Dismantling ableism in health professions education: imperative in psychosocial management of pain.

Pain and chronic pain are prevalent and costly. Integrative models of pain management including the biopsychosocial (BPS) model of pain are central to contemporary public health efforts to address pain, however, uptake of the BPS model of pain in patient care is inconsistent. Aspects of health professions education (HPE) have been identified as barriers to BPS use including emphasis on the medical model and the hidden curriculum. Similarly, hidden curriculum in HPE limits disability inclusion and reduces healthcare accessibility. This perspective offers a conceptual framing of ableism as a dri

Genetic variants of GCH1 associate with chronic and acute crisis pain in African Americans with sickle cell disease.

The multidimensional nature of pain in sickle cell disease (SCD) has rendered its therapeutic management extremely challenging. In this study, we explored the role of five single nucleotide polymorphisms (SNPs) of candidate gene GCH1 in SCD pain. Composite pain index (CPI) scores and acute care utilization rates were used as phenotype markers. Rs8007267 was associated with chronic pain (additive model: B = -3.76, p = 0.037; dominant model: B = -5.61, p = 0.021) and rs3783641 (additive model: incident rate ratio [IRR] = 1.37, p = 0.024; recessive model: IRR = 1.81, p = 0.018) with utilization r

A mouse model of diet-induced obesity and insulin resistance.

Obesity is reaching pandemic proportions in Western society. It has resulted in increasing health care burden and decreasing life expectancy. Obesity is a complex, chronic disease, involving decades of pathophysiological changes and adaptation. Therefore, it is difficult ascertain the exact mechanisms for this long-term process in humans. To circumvent some of these issues, several surrogate models are available, including murine genetic loss-of-function mutations, transgenic gain-of-function mutations, polygenic models, and different environmental exposure models. The mouse model of diet-indu

Development of a Digital Multimodal Pain Intervention (CHRONIC): Protocol for a Randomized Controlled Trial Study.

BACKGROUND: Chronic pain affects approximately 12 million individuals in Germany and significantly impairs quality of life. Although multimodal treatment approaches-combining physical, psychological, and behavioral strategies-are considered the gold standard, access to specialized multidisciplinary pain treatment, including physiotherapy and psychological interventions, is often hampered by long waiting times, which highlights the need for scalable, low-threshold treatment options. Digital health interventions, particularly those grounded in established therapeutic models, offer a promising so

Out-of-Pocket Healthcare Expenditures Associated with Prescription Opioid Use in Older Adults with Non-cancer Chronic Pain.

INTRODUCTION: Chronic pain is a long-term illness impacting quality of life in older adults, and while opioids are frequently used to manage chronic pain in all adults, there is increased risk of medical complications in older adults stemming from opioid therapy. Therefore, we aimed to examine the relationship of prescription opioids on out-of-pocket (OOP) healthcare expenditures in a cohort of older adults (≥ 50 years) with chronic pain in the United States (US). METHODS: Our analysis used data from the Health and Retirement Study, a longitudinal cohort study capturing health and lifestyle i

Asthma and Comorbid Conditions-Pulmonary Comorbidity.

Pulmonary comorbidities can increase disease severity and health care costs associated with asthma management. Vocal cord dysfunction/inducible laryngeal obstruction is a common comorbidity that results from intermittent laryngeal obstruction. Patients describe distinct episodes of dyspnea that do not respond to bronchodilators. Inspiratory stridor is common. The gold standard diagnostic testing strategy is continuous laryngoscopy performed during exercise or irritant challenges. Dysfunctional breathing (DB) is an overarching term that describes conditions with a chronic change in the pattern

[Types of Pain Management Assessment].

Pain assessments are essential for the diagnosis and treatment planning of pain disorders. Guided by the biopsychosocial model, they evaluate physical, psychological, and social factors that contribute to pain chronification. Unimodal assessments, typically performed by pain specialists, are often the first step in specialized pain care. If risk factors for chronification or established chronic pain are identified, an interdisciplinary multimodal assessment (IMA) involving physicians, psychologists, and physiotherapists is recommended. The IMA combines medical, functional, and psychological ev

Bionic meta microneedle photonic crystal patch for advanced wound care.

