[Cardiopulmonary emergencies during pregnancy and the postpartum period].

The number of patients who develop cardiac problems during pregnancy are increasing and represent to date the major cause of maternal death in western countries. Pregnancy induces several changes which together increase the hemodynamic burden on the cardiovascular system and can also cause a prothrombotic state. Hence, latent or apparent cardiac disease can acutely decompensate during pregnancy. From a cardiovascular perspective, pregnancies are most often complicated by acute coronary syndromes, peripartum cardiomyopathy, arrhythmias, or pulmonary embolism. Due to potential fetal harm convent

[Mitral insufficiency in an adult, due to an abnormal left coronary artery arising from the pulmonary trunk: echocardiographic aspects. Apropos of a case].

The case of a 20 year old woman followed up since adolescence for a progressive congestive cardiomyopathy with mitral regurgitation is reported. She was reinvestigated after hospital admission for cardiac decompensation and supraventricular arrhythmias; 2D echocardiography showed signs of severe mitral regurgitation with thickened mitral leaflets prolapsing into a dilated left atrium and, above all, abnormal diastolic wall motion related to restriction of left ventricular filling with no apparent systolic dysfunction : pulsed Doppler studies confirmed the diagnosis of mitral regurgitation but

Non-syndromic cardiac progeria in a patient with the rare pathogenic p.Asp300Asn variant in the LMNA gene.

BACKGROUND: Mutations in LMNA gene, encoding Lamin A/C, cause a diverse array of phenotypes, collectively referred to as laminopathies. The most common manifestation is dilated cardiomyopathy (DCM), occurring in conjunction with variable skeletal muscle involvement but without involvement of the coronary arteries. Much less commonly, LMNA mutations cause progeroid syndromes, whereby an early-onset coronary artery disease (CAD) is the hallmark of the disease. We report a hitherto unreported compound cardiac phenotype, dubbed as "non-syndromic cardiac progeria", in a young patient who carried a

Prognostic Thresholds of Tricuspid Regurgitation in Transthyretin Amyloid Cardiomyopathy.

IMPORTANCE: In transthyretin amyloid cardiomyopathy (ATTR-CM), tricuspid regurgitation (TR) severity may be underestimated by conventional (semi-)quantitative echocardiographic criteria derived from nonamyloid populations, given the restrictive, low-flow hemodynamics characteristic of the disease. OBJECTIVES: To derive and validate disease-specific prognostic, quantitative TR risk thresholds in ATTR-CM and to compare their prognostic performance with current guideline definitions and the Tricuspid Valve Academic Research Consortium (TVARC) 5-grade extension. DESIGN, SETTING, AND PARTICIPANTS

The Incremental Prognostic Value of Incorporating the Triglyceride-Glucose Index into the Traditional Cardiovascular Risk Factors for the Long-term Prognosis in Ischemic Cardiomyopathy Patients with HFpEF following Coronary Artery Bypass Grafting: A Multicenter Cohort Study.

AIM: The triglyceride-glucose (TyG) index, a biomarker commonly used to evaluate metabolic health status, can predict unfavorable outcomes. Thus, we aimed to explore evidence regarding the prognostic value of the TyG index in patients with ischemic cardiomyopathy and heart failure with preserved ejection fraction (HFpEF). METHODS: We enrolled 277 consecutive participants with new-onset ischemic cardiomyopathy and HFpEF who underwent coronary artery bypass grafting (CABG). The primary study endpoint was major adverse cardiovascular events (MACEs), defined as cardiac death, acute myocardial inf

Copper deficiency and heart disease: molecular basis, recent advances and current concepts.

Copper is an essential trace element and has profound influence on cardiac myopathy and heart metabolism. Dietary Cu restriction in rats results in cardiomyopathy, and affects the integrity of the basal lamina of cardiac myocytes and capillaries. Decreased levels of delta subunits of ATP synthetase and nuclear encoded subunits of cytochrome oxidase system have been observed. Alteration in expression of glutathione peroxidase and catalase in heart and liver in Cu deficiency (Cu-) has been noted involving both transcriptional and post transcriptional mechanisms. A short description of two geneti

Effects of Diet Strategy and Nutrients on the Progression and Prevention of Diabetic Cardiomyopathy: A Narrative Review.

