Primary eye care skills scores for health workers in routine and enhanced supervision settings.

INTRODUCTION: Primary health care in Tanzania is provided at two types of health units, the dispensary and the health centre. Theoretically, primary health workers (with knowledge of primary eye care [PEC]) are ideally placed to identify people in need of eye care services. In Tanzania, they are expected to be able to identify, treat, or correctly refer a number of eye conditions including cataract, trauma, presbyopia, and the 'red eye'. They are also expected to be able to measure visual acuity correctly and to educate the community about prevention. OBJECTIVES: The objective was to determin

Emotional illness in a pediatric population in Nigeria.

In a survey of 500 children aged between 5 and 15 years in a small rural community in Nigeria, 15 per cent were found to suffer from mental morbidity. Disturbances of emotion and conduct disorder constituted 67 per cent of total morbidity detected. Children from disrupted families (due to divorce, separation, and widowhood) were found to be significantly more likely to suffer from psychiatric morbidity. The need to train primary health care workers, and for them to work closely with families and school teachers in the provision of community mental health care services for children in the rural

Impact of proactive community case management of malaria on malaria prevalence in Bankass, Mali: a cluster-randomised controlled trial.

BACKGROUND: Proactive community case management (ProCCM) of malaria is a strategy to improve access to prompt and effective case management of malaria at the community level by supporting community health workers (CHWs) to visit every household in their community every 1 to 2 weeks to identify people with malaria symptoms, offer rapid diagnostic tests, and first line malaria treatment for those with positive tests. We sought to determine if this strategy could decrease malaria parasite prevalence in high malaria transmission settings. METHODS: CHWs in the intervention (ProCCM) arm were asked …

Pandemic-Related Funding Termination and Structural Drivers of Inequities: A Case Study of a Promotores/as-Focused Community-Based Organization.

During the COVID-19 pandemic, governmental agencies expanded safety net programs to ameliorate profound economic suffering. However, structural challenges excluded many low-income, immigrant, and racially minoritized communities, deepening inequities. Promotores/as and community-based organizations (CBOs), whose models focus on addressing structural drivers of inequities, were vital to communities navigating these challenges. This analytic essay examines how terminating pandemic-related funding strained CBOs and the communities they serve. Through a case study of a midsized promotores/as model

Evaluation of accuracy of rastreometer: a new equipment for hypertension tracking.

BACKGROUND: Hypertension is one of the major cardiovascular risk factors affecting more than one billion people. The low awareness of the disease is one of the greatest difficulties related to hypertension. The population screening for hypertension is widely recognized as an effective strategy to detect new cases, but until now, there is no instrument that has become feasible for mass screening of the population. OBJECTIVE: Evaluate the accuracy and reproducibility of the Rastreometer, a new instrument for the screening of hypertension for community health agents. METHODS: Cross-sectional st

Talking about health at work: a qualitative study of worker disclosures to co-workers.

PURPOSE: Co-workers can be important sources of support for workers with chronic, disabling health conditions. However, research on disclosure has largely focused on supervisors. This study examined health disclosure decisions to co-workers, what was shared, why, when, and contextual factors guiding decisions. METHODS: We conducted 25 semi-structured interviews with workers living with chronic physical and/or mental health conditions that impacted work at least some of the time. We used reflexive thematic analysis to identify themes in the data. FINDINGS: Sixteen women and nine men with dive

Assessing the hygienic disposal of patient stools among health workers at the Sunyani teaching hospital, Ghana.

Improper hygienic disposal of patient stools in healthcare settings presents considerable dangers to infection management and the well-being of healthcare personnel, patients, and the local community. Although health workers play an essential role in ensuring proper sanitation and preventing infections, investigations have revealed shortcomings in adherence to stool disposal protocols in numerous healthcare environments, especially in low- and middle-income nations such as Ghana. The study therefore assessed the hygienic disposal of patient stools among health workers at the Sunyani Teaching H

Haiti. Educating factory workers.

There are approximately 50,000 workers employed in the light assembly industry in Haiti. About 70% are women, the majority of whom are aged between 25 and 34 years, and are either single or in a nonpermanent relationship with the father of their children. Many live and work in appalling conditions, surviving on very low wages to support several children and an extended family. The acquired immunodeficiency syndrome (AIDS) is now a visible problem in many factories. In October 1988, the Center for the Promotion of Women Workers (Centre de Promotion des Femmes Ouvriers/CPFO) launched a pilot AID

Women's experiences of communication and supportive care during labour: a qualitative study in rural KwaZulu-Natal, South Africa.

BACKGROUND: Quality maternal care is crucial to improve outcomes for both mothers and newborns. Many initiatives to improve maternal care concentrate on improving clinical practice. However, women's experiences of care are also important determinants of health outcomes. Establishing strong interpersonal relationships between health workers and women is essential for delivering high-quality person-centered care, with health workers who empathize with women, respect their needs and concerns, and communicate effectively. Aligned with the World Health Organization standards of care framework, this

SARS-CoV-2 antibodies in Ontario health care workers during and after the first wave of the pandemic: a cohort study.

BACKGROUND: Health care workers have a critical role in the pandemic response to COVID-19 and may be at increased risk of infection. The objective of this study was to assess the seroprevalence of SARS-CoV-2 immunoglobulin G (IgG) antibodies among health care workers during and after the first wave of the pandemic. METHODS: We conducted a prospective multicentre cohort study involving health care workers in Ontario, Canada, to detect IgG antibodies against SARS-CoV-2. Blood samples and self-reported questionnaires were obtained at enrolment, at 6 weeks and at 12 weeks. A community hospital, t

Healthcare Seeking and Access to Care for Pneumonia, Sepsis, Meningitis, and Malaria in Rural Gambia.

Children with acute infectious diseases may not present to health facilities, particularly in low-income countries. We investigated healthcare seeking using a cross-sectional community survey, health facility-based exit interviews, and interviews with customers of private pharmacies in 2014 in Upper River Region (URR) The Gambia, within the Basse Health & Demographic Surveillance System. We estimated access to care using surveillance data from 2008 to 2017 calculating disease incidence versus distance to the nearest health facility. In the facility-based survey, children and adult patients sou

Understanding caregivers' and community influencers' perspectives on the barriers to childhood immunisation in Northern Nigerian States with public-private partnerships in routine immunisation programme.

BACKGROUND: Despite investments by the Nigerian government and international organisations in childhood immunisation to combat child mortality, coverage in many northern states remains below the national average, thereby increasing the risk of vaccine-preventable diseases. This paper examines the barriers to immunisation in six states in northeast and northwest Nigeria, which have the lowest vaccination coverage rates in the country. METHOD: We conducted 24 focus group discussions (FGDs) with mothers/caregivers and community influencers who collaborate with health workers to provide routine i

Access to and use of preventive intermittent treatment for Malaria during pregnancy: A qualitative study in the Chókwè district, Southern Mozambique.

BACKGROUND: Malaria remains a significant health problem in Mozambique, particularly in the case of pregnant women and children less than five years old. Intermittent preventive treatment with sulfadoxine-pyrimethamine (IPT-SP) is recommended for preventing malaria in pregnancy (MiP). Despite the widespread use and cost-effectiveness of IPTp-SP, coverage remains low. In this study, we explored factors limiting access to and use of IPTp-SP in a rural part of Mozambique. METHODS AND FINDINGS: We performed a qualitative study using semi-structured interviews to collect data from 46 pregnant wome
