) size predicts arrhythmia onset and adverse events in patients with lone paroxysmal atrial fibrillation (LPAF). However, the role of LA size as a predictor of LPAF recurrences is still controversial. OBJECTIVE: The potential role of LA size in affecting the frequency of recurrent episodes in patients with LPAF was investigated. METHODS: Fifty-one patients who were admitted for a first episode of LPAF and presenting with one recurrence (group A, n=20), two or three recurrences (group B, n=18), or four or more recurrences (group C, n=13) during an…

Impairment of the atrial substrates by chronic cigarette smoking in patients with atrial fibrillation.

BACKGROUND: Smoking is a major risk factor for cardiovascular disease. The impact of smoking on the right and left atrial substrates is unknown. METHODS: This study included 88 patients (age 50 +/- 13 years, M/F = 71/17) who underwent four pulmonary veins isolation (PVI) for paroxysmal atrial fibrillation (AF). The mean voltage and total activation time of the individual atria were obtained by using a NavX mapping system, and were compared between the patients with and without a previous smoking history. The dose effect was evaluated by the smoking intensity-duration, defined as the number of…

The role of premature atrial contractions as the main triggers of postoperative atrial fibrillation.

UNLABELLED: To better understand the pathogenesis of postoperative atrial fibrillation (AF), the mode of onset of AF after coronary artery bypass grafting was analyzed with respect to the autonomic balance, the heart rate (HR), and the presence of arrhythmias preceding the onset of sustained AF. METHOD: Holter recordings of 24 hours, obtained from the first postoperative morning until clinically documented sustained AF, were analyzed in 29 untreated patients and in 13 patients treated with thoracic epidural anesthesia (TEA), who all developed AF after coronary artery bypass grafting. The pres…

The effects of aging on atrial endocardial electrograms in patients with paroxysmal atrial fibrillation.

BACKGROUND: The prevalence of atrial fibrillation (AF) has been reported to increase with advancing age. Histologic studies in AF have demonstrated that the percentage of fibrosis and degenerative changes in the atrial muscle increase significantly with age. HYPOTHESIS: This study was undertaken to assess the influence of advancing age on atrial endocardial electrograms recorded during sinus rhythm in patients with paroxysmal atrial fibrillation (PAF), which had not been assessed previously. METHODS: Right atrial endocardial catheter mapping during sinus rhythm was performed in 111 patients …