Diabetic cardiomyopathy (DCM) has emerged significantly as a prevalent clinical burden since the 19th century. Currently, despite scarce evidence, dietary strategies are emerging as adjunctive measures to treat DCM. Among these dietary strategies, nutrients such as vitamin D, ω-3 PUFAs, zinc, selenium, resveratrol, anthocyanins, and curcumin have been repeatedly associated with attenuating cardiac oxidative stress, inflammation, and fibrosis in rodent models. However, these results remain scarce and inconsistent in humans. On the contrary, caloric restriction frequently improves cardiac energe

Prenatal diagnosis of glucose-6-phosphatase catalytic subunit 3 deficiency (Dursun syndrome) using whole-exome sequencing: A case report of severe fetal cardiomyopathy in a consanguineous family.

Glucose-6-phosphatase catalytic subunit 3 deficiency, also known as Dursun syndrome, is a rare autosomal recessive disorder characterized by severe congenital neutropenia and variable multisystem malformations, particularly affecting the cardiovascular system. Most reported cases have been identified postnatally, following infectious or hematologic complications. Prenatal identification remains exceptionally rare. We describe the case of a fetus from consanguineous parents with a history of multiple neonatal deaths. Serial prenatal imaging demonstrated progressive fetal growth restriction, car

Cleft mitral leaflets and severe mitral regurgitation: Testing the limits of percutaneous mitral valve repair.

Repair of mitral regurgitation (MR) with the MitraClip device (Abbot Vascular, Menlo Park, CA) to treat degenerative MR is associated with improved acute and long-term outcomes. There is an increasing adoption of the device and operators are now testing the limits of the therapy even for unfavorable anatomies. Isolated cleft mitral leaflets are rare but represent a challenge to percutaneous repair. We present two cases of successful repair of severe MR and cleft mitral leaflets. In the first case, a 52-year-old male with a dilated cardiomyopathy and an ejection fraction (EF) of 15% presented i

Dissecting the Cardiac Conduction System: Is It Worthwhile?

BACKGROUND: Pathologic examination of conduction system (CS) is not routinely performed, and histologic changes are mostly reported in forensic practice. METHODS: We studied the value of dissecting the CS in a cohort of pediatric patients with unexplained sudden death or severe, inexplicable arrhythmias. Histopathologic changes present in CS components were recorded and correlated with findings noted in other cardiac structures. RESULTS: Twenty-one subjects (11 unexplained sudden deaths and 10 life-threatening arrhythmias) were identified; 18 (86%) had CS pathologic abnormalities. In 13 pati

Tenascin-C in cardiac disease: a sophisticated controller of inflammation, repair, and fibrosis.

Tenascin-C (TNC) is a large extracellular matrix glycoprotein classified as a matricellular protein that is generally upregulated at high levels during physiological and pathological tissue remodeling and is involved in important biological signaling pathways. In the heart, TNC is transiently expressed at several important steps during embryonic development and is sparsely detected in normal adult heart but is re-expressed in a spatiotemporally restricted manner under pathological conditions associated with inflammation, such as myocardial infarction, hypertensive cardiac fibrosis, myocarditis

The effects of different levels of dietary restriction on non-neoplastic diseases in male Sprague-Dawley rats.

BACKGROUND AND AIMS: The primary purpose of the present study was to investigate the effects of 10, 25, and 40% dietary restriction (DR) on non-neoplastic diseases in rodents at 58 and 110 weeks of age, and to determine whether low-level DR (10 and 25%) can increase the survival rate and decrease variability in chronic bioassay studies. METHODS: Male Sprague-Dawley (SD) rats (NCTR colony) were divided into four nutritional groups, consisting of an ad libitum (AL) group with unlimited access to the NIH-31 diet, and three dietary restricted (DR) groups given the NIH-31 diet reduced in amount by

Subclinical thyroid disorders in patients with dilated cardiomyopathy.

