Treatment decision-making capacity in hospitalized patients with schizophrenia.

In contemporary medical ethics and clinical practice the autonomy of patients and their right to accept or refuse an offered treatment is acknowledged and respected. Prerequisite for the right of a patient to consent to any medical act is the capacity to make valid decisions regarding his/ her treatment. The objective of our study was to assess -for the first time in our country- treatment decision-making capacity of hospitalized patients with schizophrenia; to explore any possible association with demographic and clinical variables; and to compare treatment decision-making capacity of patient

[Distinguishing misconceptions between outcome prediction and treatment effect prediction models].

Prediction models are increasingly used to inform clinical decision-making. However, outcome prediction models (i.e., traditional prediction models) and treatment effect prediction models (i.e., prediction under interventions) are often confused in practice, which may mislead clinical decisions and lead to adverse clinical outcomes. Using typical study cases, this paper systematically clarifies common misconceptions and key differences between the two model types. It presents the latest advances in the development and application of treatment-effect prediction models, aiming to avoid misinterp…

Decision-Making Process and Minimally Invasive Surgical Experience to Approach Achilles Tendinopathy.

Insertional Achilles tendinopathy is a complex condition with multiple contributing factors, including biomechanical stress, anatomic variations, and systemic comorbidities. Conservative treatment options, such as activity modification, rest, and physiotherapy, remain the first-line approach. However, for patients who fail non-surgical methods, both traditional open and minimally invasive surgical options are available. The choice of surgical intervention is guided by the severity of tendon degeneration and the presence of bony abnormalities, such as Haglund's deformity. Minimally invasive pro

Bayesian Decision-Making Under Uncertainty in Borderline Personality Disorder.

Bayesian decision theory suggests that optimal decision-making should use and weigh prior beliefs with current information, according to their relative uncertainties. However, some characteristics of borderline personality disorder (BPD) patients, such as fast, drastic changes in the overall perception of themselves and others, suggest they may be underrelying on priors. Here, we investigated if BPD patients have a general deficit in relying on or combining prior with current information. We analyzed this by having BPD patients (n = 23) and healthy controls (n = 18) perform a coin-catching sen

A systematic review of the application of deep learning techniques in the physiotherapeutic therapy of musculoskeletal pathologies.

Physiotherapy is a critical area of healthcare that involves the assessment and treatment of physical disabilities and injuries. The use of Artificial Intelligence (AI) in physiotherapy has gained significant attention due to its potential to enhance the accuracy and effectiveness of clinical decision-making and treatment outcomes. Nevertheless, it is still a rather innovative field of application of these techniques and there is a need to find what aspects are highly developed and what possible job niches can be exploited. This systematic review aims to evaluate the current state of research …

Microsurgical approach for the management of gingival cleft: A case series and decision-making process.

BACKGROUND: Gingival clefts, once known as "Stillman's Cleft", now considered an obsolete phenomenon, cannot be neglected in clinical practice, especially when it is persistent and epithelialized. The attached gingiva and alveolar mucosa are composed of epithelial layers with subjacent connective tissue. Gingival clefts, notwithstanding their intrinsic differences, may exhibit keratinized or non-keratinized tissue. Coupled with additional risk factors, it can result in progressive attachment loss and gingival recession. METHODS: Two cases with three distinct types of gingival clefts were desc

Intraoperative decision-making in thrombectomy for bilateral cerebral embolism: a mimic of thrombus migration.

This study thoroughly examines the challenges in managing bilateral embolization, emphasizing intraoperative judgment. It aims to underscore the necessity of accurate etiological judgments and the limitations of cross-circulation thrombectomy. A 59-year-old male with hypertension and atrial fibrillation developed sudden limb twitching, progressing to limb paralysis and speech impairment. Neurological assessments identified multiple symptoms, while a head CT scan excluded intracranial hemorrhage. A mechanical thrombectomy revealed occlusions in the right internal cerebral artery (ICA) and left

Beyond the RCT: When are Randomized Trials Unnecessary for New Therapeutic Devices, and What Should We Do Instead?

OBJECTIVE: The aim of this study was to develop an evidence-based framework for evaluation of therapeutic devices, based on ethical principles and clinical evidence considerations. SUMMARY BACKGROUND DATA: Nearly all medical products which do not work solely through chemical action are regulated as medical devices. Their huge range of purposes, mechanisms of action and risks pose challenges for regulation. High-profile implantable device failures have fuelled concerns about the level of clinical evidence needed for market approval. Calls for more rigorous evaluation lack clarity about what ki

Transvenous or subcutaneous implantable cardioverter defibrillator: a review to aid decision-making.

The implantable cardioverter-defibrillator (ICD) is a proven treatment for preventing sudden cardiac death. Transvenous leads are associated with significant mortality and morbidity, and the subcutaneous ICD (S-ICD) addresses this. However, it is not without limitations, in particular the absence of anti-tachycardia pacing. The decision of which device is most suitable for an individual patient is often complex. Here, we review the relative merits and weaknesses of both the transvenous and S-ICD. We summarise the available evidence for each device in particular patient cohorts, namely: ischaem

Kienbock's Disease: Treatment Options - A Search for the Apt!

Kienbock's disease remains a condition of uncertain etiology and our understanding about the pathogenesis is still evolving. However, a lack of consensus on the treatment is the most striking as there are wide array of treatment options described ranging from 'doing nothing' to a free vascularized bone graft reconstruction of the lunate. Furthermore, most treatment modalities report equivalent success rate but cumulative evidence is lacking. This 'black hole' in the literature presents difficulties to surgeons treating the condition and even patient's confidence with the treatment. All the tre

Diagnostic decision-making tool for imaging term neonatal bowel obstruction.

Term neonatal bowel obstruction is common, and absence of treatment is potentially catastrophic. There is a relatively narrow differential diagnosis, with causes categorised as either low or high bowel obstruction. The commonest causes of low bowel obstruction include anorectal malformations (ARM), Hirschsprung's disease, ileal atresia, meconium ileus, meconium plug, and colonic atresia. The commonest causes of high bowel obstruction include duodenal atresia, duodenal stenosis/web, jejunal atresia, and malrotation with volvulus (and hypertrophic pyloric stenosis usually presenting in slightly

Computer algorithm can match physicians' decisions about blood transfusions.

BACKGROUND: Checking appropriateness of blood transfusion for quality assurance required enormous usage of time and human resources from the healthcare system. We report here a new machine learning algorithm for checking blood transfusion quality. MATERIALS AND METHODS: The multilayer perceptron neural network (MLPNN) was designed to learn an expert's judgement from 4946 clinical cases. The accuracy in predicting the blood transfusion was then reported. RESULTS: We achieved a 96.8% overall accuracy rate, with a 99% match rate to the experts' judgement on those appropriate cases and 90.9% on

Practice advisory on the bleeding risks for peripheral nerve and interfascial plane blockade: evidence review and expert consensus.

The risk of bleeding complications during regional anesthesia procedures is a significant patient safety consideration. Nevertheless, existing literature provides limited guidance on the stratification of bleeding risk for peripheral nerve and newly described interfascial plane blocks. Our objective was to produce an evidence-based consensus advisory that classifies bleeding risks in patients undergoing regional peripheral nerve and interfascial plane block procedures. This advisory is intended to facilitate clinical decision-making in conjunction with national or local guidelines and to guide…
