Telomere biology disorders associated with childhood interstitial lung disease.

BACKGROUND: Telomere biology disorders (TBDs) are rare inherited defects in telomere maintenance associated with multisystem diseases. Although pulmonary fibrosis has been well described in adults, data remain limited on childhood interstitial lung disease (chILD) related to TBDs. PURPOSE: This study aimed to characterize the pulmonary phenotype, genetic spectrum, and clinical course of pediatric TBD-associated chILD. METHODS: We performed this registry-based cohort study using the European chILD-EU database to identify children with telomere maintenance gene variants. Their clinical data, l

Redesigning Pediatric Preventive Care to Advance Child Health.

Pediatric preventive care, beginning at birth and extending through young adulthood to support the health of children, is a cornerstone of pediatric primary care, supporting the physical, developmental, and mental health of children through periodic health supervision visits, also known as "well-child care". Pediatric preventive care visits provide opportunities to monitor growth and development, identify physical and mental health conditions early, deliver disease prevention and health promotion services, offer anticipatory guidance and education to improve child safety and well-being, and pr

Aortopexy Complicated by ST Segment Elevations in a Four-Month-Old Infant.

A four-month-old female infant presented for a thoracoscopic aortopexy for severe tracheomalacia. The case proceeded uneventfully until a specimen bag was introduced into the chest to remove the thymus. The child developed significant ST segment elevations in all 12 leads on electrocardiogram. An emergent intraoperative echocardiogram was performed but did not reveal any findings to account for the diffuse ST segment elevations. The ST segment elevations remained elevated for 48 hours following the procedure with no apparent hemodynamic instability or structural damage to the heart. Troponin l

Reproductive Health and Bodily Integrity in Tanzania.

Several policy initiatives support the empowerment of women to improve their reproductive health. Little is known, however, about the inverse effect that reproductive health might have on women's empowerment. Women are pressured to conform to their reproductive role, and an inability to do so might affect their empowerment, including control over their own body. Using a panel dataset of 504 married women in Northern Tanzania, we find that women who experienced a pregnancy loss show more tolerant views of partner violence and that child mortality lowers their perceived control over the sexual r…

Optimising the use of routine immunisation clinics for early childhood development in sub-Saharan Africa.

There is now ample evidence that factors that account for high infant and child mortality in sub-Saharan Africa and other developing regions are also associated with lifelong developmental impairments in the survivors from early childhood. Of all routine immunisation programmes widely administered soon after birth, bacille Calmette-Guérin (BCG) and diphtheria-pertussis-tetanus (DPT) offer effective platforms to implement a package of interventions that extend beyond child survival to include the early detection and prompt management of developmental disabilities as recently demonstrated in som…

[Children arthropod bites protective measures: insecticides and repellents].

Vector transmitted diseases are often a serious threat for child health, especially for children traveller in tropical regions. Few arthropod borne diseases are preventable by immunization or chimioprophylaxis. Prevention of most of them is based on personal protection against arthropod bites. The evidence of its efficacy has been established by the use of impregnated bed nets, impregnated clothes with permethrin or mosquito repellent which reduced significantly child malaria morbidity and mortality in endemic countries. These personal protective measures are able to minimize arthropod bites a

[Anesthesiologic management of cesarean section in a patient with transposition of the great vessels].

The maternal mortality associated with cesarean section in the presence of congenital cyanotic heart disease is high. We report the anesthetic management of a 26-year-old pregnant patient with transposition of the great vessels and a functional single ventricle whose child was delivered by elective cesarean section under continuous epidural anesthesia. There were no hemodynamic problems and the outcome was successful for both mother and child, who could be discharged from the hospital on the 17th postoperative day. Elective cesarean section may be an acceptable method of delivery, and lumbar e

A community-wide infant mortality review: findings and implications.

The authors present the results of a community-wide infant mortality review, describe implications for the delivery of maternal and child health services, and discuss the value of such reviews in addressing local public health concerns. The review included an analysis of birth and death certificates and medical record data; maternal interviews; review of cases and development of recommendations by provider panels; and convening of community groups to develop strategies to improve the health and health care of women and infants. The review focused on 287 infant deaths during 1990-1993. More tha

Frequency and profile of pediatric mortality in an emergency department: a retrospective cross-sectional study from a low-middle income country.

BACKGROUND: Pediatric mortality remains a significant challenge in low- and middle-income countries (LMICs), particularly in emergency care settings. This study aimed to evaluate the frequency, causes, and clinical characteristics of pediatric deaths in a tertiary emergency department (ED) in Pakistan. METHODS: We conducted a retrospective chart review of all pediatric patients (age 0-18 years) who died in the pediatric ED at Aga Khan University Hospital, Karachi, between January 2017 and December 2021. The ED serves a catchment population of approximately 25 million. Data were extracted on d

The outcome of triplet, quadruplet, and quintuplet pregnancies managed in a perinatal unit: obstetric, neonatal, and follow-up data.

Multifetal gestation is associated with increased frequency of maternal complications and higher perinatal morbidity and mortality. The need for contemporary data on the outcome of multifetal gestations is further underscored when selective reduction is considered. The present study details the obstetric management, neonatal outcome, and follow-up data of 24 triplet, five quadruplet, and one quintuplet pregnancies delivered in a perinatal center. The early neonatal mortality rate was 31.6, the late neonatal mortality rate was 21, and the perinatal mortality rate was 51.5. Survival to discharge

Antenatal care coverage and early childhood mortality in Zimbabwe: new interpretations from nationally representative household surveys.

Zimbabwe has implemented universal antenatal care (ANC) policies since 1980 that have significantly contributed to improvements in ANC access and early childhood mortality rates. However, Demographic and Health Surveys (DHS) and Multiple Indicator Cluster Surveys (MICS), two of Zimbabwe's main sources of health data and evidence, often provide seemingly different estimates of ANC coverage and under-five mortality rates. This creates confusion that can result in disparate policies and practices, with potential negative impacts on mother and child health in Zimbabwe. We conducted a comparability

Paediatric outcomes and timing of admission.

Studies of adult patients have demonstrated that weekend admissions compared with weekday admissions had a significantly higher hospital mortality rate. We have reviewed the literature to determine if the timing of admission, for example, weekend or weekday, influenced mortality and morbidity in children. Seventeen studies reported the effect of timing of admission on mortality, and only four studies demonstrated an increase in those admitted at the weekend. Meta-analysis of the results of 15 of the studies demonstrated there was no significant weekend effect. There was, however, considerable

Late-Onset Vitamin K Deficiency Resulting in Intracranial Bleeds: A Case Report and Literature Review.

We report a case of a 24-day-old male neonate who presented with multiple episodes of apnea, poor feeding, excessive crying, irritability, vomiting, and yellowish discoloration. The patient did not receive a vitamin K injection at birth, and most likely, the neonate's dietary intake was insufficient to meet vitamin K requirements for coagulation, which resulted in an intracranial hemorrhage, which ultimately resulted in the mortality of the child.

Management of acquired cardiac disease in the obstetric patient.

Physiologic changes incurred by pregnancy can cause severe decompensation in the parturient with underlying cardiac disease. The result is increased morbidity and mortality for both mother and child. Appropriate anesthetic management can significantly impact these outcomes. This review systematically presents the pathophysiology, peripartum risk, and anesthetic management in the puerperium of specific acquired cardiac abnormalities including: valvular disease, pulmonary hypertension, cardiomyopathy, cardiac transplantation, ischemia, arrhythmias, and cardiac arrest.