UNLABELLED: Severe thyrotoxicosis can cause irreversible congestive heart failure. To investigate the coincidence of subclinical thyroid disorders and idiopathic dilated cardiomyopathy (IDC) we investigated these patients with respect to their morphological and functional thyroid status. Thyroid sonography as well as thyroid hormone levels were measured in all patients. RESULTS: Sixty-one patients (50 male, 11 female) with chronic stable IDC were included. Two out of 61 patients showed completely normal thyroid morphology and function. The other 59 patients showed either morphological or func

Anaesthetic Management in a Duchennne Muscle Dystrophy Patient for Treatment of Recurrent Pneumothorax.

INTRODUCTION: Duchenne muscular dystrophy (DMD) is an x linked recessive disorder. Long term prognosis is ominous, with development of respiratory distress and cardiomyopathy in advanced stage of the disease and expected death in the teens-to-mid 20s due to respiratory or cardiac failure. Peri-operative management of this patients is challenging due to difficult airway anatomy (macroglossia, limited neck and mandibular mobility). Additionally, they are at risk of developing malignant hyperthermia, rhabdomyolysis and hyperkalemic cardiac arrest when exposed to halogenated inhalational anaesthet

Neddylation Regulates Mitochondrial Dynamics and Turnover in the Adult Heart.

BACKGROUND: Disruption of mitochondrial homeostasis drives cardiomyopathy and heart failure, yet upstream regulatory mechanisms remain poorly defined. Neddylation, a reversible post-translational conjugation of the ubiquitin-like protein NEDD8 by E1/E2/E3 enzymes, is essential for cardiac morphogenesis, but its role in the adult heart is unknown. METHODS: We assessed the relevance of neddylation to human cardiac disease by gene set enrichment analysis of ischemic (ICM) and non-ischemic cardiomyopathy (NICM) datasets and by immunoblotting and qPCR of ventricular tissue from patients with ICM o

Clinical outcomes with vutrisiran in transthyretin amyloidosis: a systematic review and meta-analysis of randomized trials.

Background: Transthyretin amyloidosis (ATTR) is a progressive disease that causes a restrictive cardiomyopathy. Vutrisiran, a subcutaneous RNA interference (RNAi) therapy, is an approved treatment. This systematic review and meta-analysis evaluates its efficacy and safety with respect to transthyretin (TTR) reduction, functional capacity, quality of life, mortality, and adverse events. Methods: We identified 1,032 records, of which three randomized controlled trials-HELIOS-A, HELIOS-B, and a Phase 1 study-comprising 976 participants (508 vutrisiran; 468 comparator) met the inclusion criteria.

Quantitative assessment of diastolic strain in patients with cardiac amyloidosis based on magnetic resonance feature tracking technology.

BACKGROUND: Cardiac amyloidosis (CA) is a rare, infiltrative restrictive cardiomyopathy. Late gadolinium enhancement (LGE) in cardiac magnetic resonance (CMR) imaging is an important basis for the diagnosis of CA. However, in the early stage of the disease, the deposition of pathological proteins is scarce, which may result in negative LGE. This study aims to explore the diagnostic value of diastolic strain (DS) of the myocardium in CA patients with negative LGE. METHODS: A retrospective review was conducted from June 2020 to April 2024, including 41 CA patients and 18 healthy controls. Clini

SGLT2 inhibitors in cardiac amyloidosis: mechanistic insights and clinical perspectives-a narrative review.

BACKGROUND: Cardiac amyloidosis (CA) is an infiltrative cardiomyopathy characterized by progressive extracellular amyloid deposition, leading to restrictive heart failure (HF) and high morbidity. Disease-modifying therapies remain limited, particularly in patients with advanced HF. Sodium-glucose cotransporter-2 (SGLT2) inhibitors, originally developed for diabetes management, have shown consistent cardiovascular and renal benefits in large randomized trials of non-amyloid HF populations. Their potential mechanistic and clinical relevance in CA has recently gained attention, yet evidence remai
