Back pain and vertebral compression: an unusual presentation of childhood acute lymphoblastic leukemia.

Junayet, a nine years and six months old boy was admitted to the hospital because of back pain and vertebral compression fractures. The boy had been well until two months earlier, when he began to have back pain after falling on his back along with occasional fever. The pain was intermittent initially but gradually it became constant. One month before admission, he fell again and the back pain became deteriorated. He was mildly pale, liver was palpable, skin survey revealed normal, BCG scar mark was present. He had bone pain, cervical lymphadenopathy and a tender swelling on the lumbusacral re

[Ossification of the sheep skeleton].

The ossification of sheep skeleton was investigated from 4th to 10th week of gravidity by means of cryostat and paraffin serial sections, cleared specimens and grenz ray radiographs. Significant decalcification occurred in paraffin sections and cleared specimens. These artifacts were not observed in cryostat sections and grenz ray radiographs of silver impregnated specimens. The Clavicle being the oldest bone of sheep embryo contains calcium deposits for the first time at a crown rump length (CRL) of 20.5 mm. During the further development of limbs Radius (at 28 mm CRL) and Tibia (at 30 mm CRL

Early and Late Outcomes of Surgery for Neurogenic Thoracic Outlet Syndrome in Adolescents.

BACKGROUND: Neurogenic thoracic outlet syndrome is a condition that is both complex to diagnose and manage successfully. The aim of our study was to present our experience and outcomes of surgical management of thoracic outlet syndrome in adolescents. METHODS: We performed a retrospective analysis of a prospectively held database of consecutive adolescents (age 10-19 years) who underwent surgery for neurogenic thoracic outlet syndrome between 2005 and 2017 at our university hospital. RESULTS: Fourteen patients were identified (19 operations), with a mean age of 16.5 years (SD: 1.9). All pati

A hand surgeon's advanced experience with thoracic outlet compression syndrome.

Although hand surgeons may often see patients with arm and hand pain, numbness and tingling in their practice, the possibility of the presence of thoracic outlet compression syndrome (TOCS) is not often considered. In our practice almost half of newly referred patients have the complaint of upper extremity pain, numbness and tingling. In approximately 50% of these patients detailed history and physical examination are suggestive of TOCS. For this reason it is quite important to recognize the possibility of the existence of this condition. Very often in the past, and occasionally today, this co

Thoracic ossification of ligamentum flavum caused by skeletal fluorosis.

Thoracic ossification of ligamentum flavum (OLF) caused by skeletal fluorosis is rare. Only six patients had been reported in the English literature. This study reports findings from the first clinical series of this disease. This was a retrospective study of patients with thoracic OLF due to skeletal fluorosis who underwent surgical management at the authors' hospital between 1993 and 2003. Diagnosis of skeletal fluorosis was made based on the epidemic history, clinical symptoms, radiographic findings, and urinalysis. En bloc laminectomy decompression of the involved thoracic levels was perfo

[Thoracic outlet syndrome: differential diagnosis and surgical therapeutic options].

The thoracic outlet syndrome (TOS) is a disputed syndrome in the field of the so-called compression syndromes of the upper extremity. This is no surprise, as the TOS is a complex and multifactorial compression syndrome diagnosed by different medical specialities. On average 6.5 physicians of different specialities need 4.3 years to come up with the diagnosis of TOS. The correct diagnosis is of great importance and crucial for successful operative treatment. The necessary diagnostic tests can only be accomplished by team work, especially interdisciplinary cooperation. This expressly includes ph

Severity of intrathoracic injuries associated with first rib fractures.

The benign condition of isolated first rib fracture is compared with the severity of intrathoracic injuries resulting from first rib fracture associated with multiple rib injuries. Seventy-five patients with 90 first rib fractures were divided into two groups. Group 1 (n = 13) included those with isolated first rib fractures and Group 2 (n = 62), those with first-rib fractures associated with multiple rib injuries. In Group 1 patients, intrathoracic injuries were mild with no major vascular injuries. Conversely, Group 2 patients sustained severe intrathoracic injury, 58% of them with aortic in

AI-Assisted Fracture Detection in Orthopedic and Trauma Imaging: Where It Works, Where It Fails, and Principles for Safe Clinical Deployment.

BACKGROUND: Missed fractures on initial imaging assessments remain a clinically significant source of diagnostic errors in orthopedic and trauma care. AI-assisted imaging tools are increasingly integrated into fracture detection workflows. However, their diagnostic benefits and safety vary substantially across anatomical regions, clinical contexts, and levels of reader experience. PURPOSE: To synthesize the current evidence on the diagnostic impact of AI-assisted fracture detection and to discuss evidence-informed principles for safe and selective clinical deployment. METHODS: A structured n

Traumatic Posterior Atlantoaxial Dislocation with Intact Odontoid Treated by C2 Laminar Screw Fixation: A Case Report.

