Efficacy of A Virtual Agent-Based Digital Intervention (Echo App V2.0) for Patients With Alcohol Use Disorder: Randomized Controlled Trial.

BACKGROUND: Virtual agents powered by dialogue trees have emerged as promising tools to overcome limitations of text-only psychological interventions in mental health treatment. OBJECTIVE: This study evaluated whether a virtual agent-based digital psychological intervention plus treatment as usual (TAU) reduced craving and relapse and improved emotional state, sleep quality, and treatment motivation compared with TAU alone in patients with alcohol use disorder (AUD). METHODS: A 2-arm, randomized controlled trial with a parallel-group design was conducted at Fuyang No. 3 People's Hospital bet…

Differences in anxiety, depression, and heart rate variability between patients with methamphetamine use disorder and heroin use disorder patients on methadone maintenance treatment.

INTRODUCTION: Long-term substance abuse is associated with impairments in both psychological well-being and physiological indicators in patients. However, the mechanisms of impairment may differ between patients with Methamphetamine Use Disorder (MUD) and those with Heroin Use Disorder (HUD) receiving Methadone Maintenance Treatment (MMT). This study compared differences in anxiety, depression, and Heart Rate Variability (HRV) among patients with MUD, patients with HUD on MMT, and healthy controls (HCs), and examined whether these differences were associated with craving and treatment-intensit…

Changes in appetite and weight during the pharmacological treatment of patients with panic disorder.

Antidepressants reportedly increase appetite, carbohydrate craving and weight in some patients with depression and panic disorder. This paper presents the results of changes in appetite and weight in patients with panic disorder treated with imipramine, diazepam or placebo in a double-blind prospective study. Forty-four patients with panic disorder were randomly assigned imipramine, diazepam and placebo. The investigators monitored weight and the patients rated their appetite and cravings during the eight week study. No significant increases in weight or appetite were found in the patients wit…

Examining the association between social appearance anxiety and perceived social support in adolescents with butane explosion-induced burns.

BACKGROUND: Butane misuse in adolescents causes severe burns with long-term effects, but psychosocial impacts like social appearance anxiety (SAA) and its link to perceived social support are poorly understood, and relapse/craving predictors are unknown. This study aimed to assess SAA, identify factors, examine SAA-support association, and determine relapse predictors at 4 weeks post-injury. METHODS: A cross-sectional study was conducted on 48 adolescents in Northeast China who suffered from severe butane-related burns. At the 4-week mark, the participants were classified into the Non-relapse…

The Biased Reaction and Higher Cortical Coherence Under Smoking Cue in Young Smokers.

INTRODUCTION: Nicotine addiction is a widespread problem globally, causing millions of deaths and illnesses every year. People who start smoking at a young age are more likely to become addicted to nicotine. The cue-induced craving paradigm is used to study the nervous system related to nicotine dependence. The purpose of this study is to compare the coherence changes in functional connectivity in response to smoking stimuli between smokers and non-smokers. METHODS: We recruited 32 young smokers and age-matched non-smokers. We calculated the inter-hemispheric and intra-hemispheric coherences …

Education in addictive behaviors: The effects of the 'DIPENDE!' video with early adolescents.

BACKGROUND: The present study investigated the efficacy of the "DIPENDE!" video in middle school students. We hypothesized that exposure to the video in the Experimental Group (EG) would (a) increase correct cognitions about addictions (towards alcohol, nicotine, and problematic smartphone use), (b) reduce intentions to engage in these behaviours in risky situations, and (c) not produce any unintended increase in craving. METHODS: An environmental sustainability video was developed for the Control Group (CG). Both videos were designed according to the conceptual change model within a metacogn…

Mephentermine misuse: an emerging challenge and the potential role of mirtazapine-a case report.

Mephentermine is a sympathomimetic amine with structural and pharmacological similarities to amphetamines. Despite its increasing misuse, particularly among athletes and bodybuilders, there is limited literature on its management. No pharmacological treatments have been approved for mephentermine misuse. We describe two cases of young adult males presenting with symptoms of mephentermine withdrawal, including insomnia, anxiety, and craving. Both had histories of prolonged intravenous mephentermine use, with one managed on an outpatient basis and the other as an inpatient. Mirtazapine was initi…

Cue reactivity and opioid blockade in amphetamine dependence: A randomized, controlled fMRI study.

BACKGROUND: The opioid antagonist, naltrexone, has been shown to reduce the risk of relapse in amphetamine dependence, but the mechanisms behind this effect are not well understood. We aimed to investigate if naltrexone attenuates cue reactivity and craving in amphetamine dependence. METHODS: Forty men with severe, intravenous amphetamine dependence were randomized to one dose of naltrexone (50 mg) or placebo. In a BOLD fMRI cue reactivity paradigm, they were exposed to drug-related and neutral films and gave subjective ratings of craving after each film. Twenty-nine patients left data of suf…

Transcranial direct current stimulation modulates ERP-indexed inhibitory control and reduces food consumption.

Food craving can be defined as the "urge to eat a specific food". Previous findings suggest impairment of inhibitory control, specifically a regulatory deficit in the lateral prefrontal circuitry that is associated with a compulsion for food. As demonstrated by three previous studies, bilateral transcranial direct current stimulation (tDCS) of the dorsolateral prefrontal cortex (DLPFC) (anode right/cathode left) reduces food craving and caloric intake. We designed the present study to evaluate the neural mechanisms that underlie these effects. We replicated the design of one of these previous …

The effects of lamotrigine on alcohol seeking and relapse.

Lamotrigine is a clinically used drug, which inhibits Na(+) channel activity that in turn reduces glutamate release. Its downstream activity on other neurotransmitter systems such as serotonergic and dopaminergic has also been reported. Since both glutamate and monoamines might be involved in alcohol craving and relapse, our aim was to examine the effects of lamotrigine on alcohol seeking and relapse-like drinking behaviour. Thus, lamotrigine (5 and 15mg/kg) was tested in two behavioural models for alcohol seeking and relapse - the model of cue-induced reinstatement of alcohol-seeking behaviou…

Glycemic Improvement with Low-Dose Dulaglutide Is Associated with Leptin and Obestatin Modulation in Type 2 Diabetes Mellitus.

BACKGRUOUND: Glucagon-like peptide-1 receptor agonists (GLP-1 RAs) improve glycemic control through insulinotropic and anorectic effects. However, the role of adipokines and appetite-related hormones in mediating the glycemic response remains unclear. This study evaluated changes in abdominal fat, food cravings, and circulating adipokines and gut hormones following dulaglutide treatment and identified predictors of glycemic improvement in type 2 diabetes mellitus (T2DM). METHODS: In this 24-week prospective observational study, 82 patients with T2DM and glycosylated hemoglobin (HbA1c) levels …

FRONTAL WHITE MATTER CHANGES INDICATE RECOVERY WITH INPATIENT TREATMENT IN HEROIN ADDICTION.

IMPORTANCE: Amidst an unprecedented opioid epidemic, identifying neurobiological correlates of change with medication-assisted treatment of heroin use disorder is imperative. Distributed white matter (WM) impairments in individuals with heroin use disorder (iHUD) have been associated with increased drug craving, a reliable predictor of treatment outcomes. However, little is known about the extent of whole-brain structural connectivity changes with inpatient treatment and abstinence in iHUD. OBJECTIVE: To assess WM microstructure and associations with drug craving changes with inpatient treatm…
