A case of brain abscess caused by Propionibacterium acnes 13 months after neurosurgery and confirmed by 16S rRNA gene sequencing.

Propionibacterium acnes is a gram-positive anaerobic bacillus and a normal inhabitant of the skin. Although it is often considered a contaminant of blood cultures, it can occasionally cause serious infections, including postoperative central nervous system infections. Here, we report the case of a 70-yr-old man who developed a large cerebral abscess caused by P. acnes 13 months after neurosurgery. Immediate gram staining of the pus from his brain revealed the presence of gram-positive coccobacilli. However, colony growth was observed only after 5 days of culture. Therefore, we performed 16S rR

Primary determinants of ischaemic stroke/brain abscess risks are independent of severity of pulmonary arteriovenous malformations in hereditary haemorrhagic telangiectasia.

BACKGROUND: Brain abscesses and ischaemic strokes complicate pulmonary arteriovenous malformations (PAVMs). At risk individuals are poorly recognised. Stroke/abscess risk factors have not been defined. METHODS: A cohort study of 323 consecutive individuals with PAVMs (n = 219) and/or the commonly associated condition hereditary haemorrhagic telangiectasia (HHT, n = 305) was performed. Most of the 201 individuals with PAVMs and HHT had no respiratory symptoms, and were unaware they had HHT. Anderson-Gill models assessed constant and time dependent potential predictive variables for stroke/absc…

Intracranial Salmonella infections: meningitis, subdural collections and brain abscess. A series of six surgically managed cases with follow-up results.

Focal intracranial infections due to Salmonella are rare. So far, around 80 cases have been reported in the world literature. The authors present their experience of 6 cases of intracranial Salmonella infections, mainly subdural empyema in 5 and effusion in 1. In 1 case, subdural empyema was bilateral, and in another case, there was an associated brain abscess. Positive blood cultures and positive Widal tests were noticed in 2 patients each. Early diagnosis and prompt evacuation of subdural collections and brain abscess and antibiotic therapy lead to satisfactory results. This study suggests t…

Neonatal stress hyperglycemia mimicking diabetes mellitus secondary to massive intraparenchymal brain abscess due to escherichia coli sepsis: A case report.

Stress hyperglycemia is a common metabolic response in critically ill neonates but may rarely present with extreme hyperglycemia mimicking neonatal diabetes mellitus. We report a term female neonate presenting on day 18 of life with lethargy, poor feeding, dehydration, and severe hyperglycemia (>1000 mg/dL), hypernatremia, and acute kidney injury. Insulin therapy was initiated due to suspicion of neonatal diabetes mellitus. Blood cultures grew Escherichia coli, confirming neonatal sepsis. Neuroimaging revealed a massive right fronto-parieto-temporal intraparenchymal abscess with significant ma

Dual RNA-seq reveals the complement protein C3-mediated host-pathogen interaction in the brain abscess caused by Staphylococcus aureus.

This study aimed to elucidate the complement protein C3-mediated host-pathogen interaction in the brain abscess caused by Staphylococcus aureus infection. Dual RNA-seq was employed to analyze the transcriptomic differences between C3 deficiency and wild-type mice of S. aureus-induced brain abscess model, and then we investigated the potential regulatory pathways of S. aureus-host interaction mediated by C3 and S. aureus genes associated with the pathogenesis of brain abscess. Finally, C3 deficient-mice and hla mutants of S. aureus were used to verify the specific pathogen-host interaction. In …

Predictors of unprovoked seizures in surgically treated pyogenic brain abscess: Does perioperative adjunctive use of steroids has any protective effect?

OBJECTIVE: Though outcome of brain abscess has been improved due to stereotactic drainage and appropriate antibiotic treatment, late unprovoked seizure often occurs after the abscess is resolved. The purpose of this study is to reveal the factors related to the late unprovoked seizure and effect of steroid on the prevention of seizure. PATIENTS AND METHODS: From January 2002 to August 2016, 119 patients with supratentorial brain abscesses were retrospectively analyzed. Initial volume of abscess, surgical methods, use of antiepileptic drugs (AEDs), and seizure free survival according to the us…

Brain abscess and glioblastoma identified by combined proton magnetic resonance spectroscopy and diffusion-weighted magnetic resonance imaging--two case reports.

The differential diagnosis between brain abscesses and necrotic tumors such as glioblastomas is sometimes difficult to establish by conventional computed tomography and magnetic resonance imaging. Combined proton magnetic resonance spectroscopy (1H-MRS) and diffusion-weighted magnetic resonance imaging (DWI) were used to establish the preoperative diagnosis of brain abscess and glioblastoma. DWI visualized the brain abscess as a homogeneous hyperintense lesion and 1H-MRS revealed the presence of acetate, lactate, and amino acids and the absence of the normal brain components. DWI sometimes sho…

Cardiogenic brain abscesses in children: the recurrence paradox in a 24-year institutional experience with 571.9 person-years of follow-up.

PURPOSE: Cardiogenic brain abscesses (CBA) are rare infections that primarily affect children with cyanotic congenital heart disease (CCHD). Despite longstanding clinical awareness, gaps remain in our understanding of long-term outcomes and the actual recurrence risk. This study analyzed the clinical profile, management, and long-term outcomes of a large institutional cohort, with an emphasis on quantifying the recurrence risk. METHODS: We retrospectively reviewed the records of 34 pediatric patients (aged < 15 years at index abscess diagnosis) who underwent CBA management at our institution …

Therapeutic Drug Monitoring-Guided Linezolid Therapy for the Treatment of Multiple Staphylococcal Brain Abscesses in a 3-Month-Old Infant.

Brain abscesses are invasive infections of the central nervous system with a high level of treatment complexity especially in pediatric patients. Here, we describe a 3-month-old infant with multiple brain abscesses caused by methicillin-susceptible Staphylococcus aureus (MSSA). The patient was initially treated with empirical antibiotics (ceftriaxone, metronidazole, vancomycin). Upon MSSA identification, therapy was optimized by switching vancomycin to linezolid to improve tissue penetration. Therapeutic drug monitoring (TDM) was performed to check linezolid levels in the plasma and pus of the…

Bacillus Cereus bacteremia complicated by brain abscess in a severely immunocompromised patient: Addressing importance of early recognition and challenges in diagnosis.

Bacillus cereus (B. cereus) is a known cause of a food poisoning in the general population. However, it can cause life-threatening sepsis and shock in severely immunocompromised patients with hematologic malignancies, which frequently lead to central nervous system (CNS) infections associated with high mortality and morbidity. In this case report, we describe a patient with a newly diagnosed acute myeloid leukemia that underwent induction chemotherapy and developed B. cereus infection that was associated with septic shock and brain abscesses. Definitive diagnosis of multiple brain abscesses wa…

Brain Abscesses Caused by Nocardia farcinica in a 44-Year Old Woman with Multiple Myeloma: A Rare Case and Review of the Literature.

BACKGROUND Central nervous system infection by the Nocardia species is associated with high morbidity and mortality. Its occurrence in patients with multiple myeloma is rare and acquisition of the infection in such patients was associated with the use of novel therapeutic agents (eg, bortezomib and lenalidomide) or bone marrow transplantation. Here, we report the first case of Nocardia brain abscesses in a patient with multiple myeloma, without the above risk factors. CASE REPORT A 44-year-old woman with IgG-kappa type multiple myeloma presented with generalized tonic-clonic seizures. Magnetic…
