BACKGROUND: The aims of this study were to determine how often fracture reduction alone restored pulses and vascular perfusion in displaced supracondylar humerus fractures with absent distal pulses on presentation, and whether any preoperative factors were associated with the need for vascular repair and vascular complications.
METHODS: We reviewed 1255 supracondylar humerus fractures in children treated operatively over 12 years at one institution. We identified 33 patients who presented with displaced supracondylar humerus fractures and absent distal pulses. We reviewed the management and o

Reamed interlocking intramedullary fixation is the treatment of choice for displaced tibial shaft fractures in adults. In most cases it can be performed without difficulty; however, technical difficulties may be encountered during nailing in some cases. This article describes a case of closed nailing for a tibial shaft fracture in which intramedullary guide wire was obstructed by a small intramedullary bone fragment in the distal fracture segment. Forceful reaming and insertion of the nail led to a break in the cortex of the distal fragment and bending of guide wire. Finally, open reduction an

INTRODUCTION: Simultaneous ipsilateral hip and knee dislocations are extremely rare, especially when associated with fractures, and present significant challenges in diagnosis and management. These injuries, often resulting from high-velocity trauma, require prompt intervention to tackle the complications and achieve a good outcome.
CASE REPORT: We present the case of a 28-year-old male who sustained an ipsilateral hip fracture dislocation and knee dislocation following a high-velocity road traffic accident. Initial clinical examination revealed significant deformities in both joints, and ima

INTRODUCTION: The Monteggia equivalent type 1b fracture, consisting of an ulnar shaft fracture and radial head dislocation, is a rare condition first described by Giovanni Battista Monteggia. The Metaizeau technique, emphasizing percutaneous intramedullary fixation to preserve bone healing and prevent growth plate damage, has gained prominence in managing these fractures.
CASE REPORT: A 10-year-old boy presented with left forearm pain, swelling, and deformity following a fall while playing football. X-rays revealed a proximal fourth ulnar fracture with dorsal and lateral angulation, along wit

The mainstream surgery for valgus-impacted femoral neck fractures (FNFs) is closed reduction and internal fixation under fluoroscopy. However, femoral neck shortening and anterior femoroacetabular impingement are common complications in healed valgus-impacted FNFs after in situ fixation. Some methods have been reported to prevent complications, but these techniques require the use of a transfixing guide wire that passes through the femoral head, which may cause articular cartilage damage. We introduce a simple reduction technique using one Steinmann pin (S-pin) for valgus-impacted FNFs without

Two cases of fracture of the ulna with fracture-separation of the radial neck in children are presented. In one case there was no displacement of the radial epiphysis and in the other it was subluxated. The mechanism of injury is essentially a valgus strain on the extended elbow caused by a fall onto the out-stretched hand. Reduction of this fracture is difficult to attain by closed manipulation and may displace an undisplaced radial head. Open reduction is advisable and as the lesion is unstable, some form of fixation may be required; check radiographs of good quality are essential during the

McCune-Albright syndrome (MAS) is a complex endocrinopathy with polyostotic fibrous dysplasia and café-au-lait spots. Pathological femoral shaft fracture along with MAS is not a common occurrence. Bisphosphonates can be used for the management of pathological fractures. A seven-year-old male child presented with pathological femoral shaft fracture with MAS (café-au-lait macules and hyperthyroidism). The patient was managed conservatively with closed reduction along with hip spica and oral alendronate. Fracture consolidation occurred within six to eight weeks with improvement in bone pains. Ora

Background: Hand fractures and dislocations are common injuries in the pediatric population. This study aims to characterize the pediatric hand injuries that required closed reduction and identify those that required multiple reduction attempts. Methods: A retrospective cohort study was carried out in patients younger than 18 years of age with hand fractures or dislocations who underwent closed reduction in the emergency department (ED). Patients who ultimately required surgical reduction and fixation were not included. Results: Of the 310 hand injuries identified, 148 (114 fractures and 34 di

An unstable distal radius fracture is one that is not capable of resisting displacement after initial closed reduction. The challenge in managing distal radius fractures is identifying which fractures are prone to displacement. Currently, there are no standard criteria for assessing the stability of distal radius fractures. The fracture pattern and patient characteristics should be taken into consideration when treating distal radius fractures. This article discusses how to recognize and manage distal radius fractures that are prone to displacement.