BACKGROUND: Atlantoaxial dislocation (AAD) is a rare but potentially life-threatening condition characterized by an abnormal alignment between the atlas and axis. Traumatic AAD are commonly associated with severe ligamentous injury and may result in spinal cord compression or neurological deficits. Posterior atlantoaxial dislocation with an intact odontoid process and without associated fracture is extremely uncommon and has rarely been reported in literature. OBJECTIVE: To report a rare case of traumatic posterior atlantoaxial dislocation with an intact odontoid process and describe its mana

Intermediate and Long-term Outcomes Following Surgical Decompression of Neurogenic Thoracic Outlet Syndrome in an Adolescent Patient Population.

Background: Although the diagnosis of thoracic outlet syndrome (TOS) is often missed, outcomes from surgical intervention significantly improve patient satisfaction. This article seeks to highlight patient characteristics, intraoperative findings, and both short and long-term outcomes of thoracic outlet decompression in the adolescent population. Methods: A retrospective chart review of patients between the ages of 13 and 21 years with a clinical diagnosis of neurogenic thoracic outlet syndrome (NTOS) who were treated surgically between 2000 and 2015 was performed. Comparison of skeletal segmentation by deep learning-based and atlas-based segmentation in prostate cancer patients.

OBJECTIVE: We aimed to compare the deep learning-based (VSBONE BSI) and atlas-based (BONENAVI) segmentation accuracy that have been developed to measure the bone scan index based on skeletal segmentation. METHODS: We retrospectively conducted bone scans for 383 patients with prostate cancer. These patients were divided into two groups: 208 patients were injected with 99mTc-hydroxymethylene diphosphonate processed by VSBONE BSI, and 175 patients were injected with 99mTc-methylene diphosphonate processed by BONENAVI. Three observers classified the skeletal segmentations as either a "Match" or "

Sternal fractures are frequent among polytraumatised patients following high deceleration velocities in a severe vehicle crash.

Источник пометил публикацию как отозванную

INTRODUCTION: Sternal fractures are a rare entity. We hypothesised that a sternal fracture is an indicator of injury severity following traffic accidents. METHODS: Analysis of technical indicators of the collision, preclinical and clinical data of patients with sternal fractures from 1985 to 2004 among 42,055 injured patients assessed by an Accident Research Unit. RESULTS: Only 267/42,055 patients (0.64%) suffered a sternal fracture within the 20-year period. Soft tissue bruises are most often concomitant injuries (55%), followed by cervical spine injuries (23%), multiple rib fractures (14%)

Anatomical basis for a successful upper limb sympathectomy in the thoracoscopic era.

In this clinico-anatomical study, factors potentially responsible for unsuccessful upper limb sympathectomy (ULS) by the thoracoscopic route were evaluated. This study comprised two subsets: 1) in the clinical subset, 25 patients (n = 50 sides) underwent bilateral second thoracic ganglionectomy for palmar hyperhidrosis, and factors predisposing to unsuccessful ULS were identified; and 2) in the anatomical subset, the neural connections of the first and second intercostal spaces were bilaterally dissected in 22 adult cadavers (22 right, 21 left; n = 43 sides). Alternate neural pathways (ANP) we

Pedestrian intoxication and fatal traffic accident injury patterns.

INTRODUCTION: Sixteen percent of all motor-vehicle fatalities are pedestrian, and accidents involving pedestrians are associated with the highest morbidity and mortality rates. Classic pedestrian injury patterns have been described. However, it has been suggested that the pattern may differ if the pedestrian is intoxicated. The role of pedestrian intoxication on motor-vehicle accident injury patterns has not been well-delineated. HYPOTHESIS: Intoxicated pedestrian traffic victims have an injury pattern that is more serious and more rapidly fatal than is the pattern for nondrinking victims. M

Subarachnoid hemorrhage misdiagnosed as acute coronary syndrome leading to catastrophic neurologic injury: A case report.

BACKGROUND: Elevated levels of cardiac troponin and abnormal electrocardiogram changes are the primary basis for clinical diagnosis of acute coronary syndrome (ACS). Troponin levels in ACS patients can often be more than 50 times the upper reference limit. Some patients with subarachnoid hemorrhage (SAH) also show electrocardiogram abnormalities, myocardial damage, and elevated cardiac biomarkers. Unlike ACS patients, patients with SAH only have a slight increase in troponin, and the use of anticoagulants or antiplatelet drugs is prohibited. Because of the opposite treatment modalities, it is
