A prospective evaluation of pediatric emergency care during the 1996 Summer Olympic Games in Atlanta, Georgia.

OBJECTIVES: To explore the impact that a temporary influx of millions of people can make on the local pediatric emergent and urgent care systems. The spectrum of illness was also explored. DESIGN/METHODS: Prospective cohort of patients from outside the usual catchment area presenting at two children's emergency departments and their satellite urgent care centers during the 1996 Summer Olympics. A 13-point survey was completed on each which included general demographics, transportation, language, time in the area, chief complaint, past medical conditions, diagnosis, and medical complications o

Bioaccumulation of Organochlorine Pesticides, Procamallanus sp. (Baylis, 1923) infections, and Microbial Colonization in African Snakehead fish Sampled from Lekki Lagoon, Lagos, Nigeria.

The exponential rise in the Nigerian population has necessitated the use of agrochemicals for enhanced agricultural yields to meet the ever-rising demand for food. However, agrochemicals such as organochlorine pesticides (OCPs) have caused several devastating health and ecological challenges. The study was therefore aimed at assessing the bioaccumulation of OCPs and the associated parasitological and microbial susceptibility in P. obscura to determine the possible ecological impacts of the chemical. A total of 106 specimens of Parachanna obscura fish species were sampled between July and Novem

Rates and risk factors for preterm birth and low birthweight in the global network sites in six low- and low middle-income countries.

BACKGROUND: Preterm birth continues to be a major public health problem contributing to 75% of the neonatal mortality worldwide. Low birth weight (LBW) is an important but imperfect surrogate for prematurity when accurate assessment of gestational age is not possible. While there is overlap between preterm birth and LBW newborns, those that are both premature and LBW are at the highest risk of adverse neonatal outcomes. Understanding the epidemiology of preterm birth and LBW is important for prevention and improved care for at risk newborns, but in many countries, data are sparse and incomplet

The prevalence and disability of bipolar spectrum disorders in the US population: re-analysis of the ECA database taking into account subthreshold cases.

BACKGROUND: Despite emerging international consensus on the high prevalence of the bipolar spectrum in both clinical and community samples, many skeptics contend that narrowly defined bipolar disorder with a lifetime rate of about 1% represents a more accurate estimate of prevalence. This may in part be due to the fact that higher figures proposed for the bipolar spectrum (5-8%) have not been based on national data and have not included all levels of manic symptom severity. In the present secondary analyses of the US National Epidemiological Catchment Area (ECA) database, we provide further cl

Veterans' views on alcohol use and experience of alcohol withdrawal syndrome treatment and subsequent alcohol use disorder care at two highly rural-serving VA hospitals: a qualitative study.

Alcohol use disorder (AUD) is a significant cause of morbidity that disproportionately affects veterans and rural-serving hospitals. As part of a larger quality improvement project to improve care for veterans with AUD admitted to rural-serving Veterans Affairs (VA) hospitals, we conducted a qualitative study using a phenomenological research approach to incorporate the veteran perspective. Participants were n = 16 veterans with AUD and recent admissions to two highly rural-serving hospitals. Veteran views and experience were elicited by conducting 30-60 min sessions of semi-structured intervi

Group-based trajectory modeling to describe the geographical distribution of tuberculosis notifications.

BACKGROUND: Tuberculosis (TB) is a major public health problem, and understanding the geographic distribution of the disease is critical in planning and evaluating intervention strategies. This manuscript illustrates the application of Group-Based Trajectory Modeling (GBTM), a statistical method that analyzes the evolution of an outcome over time to identify groups with similar trajectories. Specifically, we apply GBTM to identify the evolution of the number of TB notifications over time across various geographic locations, aiming to identify groups of locations with similar trajectories. Socioeconomic drivers of vaccine uptake: An analysis of the data of a geographically defined cluster randomized cholera vaccine trial in Bangladesh.