An unusual case of trans-scaphoid perilunate fracture-dislocation of the carpus with radial displacement in a 19-year-old man is reported. On the basis of the clinical and radiologic findings it was assumed that the tip of the radial styloid process caused the scaphoid fracture. Following closed reduction with acceptable positioning of the carpal bones, the patient has good function and slight pain, and was able to return to almost full employment about 1 year postinjury.

The objective of this study was to evaluate the benefits of intra-operative ultrasonic guidance in the management of isolated nasal bone fractures. A prospective, randomized, controlled, double blinded study was designed. Sixty-eight patients who had isolated fracture nose were treated by either a simple closed reduction or by ultrasound-guided reduction (34 patients each) with a follow up for an average of 4.5 and 5.5 months, respectively. Post-traumatic and post-reduction nasal profiles were compared (by blinded photographer), and patient's satisfaction was analyzed after reduction in both g

The name of Ender is primarily associated with the Ender nails, which were popular for trochanteric fracture fixation more than thirty years ago. However, Ender's concepts were not limited to the implant. Ender developed a unique classification system for trochanteric and subtrochanteric fractures, which provided the theory for closed reduction maneuvers that made the method so successful in his hands. While Ender's nails have become history in the meantime, his principles of fracture reduction can be readily applied on surgery with modern implants such as proximal femoral nails. This article

Volar perilunate fracture dislocation is an extremely rare carpal injury, but associated with metacarpophalangeal joint dislocation of both the ipsilateral index and middle finger has never been reported. We report one case of a 28-year-old man following a high-energy trauma. After performing closed reduction of the metacarpophalangeal joint injury, open reduction of the wrist injury through volar approach was done, and a K-wire fixation was used to stabilize the scaphoid fracture and lunotriquetral joint. After a 16 months?follow-up period, the wrist regained a full range of motion without sy

We report a case of posterior Monteggia lesion in a 12-year-old girl consisting of a greenstick fracture of the proximal ulnar metaphysis and a radial neck fracture with a proximal and posterior 90-degree rotational displacement of the radial head. Closed reduction of the radial head resulted in complete reversal of the radial head, which was corrected by open reduction and internal fixation. Cast immobilization without correction of the plastic deformation of the proximal ulna caused posterior bowing of the ulna and posterior displacement of the fractured radial neck. Corrective osteotomy of

A case of traumatic anterior dislocation of the hip with ipsilateral subtrochanteric fracture in a child is presented. The dislocated femoral head occluded the femoral artery. Closed reduction, which was easy, proved unstable. It was found that the pull of abductors and external rotators acting on the short proximal fragment forced the femoral head to slide through the tear in the anterior capsule. This necessitated open reduction and internal fixation of the fracture to control the position of the proximal fragment. Literature review showed that this combined injury is extremely rare; only on

Femoral fracture in patients with lower extremity amputations is relatively rare (reported incidence in 341 amputee patients, 2.35%). Lack of adequate support and balance, and osteoporosis on the amputation side, may explain the predisposition to fracture of the ipsilateral extremity. The prosthesis was in use at the time of the accident in only 3 of 9 cases studied, most falls occurring during transfer activity without a prosthesis. Proximal femoral fractures in below-knee amputee patients responded to open reduction with internal fixation and early ambulation. Others were treated with closed

Treatment of mandibular pathological fractures differs according to etiology. Closed reduction with intermaxillary fixation is usually performed when fractures occur as a result of osteomyelitis. Here is reported a case of pathological fracture of the mandible resulting from osteomyelitis that was successfully treated with intermaxillary elastics only.

CASE: We report a single case of a closed intra-articular distal humerus fracture in a 28-year-old man with a preexisting fishtail deformity characterized by concavity of the central trochlea and corresponding deformity of the olecranon. The patient was treated with open reduction and internal fixation.
CONCLUSION: The case highlights the diagnosis and challenges of treatment. Conventional fixation choices and imaging techniques may need to be altered when treating a fracture with this deformity.
