BACKGROUND: With the growing adoption of nonoperative management of complicated diverticulitis, its postdischarge consequences remain unclear. This study aimed to compare the readmission outcomes between operative and nonoperative management of patients with complicated diverticulitis.
METHODS: This is a retrospective cohort study of the 2020-2021 Nationwide Readmission Database, including adult patients (≥18 years) with acute complicated diverticulitis, defined by the presence of concomitant abscess formation, obstruction, or perforation. Patients were stratified by management approach-nonop

BACKGROUND: Diverticulitis is a common medical condition that disproportionately affects older adults. The ideal management of recurrent diverticulitis, including the role of prophylactic colectomy, remains uncertain.
OBJECTIVE: This study aimed to investigate the outcomes among older patients undergoing elective surgery for diverticulitis and examine subgroups of patients with comorbid congestive heart failure and chronic obstructive pulmonary disease to determine whether outcomes in these patients are worse than in other groups.
DESIGN: This article reports a retrospective cohort study of

A 38-year-old para 2 presented with the gradual onset of nausea vomiting and increasing left lower quadrant pain, at 33 weeks' gestation. She was known to have uterine leiomyomas, with ultrasonography depicting an 8-cm intramural fundal leiomyoma. In addition a left lateral nondiscrete 10 x 8-cm mass was depicted at the point of maximum tenderness. Magnetic resonance imaging (MRI) demonstrated diverticulosis of the descending and sigmoid colon. The patient remained afebrile and received repeated doses of intramuscular analgesics and was cleared by the surgical consultant, only to be readmitted

Expedient recognition of arterial-enteric fistulas is important in optimizing patient outcomes. The most commonly described aortoenteric fistula is between the abdominal aorta and third portion of the duodenum that overlies it. This has historically been reported as a rare complication of open aortic aneurysm repair but also has been seen in endovascular repairs. Herein described is a case of a 77-year-old male with history of endovascular aortic repair and left hypogastric coiling for aneurysm who presented with a much rarer form of fistula between the residual sigmoid colon, status-post sigm

Epiploic appendagitis is a rare, often misdiagnosed condition that causes acute abdominal pain. The symptoms, such as localized pain that worsens with coughing and stretching, mimic other conditions like appendicitis and diverticulitis. Diagnosis can be made using computed tomography (CT) scans, which show characteristic signs, such as a 2-3 cm fat-density ring, colon wall thickening, and nearby fluid or inflammation. The condition usually resolves naturally or can be treated with nonsteroidal anti-inflammatory drugs (NSAIDs). In this report, a case of a 37-year-old man diagnosed with epiploic

Accidental ingestion of foreign bodies can be a common occurrence in clinical practice and subsequent perforation is quite rare. The rate of fish bone ingestion is predicted to be higher in some cultures given culinary differences. We report a case of 62-year-old gentleman in rural Australia who had severe left sided abdominal pain and fevers, with a presumptive diagnosis of diverticulitis. Computated tomography revealed intraabdominal abscess. Surgical intervention via laparotomy and right hemicolectomy was performed for an extensive abdominal abscess with a transverse colon perforation and f

Non-traumatic perforation of the digestive tract occurs most often in the duodenum and stomach (peptic ulcer), as well as the colon (diverticulitis, cancer or ischemic lesions). Perforation of the small bowel is very rare. The Authors of the study presented a case of proximal jejunum perforation, which occurred in a patient with a history of duodenal peptic ulcer disease. Diagnosis posed no difficulties, and treatment included the excision of the ulceration and suturing of the bowel. The patient recovered without complications and the histological examination failed to reveal the nature of the

The Ehlers-Danlos syndromes (EDS) are a group of connective tissue disorders characterized by triad of joint hypermobility, skin extensibility, and tissue fragility. Ehlers-Danlos syndrome type IV places patients at risk for life-threatening, spontaneous, vascular or visceral rupture due to reduced or abnormal secretion of type III collagen. We present an adolescent male who was found to have a perisigmoid abscess with a fistula connecting to adjacent sigmoid colon secondary to undiagnosed EDS type IV. Conservative management with antibiotics and bowel rest was pursued to allow for elective re

Perforation in the course of diverticulitis of the right side of the large bowel is a rare disease, which is mostly misinterpreted as appendicitis and generally disregarded in the differential diagnosis of pathological processes of the lower right abdomen. The results of radiological investigations (plane film, barium enema, CT) are demonstrated in two cases and diagnostic findings are discussed. Plane film of the abdomen allows only unspecific diagnosis of an inflammatory process in the right lower quadrant, barium enema reveals changes in the wall of the colon due to abscess formation. CT is

Aortocolic fistula occurs with spontaneous rupture of aortic and iliac aneurysms into the sigmoid colon, or due to involvement of the aneurysmal wall by acute diverticulitis. In the eight cases reviewed, this complication proved uniformly lethal, although sufficient clinical findings were present for diagnosis, and adequate time was available for a planned therapeutic approach. Lower gastrointestinal bleeding in the patient who has an aortic aneurysm and left-lower-quadrant inflammation suggests the presence of an aortocolic fistual. Angiography should be performed during a bleeding episode to

RATIONALE: Primary epiploic appendagitis (PEA) is a rare cause of acute abdomen caused by spontaneous torsion or venous thrombosis of epiploic appendices, it commonly manifests with acute lower quadrant pain, thus may mimic acute diverticulitis, appendicitis, or mesenteric infarction.
PATIENT CONCERNS: In this case report, we report a 44 years old man who presented with persistent sharp pain in the left lower quadrant abdomen, Laboratory tests were mostly normal, contrast enhanced computed tomography (CECT) revealed a slightly high density shadow with fat foci in the middle was presented arou

To date, the appropriate management of diverticular disease is still controversial. The American Society of Colon and Rectal Surgeons declared that the decision between conservative or surgical approach should be taken by a case-by-case evaluation. There is still lack of evidence in literature about long-term outcomes after elective sigmoid resection for diverticular disease. Considering the potentially key role of the surgical technique in long-term outcomes, there is the need for surgeons to define strict rules to standardize the surgical technique. Currently there are 5 areas of debate in e

BACKGROUND: Following colon resection, the construction of a well-perfused, tension-free isoperistaltic anastomosis can be made difficult by multiple factors including prior abdominal surgery or compromised vascular supply. Here, we describe the technique of antiperistaltic cecorectal anastomosis as a method for preserving viable colon without compromising functional outcome.
TECHNIQUE: Following extensive colorectal resection, different techniques for isoperistaltic reconstruction using the cecum and ascending colon have been described, including the Deloyers procedure and limited isoperista

Spontaneous ureterocolic fistula is rare and usually caused by urinary calculi. We present the fifth reported case of spontaneous ureterocolic fistula caused by diverticular disease of the colon. Review of these cases revealed a preponderance of women (3:1 ratio), with mean age of 77 years. These patients may have a protracted course before an accurate diagnosis is made (up to 10 years) because of the typical vague presentation. Urologic symptoms predominate, especially urinary tract infection (100%), fecaluria (75%), and abdominal (75%) or flank pain (50%). Barium enema is the most reliable d

Experience with 37 patients with adult colovesical fistula over the past 19 years is reviewed. Specific guidelines for treatment of adult colovesical fistula are influenced by the location and cause of the fistula, the patient's general condition, the presence of a pelvic abscess and the presence of colonic obstruction. When criteria are met, a one-stage procedure is safe. The two-stage approach should enjoy wider application, with the three-stage approach reserved for patients who are unprepared or who have a large pelvic abscess. In patients with colovesical fistula due to cancer, the extent