BACKGROUND: Evaluations of oral cholera vaccines (OCVs) have demonstrated their effectiveness in diverse settings. However, low vaccine uptake in some settings reduces the opportunity for prevention. This paper identifies the socioeconomic factors associated with vaccine uptake in a mass vaccination program. METHODS: This was a three-arm (vaccine, vaccine plus behavioral change, and non-intervention) cluster randomized trial conducted in Dhaka, Bangladesh. Socio-demographic and vaccination data were collected from 268,896 participants. A geographical information system (GIS) was used to desig

Biological and psychosocial risk factors for psychotic major depression.

AIMS: Few studies have investigated risk factors for psychotic major depression (PMD). We aimed to investigate the biological and psychosocial risk factors associated with PMD compared with other psychotic disorders. METHODS: Based on the aetiology and ethnicity in schizophrenia and other psychoses (ÆSOP) study, we used a case-control study to identify and recruit, at baseline and 10-year follow-up, all first episode cases of psychosis, presenting for the first time to specialist mental health services in defined catchment areas in the UK. Population-based controls were recruited from the sam

Access of Medicaid recipients to outpatient care.

BACKGROUND: Visits to the emergency department by Medicaid recipients for nonemergency problems are common and contribute to rising health care costs. However, such patients may have few alternatives. We conducted a telephone survey of 953 ambulatory care sites in 10 cities to determine the availability of appointments for Medicaid recipients with common problems. METHODS: Research assistants telephoned all ambulatory care clinics and a stratified sample of private primary care practices in the catchment area served by the hospital emergency department in each city. The assistants identified

Patient Portal Activation Disparities by Ward, Census Tract, and Zip Code in an Urban Landscape Among Neurology Patients: Cross-Sectional Multi-Scale Analysis.

BACKGROUND: Patient portals are essential infrastructure, reinforced by the 21st Century Cures Act, yet adoption remains inequitable. The COVID-19 pandemic accelerated portal adoption as telehealth and remote result delivery made electronic access integral to care, but racial and ethnic disparities persisted. Understanding activation determinants is critical for addressing digital health disparities, particularly among neurology patients, for whom cognitive, speech, and mobility impairments can complicate portal use. OBJECTIVE: We examined the demographic, geographic, and neighborhood-level f

Azithromycin mass drug administration to reduce child mortality in Niger (AVENIR II): a master protocol for a cluster-randomized adaptive platform trial to evaluate community-based health interventions.

BACKGROUND: Trials have demonstrated that azithromycin mass drug administration (MDA) to children 1-59 months old reduces mortality but increases antimicrobial resistance (AMR). The World Health Organization recommends that programs include mortality and AMR monitoring. Niger is expanding the azithromycin MDA for child survival program nationwide. METHODS: To establish program monitoring and leverage the infrastructure to evaluate other community health interventions, AVENIR II is designed as a cluster-randomized adaptive platform trial with monitoring and re-randomization every 2 years. Childhood cancer care in Northern Tanzania: hospital infrastructure and provision of services.

INTRODUCTION: Survival rates for childhood cancer remain far lower in low- and middle-income countries (LMICs) compared to high-income countries (HICs). In Tanzania, challenges in cancer care for children are driven by shortages of trained providers, limited infrastructure, and constrained access to essential medications. This study evaluates pediatric oncology capacity and infrastructure in Northern Tanzania to identify system gaps and opportunities for improvement. METHODS: A cross-sectional survey of capacity for pediatric cancer care was conducted at 25 hospitals across the Kilimanjaro, A

Archetype-based stratification to inform sub-national tailoring of malaria interventions in Zambia.

INTRODUCTION: Although Zambia does not rank among the highest contributors to global malaria cases, in 2023, it had approximately eleven million cases. Furthermore, Zambia's malaria incidence distribution is spatially heterogeneous at fine scales such as health facility catchment areas (HFCAs). HFCA-level malaria incidence stratification currently guides implementation and targeting approaches. We aim to enhance the current stratification approach by incorporating archetypical characteristics associated with malaria, including environmental, access, epidemiological, and socioeconomic factors.
