Working memory in children with cochlear implants: problems are in storage, not processing.

BACKGROUND: There is growing consensus that hearing loss and consequent amplification likely interact with cognitive systems. A phenomenon often examined in regards to these potential interactions is working memory, modeled as consisting of one component responsible for storage of information and another component responsible for processing of that information. Signal degradation associated with cochlear implants should selectively inhibit storage without affecting processing. This study examined two hypotheses: (1) A single task can be used to measure storage and processing in working memory,

Cochlear implants and magnetic resonance scans: A case report and review.

OBJECTIVE AND IMPORTANCE: Traditionally, magnetic resonance imaging (MRI) was contraindicated for patients with cochlear implants (CIs), due to concern about device displacement, overheating of the device or tissues, or direct damage to the device electrode. In addition, image artifact from the device magnet gave poor imaging information in the cranial and upper cervical spine region. Today, MRI is increasingly required in patients who have undergone cochlear implantation, and CI design and MRI protocols have therefore changed to allow implanted patients to safely enter the MRI field, in some …

Single and multiple microphone noise reduction strategies in cochlear implants.

To restore hearing sensation, cochlear implants deliver electrical pulses to the auditory nerve by relying on sophisticated signal processing algorithms that convert acoustic inputs to electrical stimuli. Although individuals fitted with cochlear implants perform well in quiet, in the presence of background noise, the speech intelligibility of cochlear implant listeners is more susceptible to background noise than that of normal hearing listeners. Traditionally, to increase performance in noise, single-microphone noise reduction strategies have been used. More recently, a number of approaches

Postoperative complications in patients with cochlear implants and impacts of nursing intervention.

CONCLUSION: This study shows that cochlear implantation is relatively safe surgery with few major complications and within acceptable limits. However, close follow-up observation and effective medical and nursing intervention could alleviate further complications and thus become key elements for promoting recovery of patients undergoing such surgery. OBJECTIVES: Cochlear implantation has become an effective method for curing patients disabled by profound hearing loss in China. However, full exploration of the associated complications remains to be completed. The objective of this study was th…

UK and Ireland experience of cochlear implants in children with Down syndrome.

OBJECTIVES: Down Syndrome (DS) is associated with a high incidence of hearing loss. The majority of hearing loss is conductive, but between 4 and 20% is sensorineural, which in the main is mild or moderate and is managed with conventional behind-the-ear hearing aids. Cochlear implantation is an elective invasive procedure, performed to provide some form of hearing rehabilitation in individuals with severe to profound sensorineural hearing loss, and initially candidacy criteria were strict--excluding patients with additional disabilities. With good results and expanding experience, more candida…

Comparison of Electroaudiometry with cochlear implant in children with inner ear anomaly.

The promontory stimulation test (PST) using a needle electrode has been used to evaluate the sense of the auditory nerve as a preoperative examination for cochlear implant in adults. Because this is a painful test, it is not suitable for children. It has been reported that children with inner ear anomaly showed poorer outcomes of hearing after cochlear implant. Electroaudiometry developed by Med-El Corporation, which is noninvasive, is a more suitable procedure for young children. Patients were three children less than five years old with inner ear anomaly. Two patients showed common cavity, a

Predicting cochlear implant outcome from brain organisation in the deaf.

PURPOSE: Cochlear implantation is an effective technique for restoring hearing in the profoundly deaf. Although cochlear implants are a therapeutical success, huge performance variability in speech comprehension is observed after implantation. The reason for this remains incompletely understood after 20 years of clinical practice and basic research. Which patients are going to respond well and why is an unresolved question. The duration of auditory deprivation plays an important role, and currently is the main predictor of implantation success in children; basically, the earlier the better. Ho…

Elimination of facial nerve stimulation by reimplantation in cochlear implant subjects.

HYPOTHESIS: Perimodiolar intracochlear electrodes with contacts facing towards the modiolus have limited current flow towards the outer wall of the cochlea and therefore, may reduce the occurrence of facial nerve stimulation (FN) in cochlear implant subjects. BACKGROUND: Facial nerve stimulation is a well-known complication in cochlear implant treatment especially in the group of subjects with otosclerosis. The possible explanation of this side effect is a change of the electrical properties of the otosclerotic bone leading to leakage current and resulting in facial nerve stimulation. METHOD

Cochlear implants: indications in groups of patients with borderline indications. A review.

Cochlear implants (CI) represent the current treatment for patients affected by profound sensorineural hearing loss (SNHL). Initially only deaf adult patients were considered to be candidates for a CI; however, the development of technology and matured experience have expanded the indications for cochlear implantation. Today, CIs are implanted in adults and children and broader indications are followed. There are, however, a number of patients who do not completely fulfill the current indications and who are potential candidates for CI. The duration of deafness and residual hearing represent p…

[Caloric stimulation of the vestibular organ after cochlear implant surgery].

BACKGROUND: Alterations of the vestibular organ after intracochlear insertion of the electrode array in cochlear implant (CI) surgery are estimated in the literature to be more than 30 % . By performing a most atraumatic electrode insertion following the "soft surgery technique" as described by Lehnhardt , the risk of postoperative vestibular dysfunction should be minimised. METHODS: In the present study we compared pre- and postoperative results of the caloric stimulation test in cochlear implant patients and analysed the influence of untimely opening of the perilymphatic space under cochleo

Cochlear implants and electrical brainstem stimulation in sensorineural hearing loss.

Cochlear implants and multichannel auditory brainstem implants enable patients with bilateral total or profound hearing loss to receive at least acoustic information. Both types of prosthesis are based on electrical stimulation of the auditory pathway. Different speech coding strategies and the number of electrodes used may influence the postoperative results. The preoperative evaluation of patients is of utmost importance. The cochlear implant is suitable for patients with hearing loss due to inner ear disorders, but who have functioning hearing nerve. Patients with a defect of the hearing ne

Comprehension of language in congenitally deaf children with and without cochlear implants.

The consequences of profound early deafness on oral language in children are drastic. The modern cochlear implant (CI) has been shown to enhance speech production skills in prelingually deaf children. Many factors may contribute to a poor or an excellent outcome, making it difficult to compare groups of children wearing or not wearing cochlear implants. The present study compared receptive language levels in matched pairs of children from CI group and non-CI groups. The pre-op receptive language development curve suggest a possible growth over time with the maturation and the speech therapy. C

Histopathological features of a cochlear implant and otogenic meningitis in Mondini dysplasia.

The temporal bones of a 6-year-old boy with a cochlear implant for profound hearing loss associated with Mondini dysplasia were studied histopathologically. Despite having severe Mondini dysplasia, he was able to detect sound with the implant. On histological examination of the temporal bone, he had more than 10000 spiral ganglion cells. Histopathological changes in the inner ear associated with the cochlear implant were minimal. Patients with severe Mondini dysplasia and profound hearing loss may, therefore, benefit from cochlear implantation. In the contralateral ear, the patient had suppura…

Successful revision of failed cochlear implants in severe labyrinthitis ossificans.

Labyrinthitis ossificans may complicate the insertion of a multichannel cochlear implant in patients deafened after meningitis. Two children who initially underwent partial insertion of a 22-channel cochlear implant because of severe cochlear ossification required revision surgery after several months of unsuccessful device use. At the time of revision, resection of the car canal, tympanic membrane, malleus, and incus provided access to the lateral wall of the cochlea, permitting extensive drilling of the basal turn and a circumodiolar placement of the electrode. Functional integrity of the el
