Colostomy in children--an evaluation of acceptance among mothers and caregivers in a developing country.

BACKGROUND: Colostomy is a common procedure performed in children, mostly for anorectal malformation and Hirschsprung's disease. However, acceptance by parents in developing countries is thought to be poor. METHODS: A prospective evaluation was done to determine the acceptability of colostomy among parents and caregivers, and their attitude towards colostomy in their children. RESULTS: During the period January 1999 - January 2004 a structured questionnaire was administered to 57 parents and caregivers of children with colostomies (41 boys and 16 girls). The indication for colostomy was anor…

Acute renal failure in pregnancy in a developing country: twenty years of experience.

UNLABELLED: Acute renal failure (ARF) has become a rare complication of pregnancy in developed countries. The aim of this study was to describe changing trends in pregnancy-related acute renal failure (PR-ARF) in two successive periods; 1982-1991 and 1992-2002. From July 1982 to December 2002, 190 cases of PR-ARF were observed in Eastern India (11.6% of total number of ARF needing dialysis). Obstetrical complications were causative factors for ARF in 15% (65/426) and 10% (125/1201) of patients in the two periods, respectively. The incidence of PR-ARF fell from 15% in 1982-1991 to 10% in 1992-2

Immunization strategies for the control of serogroup C meningococcal disease in developed countries.

In developed countries, the epidemiology of serogroup C meningococcal disease is characterized by unpredictable outbreaks and a bimodal distribution of cases, with the highest incidence rate among those below 1 year of age and a second peak in teenagers. Serogroup C meningococcal conjugate vaccines elicit a thymus-dependent immunological response that is already present in young infants and is characterized by the production of protective antibodies and the development of memory. Results from immunogenicity and effectiveness studies indicate that waning of immunity occurs over time, and the pr

Early onset of endocrine abnormalities in beta-thalassemia major in a developing country.

Endocrine complications in patients with thalassemia major in developing countries may be frequent due to suboptimal iron chelation. Data from developing countries are scant. We prospectively evaluated growth, growth hormone (GH), insulin-like growth factor I, thyroid hormone, cortisol and glucose tolerance in 84 patients over one year. Height standard deviation (SD) score of patients > 8 years (-2.2 +/- 1.5 against National Center for Health Statistics references) was significantly lower than that of normal controls (-1.0 +/- 0.7, p < 0.001). 51% of patients had GH deficiency, 13% hypocortiso…

Evaluation of trauma care in a developing country highlighted by a major aircraft accident.

The aim of this study was to evaluate the day-to-day trauma care in a developing country highlighted by a major accident. In this accident, early management was not carried out according to triage principles. Scene mortality and in-hospital mortality were 72% (n = 55) and 14% (n = 3), respectively. Overall mortality rate was 76%. Five survivors were minor wounded. Three laparotomies, one thoracotomy and three tube thoracostomies were performed in the acute phase. Skeletal injuries, mainly rib fractures (43.3%) and haemothorax (10.8%), were the most frequent pathologies seen. One liver lacerati

Developed countries should use inactivated polio vaccine for the prevention of poliomyelitis.

As the public health situation regarding polio changes, we should not be afraid to change our thinking about vaccination policy. OPV is still the vaccine of choice to eradicate wild poliomyelitis from the developing world. However, in developed countries free of poliomyelitis there is no longer a necessity to tolerate VAPP as the consequence of vaccination. IPV as part of combination vaccines or in mixed schedules with OPV is preferable and is in process of becoming the norm for those countries. Copyright 1997 John Wiley & Sons, Ltd.

Nosocomial tuberculosis in the AIDS era: strategies for interrupting transmission in developed countries.

In developed countries where tuberculosis is increasing in association with HIV infection, nosocomial transmission among patients and from patients to health care workers is being increasingly reported. Nosocomial tuberculosis among HIV-infected patients is difficult to prevent by conventional control measures because the clinical presentation of the disease may be atypical and confused with other respiratory infections, because the tuberculin skin test is less reliable, because cough generating procedures may increase the probability of transmission, because HIV-infected patients may progress

The incidence and development of resistance in Staphylococcus aureus from three European countries.

In a multicentre study conducted by the Paul Ehrlich Society we tested 3482 Staphylococcus aureus strains, isolated in different hospitals in West Germany, Austria and Switzerland, against 12 antibiotics with standardized techniques between 1976 and 1982. In order to avoid mistakes in the classification of the strains into sensitive, intermediate and resistant, we adapted breakpoints to the population distribution. There were no marked changes in the incidence of sensitive strains during the period. Striking differences from one hospital to another were observed. The percentage of strains sens

The incomplete cone in carcinoma in situ of the cervix: A prospective study in a developing country.

The diagnostic procedures and treatment of carcinoma in situ and severe dysplasia of the cervix are becoming more conservative. In a developing country special problems make such an approach more hazardous. During a 15-month period the diagnosis in 25 of 206 patients (12,1%) with a smear positive for carcinoma in situ could not be confirmed histologically at Tygerberg Hospital. A prospective study of 147 cases (71,4%) in which the diagnosis was confirmed revealed that they were mostly young patients of relatively high gravidity. The difficulty of assessing the completeness of a cervical cone a

Causes of blindness among students in blind school institutions in a developing country.

Out of 270 students in 17 blind school institutions in Malawi 73 per cent were blind before the age of three. The most common cause for the blindness was ocular infection (75-2 per cent). Meales, as a single cause, was responsible for 43-7 per cent of the cases and smallpox for 5-2 per cent. Bacterial infections were incriminated in 26-3 per cent of the cases. Most of these had received traditional medicine during the acute phase of the disease. Hereditary factors as causes of blindness were found in 7-8 per cent of the cases. These included congenital cataracts (2-6 per cent), optic atorphy o

Role of the medical auxiliary in the control of sexually transmitted disease in a developing country.

Venereal diseases are becoming a major health problem in many developing countries where the greater part of primary medical care is undertaken by medical auxiliaries. Under these circumstances, the medical auxiliary has an important role to play in the control of these diseases but he can only do this with adequate training, support, and supervision from the professional doctors and specialists. In this paper, the role of the medical auxiliary is outlined and a case is also made for a specially-trained cadre for venereal disease work in busy urban clinics in developing countries.

Comparison of mucosal B- and T-cell responses in Helicobacter pylori-infected subjects in a developing and a developed country.

Helicobacter pylori is highly endemic in developing countries, but comparatively little is known about mucosal immune responses to H. pylori in these settings. Therefore, we have compared B- and T-cell responses, with a focus on the gastrointestinal mucosa, in H. pylori-infected adults in a developing (Bangladesh) and a developed (Sweden) country. We found comparable numbers of CD19(+) B cells and CD4 (+)T cells and similar levels of H. pylori-specific IgA antibodies in gastric mucosa from Bangladeshi and Swedish volunteers. However, about threefold higher numbers of CD19(+) B cells and 12-fol…

Deployment of Low-Latency Satellite Technology for Telemedicine: A Backhaul Solution for Improving Rural Health Care Connectivity in Developing Countries.

BACKGROUND: In developing countries, satellite-based technology can aid critical telemedicine applications and other digital health services in critically underserved areas. Affordable, high-speed broadband services can and should be accessible to all citizens. Remote locations are necessary to support various critical services, including education and training, telehealth applications, remote patient monitoring, and warning systems, particularly during disasters. Currently, however, these services are limited to urban centers, leaving rural areas without access to specialized health care serv…
