The maternal and child mortality in the Middle East and North Africa between 2000 and 2020: the role of health financing.

BACKGROUND: Improving maternal and child health (MCH) outcomes is a critical agenda item in global development. Health financing factors play a crucial role in affecting MCH outcomes, which vary substantially in the Middle East and North Africa (MENA) region. This study aims to examine the trends in maternal mortality rate (MMR), infant mortality rate (IMR), and under-5 mortality rate (U5MR) in the MENA region and the potential impact of health financing factors on them. METHODS: We compiled data on MCH mortalities and potential determinants, including health financing factors, for all countr…

Public Involvement in Complex Theorising: A Co-Produced Logic Model of the Role of Context in Shaping Child Health.

INTRODUCTION: There is increasing work towards drawing on theory, implementing co-production and accounting for complexity within the production of systematic reviews for public health. In this paper, we report on the process of co-producing a theory; in this case, a graphical articulation of theory in the form of a logic model, which describes how contextual factors influence children's health. METHODS: We undertook a series of three online co-production workshops, involving 18-20 participants in each, and worked with an advisory group of experts with professional and lived expertise. An onl

Prevalence and determinants of child immunization coverage in Ethiopia: Evidence from the 2019 mini-demographic and health survey.

Immunization is a crucial public health intervention, and in Ethiopia, a nation characterized by diversity, immunization coverage shows considerable variations. This study aims to assess immunization coverage across various vaccines in Ethiopia and understand the prevalence and factors affecting immunization rates. This study utilized secondary data from the 2019 Ethiopian Mini Demographic and Health Survey (EMDHS), a community-based cross-sectional study. A Poisson regression model was used to evaluate the children's immunization coverage: the number of vaccinations a child received across 18

Effects of household concrete floors on maternal and child health: the CRADLE trial - a randomised controlled trial protocol.

INTRODUCTION: Early life soil-transmitted helminth (STH) infection and diarrhoea are associated with growth faltering, anaemia, impaired child development and mortality. Exposure to faecally contaminated soil inside the home may be a key contributor to enteric infections, and a large fraction of rural homes in low-income countries have soil floors. The objective of this study is to measure the effect of installing concrete floors in homes with soil floors on child STH infection and other maternal and child health outcomes in rural Bangladesh. METHODS AND ANALYSIS: The Cement-based flooRs AnD …

Is child anemia associated with early childhood development? A cross-sectional analysis of nine Demographic and Health Surveys.

Anemia is a significant public health problem in many low- and middle-income countries (LMICs), with young children being especially vulnerable. Iron deficiency is a leading cause of anemia and prior studies have shown associations between low iron status/iron deficiency anemia and poor child development outcomes. In LMICs, 43% of children under the age of five years are at risk of not meeting their developmental potential. However, few studies have examined associations between anemia status and early childhood development (ECD) in large population-based surveys. We examined the associations

Maternal Antenatal Depression Is Associated With Metabolic Alterations That Predict Birth Outcomes and Child Neurodevelopment and Mental Health.

BACKGROUND: Evidence regarding metabolic alterations associated with maternal antenatal depression (AD) is limited, and their role as potential biomarkers that improve the prediction of AD and adverse childbirth, neurodevelopmental, and mental health outcomes remains unexplored. METHODS: In a cohort of 331 mother-child dyads, we studied associations between AD (a history of medical register diagnoses and/or a Center for Epidemiological Studies Depression Scale score during pregnancy ≥ 20) and 95 metabolic measures analyzed 3 times during pregnancy. We tested whether the AD-related metabolic m

Geodetector analysis of individual and joint impacts of natural and human factors on maternal and child health at the provincial scale.

This ecological study examined the individual and joint impacts of natural-human factors on the spatial patterns of maternal and child health status in China at the provincial scale in 2020. We considered natural factors (forest coverage, average temperature, and total sulfur dioxide and particulate matter emissions) and human factors (economic development, urbanization, healthcare access, and education level). We combined maternal, infant, and under-five mortality rates into a composite maternal and child health index using the entropy method. The spatial autocorrelation analysis of this inde…

Prenatal exposure to synthetic phenols and phthalates and child respiratory health from 2 to 36 months of life.

Exposure to phthalates and synthetic phenols is ubiquitous. Some of them are suspected to impact child respiratory health, although evidence still remains insufficient. This study investigated the associations between prenatal exposure to phthalates and phenols, individually and as a mixture, and child respiratory health assessed by objective lung function measures since 2 months of age. Among 479 mother-child pairs from the SEPAGES cohort, 12 phenols, 13 phthalate and 2 non-phthalate plasticizer metabolites were measured in 2 pools including each 21 urine samples collected at the 2nd and 3rd

The case of an abused child exposed to the direct danger of serious damage to health in the form of a permanent mental illness.

Prolonged exposure to violence in any form takes a toll on children's health. It may increase the risk of multiple mental disorders appearing in adult life. Such disorders may be of severe intensity, and in extreme cases, they can even disable normal functioning both individually and socially. The present case concerns an eight-year-old girl subjected to all forms of abuse (sexual, physical, psychological and negligence) in her biological family. The foster parents made a notification of a suspected criminal offence based on their multiple observations of behavioral disorders of the child. Aft

A Survey on Cases of Serious and High-Risk Child Abuse with Trauma Using the Database of Health Insurance Claims.

In recent years, the annual number of child deaths due to child abuse has been around 50 per year in Japan. On the other hand, the actual situation of dangerous physical abuse cases such as abuse with residual has not been clarified. Therefore, this study investigated children with trauma suspected of being physically abused, using the health insurance claims data of Japan. There were cases with trauma which are likely to have sequelae. Since this study used the sampling data, there is a high possibility that there will be a considerable number of cases of high-risk abuse.

A Path FORWARD: Development of a Comprehensive Multidisciplinary Clinic to Create Health and Wellness for the Child and Adolescent with a Fontan Circulation.

Today, it is anticipated most individuals diagnosed with single-ventricle malformation will survive surgical reconstruction through a successful Fontan operation. As greater numbers of patients survive, so has the recognition that individuals with Fontan circulation face a variety of challenges. The goal of a normal quality and duration of life will not be reached by all. The hurdles fall into a variety of domains. From a cardiovascular perspective, the Fontan circulation is fundamentally flawed by its inherent nature of creating a state of chronically elevated venous pressure and congestion,
