Increased usage of the high flow nasal cannula in COVID-19 cases in Japan -from the online questionnaire survey by the Japanese Respiratory Society.

An online questionnaire survey was used to assess the application of noninvasive strategies in the management of COVID-19 patients. We found a marked increase in the proportion of facilities using high flow nasal cannula (HFNC) oxygen therapy successfully in the 3rd epidemic wave as compared to the 1st wave (49% vs. 12%) and rare reports of associated nosocomial infection in the medical personnel. Furthermore, the proportion of facilities using HFNC as first-line therapy for half or more than half of their patients who did not respond to conventional oxygen therapy increased from 9% in the fir…

Using side-opening injection cannulas to prevent cement leakage in percutaneous vertebroplasty for osteoporotic vertebral compression fractures, does it really work?

BACKGROUND: Percutaneous vertebroplasty has been widely applied in the treatment of osteoporotic vertebral compression fractures over the past two decades. However as one of the major complications, the rate of cement leakage seems not to be decreased significantly. In this study, the rate of cement leakage was compared between two groups using two different cement injection cannulas. The purpose was to determine the efficacy of side-opening cannula on preventing cement leakage in vertebroplasty for the treatment of osteoporotic vertebral compression fractures. METHODS: A retrospective study …

Comparison of four different pediatric 10F aortic cannulae during pulsatile versus nonpulsatile perfusion in a simulated neonatal model of cardiopulmonary bypass.

We compared four commercially available 10F pediatric aortic cannulae with different geometric designs (DLP-Long tip, DLP-Short tip, RMI-Long tip, and Surgimedics-Short tip) during pulsatile versus nonpulsatile perfusion in terms of pressure drops and surplus hemodynamic energy (SHE) levels in an in vitro neonatal model of cardiopulmonary bypass. The pseudo patient was subjected to seven pump flow rates at 100 ml/min increments in the 400-1,000 ml/min range. A total of 44 experiments (n = 22, nonpulsatile; n = 22, pulsatile) were performed at each of the seven flow rates. Surgimedics had signi…

Rapid and Efficient Aerosol Delivery During 60 L/min High Flow Nasal Cannula Therapy-In Vitro Development of a Novel Dry Powder Delivery Platform.

High flow therapy (HFT) delivers heated and humidified gas at flow rates up to 60 L/min to hypoxemic subjects, but is not conducive to simultaneous administration of pharmaceutical aerosols. Aerosol losses occur due to circuit wall impaction, environmental loss and particle growth from high humidity; resulting in poor lung delivery efficiency. This study compares two strategies for delivering dry powder aerosols during 60 L/min humidified HFT: a circuit connector (HFT-CC) which integrates directly into the standard HFT flow pathway, and an interface connector (HFT-IC) designed to bypass delive

Efficacy and safety of nasal mask compared to nasal cannula during procedural sedation in gastroscopy: a systematic review, meta-analysis, and trial sequential analysis.

BACKGROUND: Hypoxemia is the most common adverse event during propofol-based procedural sedation for gastroscopy. Conventional nasal cannula oxygenation provides low flow rates, permitting room air entrainment and often proving insufficient. Nasal mask oxygenation delivers higher inspired oxygen concentrations, potentially enhancing respiratory safety during sedation. OBJECTIVE: This study compared the efficacy and safety of nasal mask versus nasal cannula oxygen delivery during propofol-based procedural sedation for gastroscopy. METHODS: Electronic databases/registries were systematically s…

High-flow nasal cannula oxygen therapy versus noninvasive ventilation for elderly chronic obstructive pulmonary disease patients after extubation: a noninferior randomized controlled trial protocol.

INTRODUCTION: Noninvasive ventilation (NIV) is widely used for sequential extubation in patients with chronic obstructive pulmonary disease (COPD). However, NIV may cause many adverse events such as claustrophobia, facial skin compression, air leakage, bloating, and even reflux aspiration, resulting in poor patient compliance/tolerance and high failure rate, especially for older adults who are at high risk of communication difficulties and consciousness disorder. High-flow nasal cannula (HFNC) oxygen therapy is a new alternative support to NIV, but whether it can effectively reduce the rate of…

Respiratory-rate Oxygenation index for Predicting Noninvasive Ventilation Associated With High-flow Nasal Cannula Failure in Acute Respiratory Failure Due to SARS-CoV-2.

PURPOSE: The respiratory rate-oxygenation (ROX) index is used to predict high-flow nasal cannula (HFNC) success in acute respiratory failure, including in Coronavirus disease 2019 (COVID-19) patients. However, no study has described its performance to predict failure of alternating sessions of noninvasive ventilation (NIV) and HFNC in severe COVID-19 patients. MATERIAL AND METHODS: We conducted a monocentric retrospective cohort study. COVID-19 patients admitted in the intensive care unit (ICU) for acute respiratory failure were treated by alternating sessions of HFNC and NIV. The primary end…

Use of high flow nasal cannula in critical burn patient during deep sedation in enzymatic bromelain debridement (nexobrid®): a single center brief report.

The use of new oxygen supports associated to non-invasive respiratory strategies is well-established in clinical practice, especially after its extensive application in the management of Covid-19 respiratory failure. The use of high flow nasal cannula (HFNC) in patients undergoing procedural sedation and analgesia (PSA) is dramatically increasing. Enzymatic debridement in critical burn patients is a painful treatment that requires an optimal burn pain control protocol as well as a deep sedation for the entire duration of the procedure. Both hypnosis and opioid-analgesia may lead to significant…
