What is the fracture displacement influence to fracture non-union in intramedullary nail treatment in subtrochanteric fracture?

INTRODUCTION: The fracture displacement in intramedullary nail in femoral subtrochanteric fracture may cause fracture non-union. We retrospectively analysed our recent experience to clear the influence about fracture displacement in intramedullary nail in femoral subtrochanteric fracture. MATERIALS AND METHODS: This study includes 36 patients in the intramedullary nail group followed up for more than 12 months; these patients suffered from femoral subtrochanteric fracture from 2009.1-2014.12 in our hospital. The operation time, amount of bleeding, length of hospital stay, fracture healing tim

MRI of squamous cell carcinoma of the nail apparatus: report of 6 cases.

Malignant tumors of the nail apparatus are rare and dominated by squamous cell carcinomas (SCC). Routinely, their pre-therapy imaging is limited to radiography. Our purpose is to determine the MRI characteristics in the locoregional assessment of SCC of the nail apparatus through a series of 6 consecutive cases explored by MRI and operated, carried out over a period of 12 years. IRB approval was obtained. Two in situ and 4 invasive squamous cell carcinomas were found, sex ratio was 0.5, and the age was 55 ± 10 years (mean ± SD). Most tumors showed specific signal behavior different from that o

Finite element analysis of distraction osteogenesis with a new extramedullary internal distractor.

Distraction osteogenesis (DO) is a bone regenerative maneuver, which is conventionally done with external fixators and, more recently, with telescopic intramedullary nails. Despite the proven effectiveness, external approaches are intrusive to the patient's life while intramedullary nailing damages the growth plates, making them unsuitable for pediatric patients. An internal DO plate fixator (IDOPF) was developed for pediatric patients to address these limitations. The objective of this study was to test the hypothesis that the IDOPF can withstand a partial weight bearing scenario and create a

Biological enhancement of tibial diaphyseal aseptic non-unions: the efficacy of autologous bone grafting, BMPs and reaming by-products.

The mandatory stimulus that can optimise the healing pathway can be electrical, mechanical, biological, or a combination of all these parameters. A variety of means has been utilised for biological enhancement, including extracorporeal shock wave, electrical, ultrasound stimulation, the reaming technique of IM nailing, bone graft substitutes, osteogenic cells and bioactive molecules produced by tissue engineering techniques. The aim of this study is to present a review of the existing evidence for the efficacy of reaming, autologous bone grafting and the commercially available growth factors (

[Genetic basis for skeletal disease. Orthopaedic intervention for bone dysplasias].

The aims of orthopaedic intervention for bone dysplasias are to maintain joint function, to prevent neurological complications, and then to improve the patient's quality of life. Bone lengthening technique can not only increase the body height, but also correct malalignment of the limb at the same treatment session, which is important to prevent early occurrence of the degenerative osteoarthritis. Intramedullary nailing for osteogenesis imperfecta protects long bones from fractures and prevents malunion when fracture takes place. Spinal instrumentation surgery is effective for prevention of th

Acral dysostosis dyserythropoiesis syndrome.

UNLABELLED: The term congenital dyserythropoietic anaemia (CDA) designates a group of rare but well defined erythrocytic disorders. Type I is defined by macrocytosis and megaloblastic changes of the bone marrow cells. Two unrelated children with CDA are described with associated defects: absence of nails and short or absent phalanges, polysyndactyly of the fourth metacarpal. One of them had also areas of depigmentation. CONCLUSION: The association of congenital dyserythropoietic anaemia with morphological defects of hands and feet is suggested to constitute a new syndrome caused by a single m

[Pseudofluorosis: psychologic aspects of environmental pollution].

A middle-aged woman who had been living in close surroundings of a fluorine-emitting factory for many years, was examined because she claimed to be suffering from skeletal fluorosis. Radiological, histological and laboratory findings did not give evidence of fluorosis. Fluoride concentrations were normal in samples of blood, bone, pooled urine, nails and hair. Even this proof to the contrary did not shake her conviction of many years that she was suffering from fluorosis. Thus, psychological aspects have to be considered in the assessment of health implications of pollutants.

Dyskeratosis congenita: delay in diagnosis and successful treatment of pancytopenia by bone marrow transplantation.

Dyskeratosis congenita is an inherited disorder characterized by nail dystrophy, skin pigmentary changes, mucosal leukoplakia, pancytopenia and an increased incidence of malignancy. Because of a widely held view that the outcome of bone marrow transplantation in dyskeratosis congenita is poor, this treatment option is sometimes not considered when pancytopenia develops. We present a child currently doing well 3 years after bone marrow transplantation, and review the literature.

Comparative study of a novel proximal femoral bionic nail and three conventional cephalomedullary nails for reverse obliquity intertrochanteric fractures: a finite element analysis.

PURPOSE: Conventional cephalomedullary nails (CMNs) are commonly employed for internal fixation in the treatment of reverse obliquity intertrochanteric (ROI) fractures. However, the limited effectiveness of conventional CMNs in addressing ROI fractures results in significant implant-related complications. To address challenges associated with internal fixation, a novel Proximal Femoral Bionic Nail (PFBN) has been developed. METHODS: In this study, a finite element model was constructed using a normal femoral specimen, and biomechanical verification was conducted using the GOM non-contact opti

Tibial lengthening and concomitant foot deformity correction in 14 patients with permanent deformity after poliomyelitis.

BACKGROUND: In neuromuscular diseases, limb lengthening and foot deformity correction are associated with a high risk of complications associated with distraction callus and joint contracture. We have found no published articles of tibial lengthening and concomitant foot deformity correction using the Ilizarov method or traditional methods. To compare result of gradual distraction with triple arthrodesis for foot deformity combined with tibial lengthening, we investigated healing index and complications of two methods. PATIENTS AND METHODS: We reviewed 14 patients with permanent deformity aft

Brachyonychia Associated with Acroosteolysis in Chronic Kidney Disease: How Phalange Shape Influences Nail Morphology.

Brachyonychia is a rare manifestation in patients with chronic kidney disease. Longtime disease, secondary hyperparathyroidism, and hemodialysis are common conditions among those who present it. We evaluated 8 cases who presented brachyonychia in the nephrology department and compared the clinical versus the radiographic findings, and evaluated how the tissue adjusts to the underlying bone structure, giving different forms to the nails. We conclude that brachyonychia and acroosteolysis in chronic kidney disease suggest long-term disease, secondary hyperparathyroidism, and hemodialysis, besides

Staphylococcus caprae: A Skin Commensal with Pathogenic Potential.

Staphylococcus caprae (S. caprae) is a catalase-positive, coagulase-negative organism that was first isolated from goat milk, and was later found to colonize healthy human skin, nails, and nasal mucosa. Rarely, this commensal organism can become pathogenic in humans. S. caprae has been implicated in a variety of human infections, with the highest incidence being in bone and joint infections. We describe a man who, after receiving facet joint injections for back pain, developed native vertebral discitis, vertebral osteomyelitis with phlegmon, and bilateral psoas abscesses, from which S. caprae

[Multiple exostoses. Review of the literature. Apropos of a case disclosed during an injury].

The authors report a case of bilateral fracture of the thigh bone that occurred in a 22 year old man with multiple exostosis. Clinical deformities, radiology and histology confirmed the diagnosis. Traumatic fractures were caused by a road accident. The treatment, because of deformities, consisted of an open fire synthesis of the both thigh bones with Küntscher's nails. The post-operative evolution was marked by rapid healing of fractures. The scarcity of this genetic affection and the symmetrical lesions have lead the authors to conduct a literature review.