Chronic wounds remain difficult to manage due to unstable adhesion, uncontrolled therapy, and the absence of real-time monitoring. Here, we present a bionic meta microneedle photonic crystal patch that integrates robust tissue adhesion, spatiotemporal drug delivery, and optical biosensing in a single platform. The patch combines drug-loaded biodegradable microneedles with a micropillar-inspired interface to establish a tissue-mimetic three-dimensional contact, ensuring secure attachment on moist and dynamic skin surfaces. Embedded photonic crystal structures function as high-sensitivity optica

Many strategies enabling us to optimize kidney care may also be planet friendly.

Kidney replacement therapy is one of the most energy-consuming and waste-producing medical treatments. Reducing the need of dialysis is therefore an environmentally friendly choice. However, preferring prevention, lifestyle-related interventions and patient education to drugs is time consuming, and most physicians are already overburdened by the many demands of routine clinical practice. In this mini review, we discuss the role that could be played by prevention, diet, and nonpharmacologic interventions in reducing the impact of care of chronic kidney disease and kidney replacement therapy. Reimagining kidney value-based care: leveraging data science for dynamic, clinician-level risk prediction.

PURPOSE OF REVIEW: Chronic kidney disease (CKD) remains one of the costliest conditions in healthcare, yet value-based payment (VBP) reforms have produced limited improvements in cost or quality. Existing renal payment models rely on annual benchmarks that fail to reflect the longitudinal reality of CKD care. This precludes the possibility of observing a dose-response relationship in clinician-level interventions, effectively invisible in current VBP structures. RECENT FINDINGS: Advances in clinical informatics and predictive modeling demonstrate the feasibility of sub-annual, patient-level r

Effect of evidence-based nursing supervision on hemodialysis patients with chronic kidney failure.

This study aims to evaluate the effects of an evidence-based supervision nursing model on self-management, quality of life, emotional well-being, and complication rates in patients with chronic renal failure (CRF) undergoing hemodialysis. A retrospective cohort analysis was conducted on 160 patients with CRF receiving hemodialysis between January 2022 and December 2023. After minimizing potential biases, patients were randomly assigned to either Group A (n = 80), who received evidence-based supervision nursing under a high-quality management model, or Group B (n = 80), who received traditional

Home-Based Primary Care: Adaptability Criteria for the Bedroom Layout and the Furnitures/Technological Equipments.

Within the past decade, advances in medical technology, the desires and complex care needs of an ageing population, and innovative care delivery models have initiated a shift from providing care in hospitals to outpatient settings. And more recently, the acceleration and amplification of these factors is pushing healthcare options even further from the traditional inpatient and outpatient settings towards acute and subacute care in the home. This has led the medical community to look toward providing more tools and methods of care that patients can access safely right from home and the designe

A qualitative study of the palliative care needs of primary care patients with COPD: Experiences of COPD patients, their families and healthcare professionals.

BACKGROUND: The majority of deaths from chronic obstructive pulmonary disease (COPD) occur in low- and middle-income countries. Primary palliative care is commonly proposed as the best model to ensure access to appropriate care. However, the existing evidence on needs, models and outcomes comes from high-income settings. The purpose of the study was to determine the experiences of adults with COPD attending primary care, their informal caregivers and healthcare professionals to identify current care provided and gaps in this care in primary care settings in South Africa. METHODS: Cross-sectio

Primary Care Accessibility Effects on Health Care Utilization Among Urban Children.

OBJECTIVE: Evidence suggests that spatial accessibility to primary care is a contributing factor to appropriate health care utilization, with limited primary care access resulting in avoidable hospitalizations and emergency department visits which are burdensome on individuals and our health care system. Limited research, however, has examined the effects on children. METHODS: We evaluated associations of spatial accessibility to primary care on health care utilization among a sample of 16,709 children aged 0 to 3 years in Philadelphia who were primarily non-White and publicly insured.
