Guidance for dysmorphic sacrum fixation with upper sacroiliac screw based on imaging anatomy study: techniques and indications.

OBJECTIVE: This study aimed to investigate the techniques and indications of upper sacroiliac screw fixation for the dysmorphic sacrum. METHODS: The dysmorphic sacra were selected from 267 three-dimensional pelvic models. The dysmorphic sacra which couldn't accommodate a 7.3 mm upper trans ilio-sacroiliac screw were classified as the main dysmorphic sacra. Then, the size of the bone corridor, the length of the screw in the corridor, and the orientation of the screw were measured. The insertion point on the sacrum was identified by two bone landmarks. RESULTS: totally, 30.3% of sacra were ide

Compressive fixation of osteochondritis dissecans fragments with Herbert screws.

Current methods of fixation of femoral osteochondritic fragments do not provide rigid enough stabilization to allow early postoperative motion. We describe a technique using Herbert screws in a patient to secure a large unstable medial condylar fragment. A second procedure for removal of the screws was not required, and the patient had an excellent clinical result. The Herbert differential pitch bone screw is able to provide compressive fixation, allow early motion, and minimize the potential for complications.

Three-dimensional finite-element analysis of a pedicle screw system combined with a titanium mesh support and fixation in the treatment of L4-5 vertebral tuberculosis.

This study aimed to assess the biomechanical stability of pedicle screws combined with titanium mesh bone grafting for spinal reconstruction and fixation during surgery for lumbar spine (L4-L5) tuberculosis using the finite-element method. A model of the L1-sacral segment of the human body was developed using the finite-element method. After verifying its effectiveness, a postoperative model was developed for treating vertebral tuberculosis at the L4-L5 level, which involved implanting an interbody titanium mesh and securing it with pedicle screws. Subsequently, the internal fixation materials

Stability of bioresorbable plates following reduction of mandibular body fracture: Three-dimensional analysis.

PURPOSE: The recent development of bioresorbable bone plates and screws allows plates to be applied to the load-bearing regions of the mandible and to remain in place over time without the need for removal. We hypothesized that the stability of composite plates and screws forged from unsintered hydroxyapatite particles and poly-l-lactide (u-HA/PLLA) is comparable to that of standard titanium fixation systems for the reduction of fractures of load-bearing regions of the mandibular body. MATERIALS AND METHODS: 40 patients underwent open reduction and internal fixation of the fractured mandibula

Radiolucency around highly porous sockets and hydroxyapatite-coated porous sockets in total hip arthroplasty for hip dysplasia.

Studies over the past decade have reported that the use of highly porous sockets in total hip arthroplasty (THA) results in osseointegration and long-term implant stability. However, some reports have raised concerns regarding radiographic evidence of poor osseointegration with features of fibrous tissue ingrowth. The purpose of this study was to compare clinical and radiographic assessments of highly porous sockets with those of hydroxyapatite (HA)-coated porous sockets in THA for hip dysplasia (DDH) at least 1 year after surgery. A total of 127 patients (136 hips) were recruited for the stud

The Scaphoid Safe Zone: A Radiographic Simulation Study to Prevent Cortical Perforation Arising from Different Views.

PURPOSE: The purpose of this study was to simulate and calculate the probability of iatrogenic perforation of the scaphoid cortical bone when internal fixation appeared to be safe on radiographs. The results will assist surgeons in determining proper screw placement. METHODS: Thirty scaphoids were reconstructed using computed tomography data and image-processing software. Different central axes were determined by the software to simulate the surgical views. The safe zone (SZ) and risk zone (RZ) were identified on the axial projection radiographs by comparing the scaphoid bone stenosis measure

Incidence and risk factors for aseptic baseplate loosening of reverse total shoulder arthroplasty.

