Modulation of cyst growth in autosomal dominant polycystic kidney disease: mechanistic insights and therapeutic opportunities.

Autosomal dominant polycystic kidney disease (ADPKD) is one of the most common inherited kidney disorders and is characterized by the progressive formation and expansion of fluid-filled cysts, ultimately leading to kidney failure. Although caused by reduced dosage of the polycystin proteins, the disease phenotype arises from a broad disruption of epithelial physiology rather than a single linear pathway. Loss of polycystin function destabilizes epithelial homeostasis and sensitizes cyst-lining cells to proliferative and secretory cues. A central consequence is the emergence of a self-reinforci

A novel ultrasonic technique for differentiating cysts from solid lesions: preliminary results in the breast.

The feasibility of a new ultrasonic technique to distinguish cysts from solid lesions is explored. High intensity pulses are used to induce acoustic streaming in cyst fluid, and this motion is detected using Doppler techniques. Acoustic streaming cannot be generated in solid lesions, therefore, its detection would indicate a cyst. In six of seven breast cysts motion was clearly generated and detected in vivo. Ultrasonic pulses with intensities up to 4.4 W cm-2 (I(spta) in water) were focused on the cysts for 10 s. Lesion diameters ranged from 0.6 to 2.5 cm; induced flow velocities were less th…

Incidental optic nerve sheath arachnoid cyst: A rare case finding with literature review.

Arachnoid cysts are the most common benign cystic abnormalities formed due to congenital splitting of the arachnoid layer. They comprise 1% of intracranial masses, and the orbital location is even more rarely reported in history especially in the pediatric population. They might be discovered as an asymptomatic finding on imaging performed for a concomitant condition or, in most reported cases, as a result of ophthalmic impairment. They can be isolated or associated with gliomas, neurofibromas, empty sella syndrome, and frontotemporal porencephalic cysts. Computed tomography scan shows a non-e

Symptomatic Cavum Septum Pellucidum Cyst: A Rare Presentation.

A cavum septum pellucidum is a cerebrospinal fluid (CSF) filled cavity situated between the lateral ventricles and is considered as a normal anatomic variant sporadically seen on neuroimaging. While a cavum septum pellucidum is a relatively uncommon incidental neuroimaging finding, symptomatic cysts of the cavum septum pellucidum are very rare, with only a few cases reported in the literature so far. They are defined as fluid-filled structures with lateral bowing of the walls and membranes separated by at least 10 mm or more. We present the case of a 25-year-old male patient with a rapidly exp

Gastric duplication cyst masquerading as a mucinous pancreatic cyst: case report and literature review.

Gastric duplication cysts are rare cystic neoplasms that are often difficult to distinguish from other entities. We describe a healthy 44-year-old woman who presented with acute right lower quadrant abdominal and flank pain as well as chronic nausea and constipation. Her physical examination was unremarkable but contrasted computed tomography revealed a 6 cm cystic lesion between the stomach and body of the pancreas. Endoscopic ultrasonography and fluid analysis were consistent with a mucinous cyst with a markedly elevated fluid carcinoembryonic antigen level. The patient subsequently underwen

Recent advances in the understanding of polycystic kidney disease.

Polycystic kidney disease is characterized by localized autonomous cellular proliferation, compartmentalized fluid accumulation within the cysts, and intraparenchymal fibrosis of the kidney. The clinical features include renal failure, liver cysts, and vascular and cardiac valve abnormalities. Recent developments have extended our understanding of cyst formation, fluid secretion, and the genetics of polycystic kidney disease. Two causal genes for polycystic kidney disease, PKD1 and PKD2, that are responsible for greater than 95% of cases of autosomal dominant polycystic kidney disease, have be

False-positive iodine-131 whole-body scans due to cholecystitis and sebaceous cyst.

False-positive whole-body 131I scans are not frequent but have serious consequences in the management of patients with thyroid cancer. They can be classified in four main groups: elimination of iodine in body fluids, infection or inflammation, cysts or transudates and nonthyroid tumors. We report on two patients with false-positive post-therapy 131I scans. The first patient had uptake projected in the right pelvic area which was later proven to be a large gluteal sebaceous cyst. The second patient had uptake in the gallbladder area that did not disappear after 131I treatment; she underwent exp

Fluid-fluid Levels in Musculoskeletal Tumor Imaging.

Fluid-fluid levels result from the separation of two fluids of differing densities within a cavernous space with the boundary between the two layers running in a horizontal plane at 90 degrees to the direction of gravity. Magnetic resonance imaging is the most sensitive imaging modality to identify fluid-fluid levels. Although the most common bone lesions containing fluid-fluid levels are aneurysmal bone cyst and telangiectatic osteosarcoma, fluid-fluid levels can be observed in a wide variety of bone and soft tissue lesions. Therefore, fluid-fluid levels cannot be considered diagnostic of any

A Brief Review on the Regulatory Roles of MicroRNAs in Cystic Diseases and Their Use as Potential Biomarkers.

miRNAs are small endogenous conserved non-coding RNA molecules that regulate post-transcriptional gene expression through mRNA degradation or translational inhibition, modulating nearly 60% of human genes. Cystic diseases are characterized by the presence of abnormal fluid-filled sacs in the body, and though most cysts are benign, they can grow inside tumors and turn malignant. Recent evidence has revealed that the aberrant expression of a number of miRNAs present in extracellular fluids, including plasma or serum, urine, saliva, follicular fluid, and semen, contribute to different cystic path

Effect of uterine inflammation and ultrasonically-detected uterine pathology on fertility in the mare.