BACKGROUND: Aseptic glenoid baseplate loosening (AGBL) is a catastrophic complication after reverse total shoulder arthroplasty (RTSA). Our goals were to determine the incidence of AGBL in patients who underwent RTSA and identify risk factors for AGBL after RTSA. METHODS: We analyzed 202 shoulders that underwent primary or revision RTSA using 1 implant system and evaluated baseplate loosening at a minimum 2-year follow-up. The associations between AGBL and the following variables were investigated: patient age, sex, primary vs. revision RTSA, scapular notching, use of bone graft, and type of

Comparison of femoral mechanics before and after internal fixation removal and the effect of sclerosis on femoral stress: a finite element analysis.

BACKGROUND: Femoral neck fractures are a common traumatic injury. The removal of the internal fixation remains controversial, especially in terms of mechanical stability. Moreover, collapsed necrosis of the femoral head continues to occur after fracture healing. We believe that sclerotic cancellous bone (SCB) formation around the screw is associated with femoral head necrosis. We aimed to compare mechanical features before and after implant removal and determine the effect of SCB formation on stress distribution. METHODS: Cylindrical cancellous bone sections were collected from a relatively n

Effective Treatment for Breakage of a Solid Long Cementless Stem with a Distal Locking Screw: A Case Report.

INTRODUCTION: Fractures around the femoral stem are difficult complications to treat. In revision surgery, good results can be obtained by a long cementless stem with a distal locking screw. However, there are few reports of treatment for cases where the stem itself is broken. CASE REPORT: The case of a 69-year-old woman in which the femoral stem that was broken 5 years after performing revision surgery for a right periprosthetic femoral fracture was repaired using a solid long stem with a distal locking screw is presented. Her past history included osteoporosis and rheumatoid arthritis. She

ORIF Patella Fracture With a Tension Band Construct.

Patellar fractures can occur as a result of direct or indirect injuries. Indirect injury is more common and occurs when the patella fails in tension when the force exerted by the extensor mechanism exceeds that of the bone. Common mechanism of indirect injury is rapid knee flexion with fully contracted quadriceps muscle. Treatment with a tension band construct is indicated for transverse patella fractures without significant comminution. This can be performed with 18-gauge wire and either 2 parallel, longitudinally oriented cannulated screws or 2 Kirschner wires. Biomechanical studies have sho

Development of a Synovial Fistula Through a Screw Hole Following Supramalleolar Osteotomy Hardware Removal: A Case Report.

A synovial fistula is the communication between the synovial space and the skin. In most cases, the fistula tract is located within the soft tissue; therefore, excision and closure of the fistula have been described as surgical treatment. Rarely, fistulas may form within the bone following procedures around the joint, such as core biopsy and bone tunneling for ligament reconstruction. In such cases, the insertion of materials filling the bone tunnel with cement or bone graft was introduced. This report describes a case of synovial fistula in the distal tibiofibular joint through a screw hole f

Comparison between two pedicle screw augmentation instrumentations in adult degenerative scoliosis with osteoporosis.

BACKGROUND: The operative treatment of adult degenerative scoliosis combined with osteoporosis increase following the epidemiological development. Studies have confirmed that screws in osteoporotic spines have significant lower-screw strength with more frequent screw movements within the vertebra than normal spines. Screws augmented with polymethylmethacrylate (PMMA) or with autogenous bone can offer more powerful corrective force and significant advantages. METHODS: A retrospective analysis was conducted on 31 consecutive patients with degenerative lumbar scoliosis combined with osteoporosis

A finite element analysis and cyclic load experiment on an additional transcortical-type hole formed around the proximal femoral nail system's distal locking screw.

BACKGROUND: A complication associated with the distal locking screw used in the proximal femoral nail (PFN) system is the formation of accidental additional holes. We hypothesized that an increase in stress around additional holes is a relevant factor contributing to fractures. This study aimed to evaluate stress changes in the cortical bone around additional screw holes using finite element analysis. METHODS: Proximal femoral nail antirotation (PFNA)-II (Synthes, Solothurn, Switzerland) was inserted into a femur model. An additional 4.9-mm transcortical hole was made either anteriorly (anter