The incidence of intrauterine fluid collections during dioestrus (12/43, 28%) and uterine cysts throughout the oestrous cycle (11/73, 15%) found in this study indicates that these ultrasonically detectable abnormalities are prevalent in mares. The hypothesis that uterine cysts do not affect pregnancy was not supported. Intrauterine fluid collections at dioestrus represented the presence of an inflammatory process as indicated by a high biopsy score, reduced progesterone concentrations, and a shorter interovulatory interval. Mares with fluid collections at dioestrus had a lower pregnancy rate a

Suprasellar arachnoid cyst causing pituitary stalk compression and endocrine dysfunction in a child: MRI findings.

Suprasellar arachnoid cysts are rare congenital lesions that may cause neurological, visual, or endocrine manifestations depending on their size and anatomical relationships with the hypothalamic-pituitary axis. Endocrine dysfunction in such cases is clinically significant as it may reflect hypothalamic-pituitary axis impairment and requires early recognition and management to prevent long-term sequelae We report the case of a 5-year-old girl presenting with endocrine abnormalities including growth hormone deficiency. Magnetic resonance imaging (MRI) of the hypothalamic-pituitary region reveal

Simple bone cyst of the hyoid: A radiological diagnosis and follow-up.

Simple bone cyst (SBC), also known as unicameral bone cyst, is a benign, fluid-filled cystic lesion that mainly appears in the long bones of young patients. Simple bone cysts are considered dysplastic or reactive lesions of bone. The most common locations for SBCs are the proximal humerus and femur. Lesions may occasionally be found in the jawbones. Simple bone cysts often have no clinical impact and are usually detected during routine radiographic examinations unless a gross pathologic fracture occurs. When symptoms develop, they may include mild pain, local tenderness and swelling. Computed

Infant subcortical cystic leucomalacia: a distinct pathological entity resulting from impaired fluid handling.

BACKGROUND: In the first months of life the subcortical white matter appears prone to fluid accumulation and cystic change. This condition has generally been considered to be due to hypoxic-ischaemic injury (HII) and is grouped with other forms of white matter disease including periventricular leucomalacia (PVL). AIMS: Our aim was to describe the sequential tissue changes in the formation of subcortical cystic leucomalacia in the infant brain and to delineate this from other forms of white matter disease in order to better understand its pathogenesis and aetiology. STUDY DESIGN: Standard sam

Adrenal steroid secretion in girls with pseudoprecocious puberty due to autonomous ovarian cysts.

To evaluate the role of adrenal steroids in pseudoprecocious puberty due to large ovarian follicular cysts, we studied the serum 17-hydroxyp … ne sulfate, and another unidentified steroid sulfate. These steroid sulfates were also found in the cyst fluid from the one patient from whom the fluid was obtained. These results suggest that steroid production by the adrenal gland may stimulate the development of small ovarian cysts (which may be present in normal prepubertal girls) into large ovarian cyst …

Pseudomalignant cytologic presentation of spermatic hydrocele fluid.

An unusual cytologic presentation of spermatic hydrocele fluid is reported. The aspirate contained highly atypical cells, exhibiting marked anisocytosis and nuclear abnormalities strongly suggestive of cancer. On histologic examination the lesion displayed pseudosarcomatous fasciitis in the cyst wall and foci of atypical reactive mesothelial hyperplasia in the lining. This case illustrates a potential pitfall in the cytology of spermatic hydrocele fluid.

Neonatal ovarian cysts: sonographic-pathologic correlation.

The authors reviewed the prenatal (11 infants) and postnatal (17 infants) sonograms and the clinical, surgical, and pathologic findings in 17 infants with an ovarian cyst to determine the sonographic features and natural history of neonatal ovarian cysts. An uncomplicated cyst (nontwisted, nonhemorrhagic) was completely anechoic and the cyst wall was imperceptible with sonography (five cases). A twisted or hemorrhagic cyst was cystic with a fluid-debris level, cystic with a retracting clot, septated with or without internal echoes, or solid (12 cases). These complicated cysts contained liquid

Bilateral cystic rete testis in an alpaca (Lama pacos).

A 9-year-old intact male alpaca (Lama pacos) was examined because of marked enlargement of the left scrotum. Ultrasound examination revealed a thin-walled anechoic structure in the area of the left testis. Aspirated fluid contained spermatozoa, many of which had abnormal morphology. Castration was performed and the left testis was markedly enlarged with a clear fluid-filled cyst. The cyst was lined by a single layer of squamous to cuboidal epithelial cells consistent with those originating from rete testis. The right testis was of a comparable size and shape to that of normal alpaca testis, bu

Reattachment of retina and retinoschisis in pit-macular syndrome by surgically-induced vitreous detachment and gas tamponade.

The origin of the subretinal fluid in pit-macular syndrome is unknown. Using optical coherence tomography, the authors observed that an optic disk pit is not a true pit but a cystic cavity covered with a superficial layer of the optic disk in a patient with retinal detachment and retinoschisis. The cyst was connected to the intraretinal space of the retina, which was split in several layers, or the retinoschisis in the papillomacular area. Vitrectomy and gas tamponade were performed to treat the patient. During surgically-induced vitreous detachment, strong vitreoretinal attachment at the disk

Percutaneous fluoroscopic lumbar facet joint synovial cyst aspiration for manifesting with radiculopathy and low back pain.

Lumbar facet joint synovial cysts are benign degenerative abnormalities of the lumbar spine and can cause lower extremity ra-diculopathy, spinal stenosis, and low back pain. Herein, we report a case with a synovial cyst treated by percutaneous fluoros-copic aspiration via the facet joint. A 46-year-old woman presented to the neurosurgery clinic complaining of a 2-month history of low back pain with left-sided radicular symptoms. Her physical examination was consistent with a left L5 radiculopathy, and MRI confirmed a left L5-S1 facet joint synovial cyst compressing the nerve root. Percutaneous
