Is the ulnar nerve damaged in 'handball goalie's elbow'?

'Handball goalie's elbow' has been defined as pain in the elbow region due to repetitive forced hyperextensions of the elbow. Goalkeepers are the players most often suffering from hyperextension trauma to the elbow in European team handball. They may complain of radiating pain or numbness in the ulnar aspect of the forearm in addition to local pain in the elbow region. To detect any injury to the ulnar nerve that could explain the symptoms, we performed a neurological and neurophysiological study in goalkeepers with elbow pain. Nine goalkeepers, with a total of 15 'handball goalie's elbow', we…

Nerve injuries of the elbow, wrist, and hand in athletes.

Upper extremity compression neuropathies are fairly rare in athletes. Initially, most can be managed conservatively. These conditions can follow direct contusion of the tissues that overlay these peripheral nerves or can result from vigorous, repetitive, athletic activity leading to tissue swelling and ischemia with nerve compression symptoms. A complete history and physical examination, including a neurologic examination, should be paramount when treating athletes with upper extremity injuries. Early diagnosis and treatment with conservative measures such as splinting, rest, activity modifica

Arthroscopic treatment of posterior impingement of the elbow in athletes.

Arthroscopic treatment of posterior impingement of the elbow in athletes consistently can improve comfort and function. Arthroscopic treatment is particularly valuable because of increased intra-articular visualization of the anterior and posterior compartments and diminished soft tissue trauma. Arthroscopic visualization also can help the surgeon assess small pathologic changes in joint congruity seen in subtle forms of ligamentous instability, which are often symptomatic in high-demand athletes. Treatment of these subtle instabilities can be individualized to the athlete. Most athletes are a…

Treatment of lateral epicondylitis of the elbow with shock waves.

In a prospective clinical study, the effectiveness of shock wave treatment for lateral epicondylitis in 56 elbows in 53 patients (27 men and 26 women) with an average age of 46 years was investigated. Three patients received treatment for both elbows. Each elbow was treated with 1,000 impulses of shock waves at 14 kV. A 100-point scoring system was used for evaluation including 40 points for pain, 30 points for function, 20 points for strength, and 10 points for elbow motion. The intensity of pain was measured using a visual analogue scale from 0 to 10. The overall results were 13.2% excellent…

[Overuse injury syndromes of the hand, forearm and elbow].

The basic role of the shoulder, upper arm, elbow, forearm, and wrist is to place the hand in the appropriate position, so that it can fulfill its function. The upper extremity, especially the wrist and the hand, has a very complex anatomy with a large number of bones, muscles, tendons and nerves. They are essential to work and sport activities and are subject to acute and/or chronic mechanical injuries. This is why overuse injuries are the most common in these regions. They are often characterised by tendinitis, tenosynovitis, tunnel syndromes, or stress fractures. This paper gives an overview…

[Arthroscopic therapy in limited mobility of the elbow joint].

Limitation of elbow mobility constitutes a grave problem for therapy. Arthroscopy offers a therapeutic option, but in cases of loss of motion poses a considerable challenge to the operative technique. Placement of the portals already carries an increased risk of neurovascular lesions due to the altered anatomy and reduced distension capacity of the joint. Thus, particular significance attaches to the standardized arthroscopic procedure for localization and placement of the cannulas, intra-articular assessment, differentiated evaluation of the dorsal joint regions, and operative tactics for tra…

The treatment of pulled elbow: a prospective randomized study.

To evaluate the effectiveness in decreasing recurrence of cast application after manual reduction of pulled elbow. Sixty-four children with pulled elbow were randomized into two treatment groups: Group A underwent manipulative reduction followed by splinting the elbow in a flexed and supinated position for 2 days; group B underwent manipulative reduction only. Both groups were examined 2, 5, and 10 days later. None of the 33 patients in group A had a pulled elbow at follow-up. Four (13%) of 31 patients in group B had a pulled elbow 2-5 days later. Immobilizing the elbow for 2 days after manipu…

Radiographic studies of the wrist and elbow in cerebral palsy.

We retrospectively studied and evaluated radiographs of the bilateral wrists and elbows in 96 patients with cerebral palsy. There were 55 patients (57.3%) with athetospastic quadriplegia, 30 (31.3%) with spastic diplegia, and 11 (11.4%) with spastic hemiplegia. Plain antero-posterior and lateral roentgenograms were taken of both wrists and both elbows. Overall negative ulnar variance was seen in 18.2% of wrists, and, the variance was highest in athetospastic quadriplegia. We could not find any case of Kienböck's disease. The radiolunate angle was negative in the wrists of those with athetospas…

[Closed elbow dislocation with rupture of the brachial artery].

The case of a closed elbow dislocation with complete rupture of the brachial artery and its veins is described. This combination of injuries is rather rare and only 25 cases were found in the literature. The sharp lesion of the vessels was remarkable, probably due to the edge of the distally ruptured capsule. Immediate revision with suture of the vessels and an immobilization for three weeks gave a good result.

[Dislocation of the elbow joint--therapy and treatment outcome].

At our hospital, closed dislocations of the elbow without neurovascular or osseous lesion are treated conservatively after successful reduction in the absence of a tendency to redislocation. Indications for surgery are irreducible dislocations and those with a tendency to redislocation as well as open soft tissue injuries, neurovascular and osseous concomitant injuries. During a period of 10 years 113 patients were treated according to this strategy. Of these patients, 46 had simple dislocations of the elbow and 67 suffered from dislocations with osseous lesions. In 95 patients follow-ups coul…

Lateral tennis elbow: "Is there any science out there?".

As orthopaedic surgeons, we are besieged by myths that guide our treatment of lateral epicondylitis, or "tennis elbow." This extends from the term used to describe the condition to the nonoperative and operative treatments as well. The term epicondylitis suggests an inflammatory cause; however, in all but 1 publication examining pathologic specimens of patients operated on for this condition, no evidence of acute or chronic inflammation is found. Numerous nonoperative modalities have been described for the treatment of lateral tennis elbow. Most are lacking in sound scientific rationale. This …

Elbow dysplasia in the dog: pathophysiology, diagnosis and control.

Elbow dysplasia is a non-specific term denoting abnormal development of the elbow. Elbow dysplasia encompasses the clinical and radiographic manifestation of ununited anconeal process, fragmented medical coronoid process, osteochondritis dissecans, erosive cartilage lesions and elbow incongruity. The net result is elbow arthrosis, which may be clinically inapparent or result in marked lameness. These conditions may be diagnosed by means of routine or special radiographic views and other imaging modalities, or the precise cause of the arthrosis or lameness may remain undetermined. Breeds most c…

The arthroscopic management of osteochondritis dissecans of the adolescent elbow.

The management of avascular necrosis of the capitellum of the adolescent elbow continues to be a dilemma. This article is a critical retrospective analysis of 12 pediatric patients (mean age at surgery 14.5 years) who underwent arthroscopic debridement alone followed by early range of motion. Follow-up at a mean of 3.2 years (range, 2.0 to 5.7 years) indicated that the average flexion contracture improved from 23 degrees preoperatively to 10 degrees postoperatively. All patients had remodeling of the capitellum by plain radiographs; however, five patients had associated enlargement of the radi…

The extraosseous and intraosseous arterial anatomy of the adult elbow.

UNLABELLED: We investigated the extraosseous and intraosseous arterial anatomy of the human adult elbow. Twenty-two fresh adult cadaveric upper extremities were studied with a technique of combined India-ink and latex injection followed by chemical débridement. The intraosseous vascularity of twelve extremities was then evaluated with a rapid Spalteholz clearing technique. Our findings demonstrated consistent patterns of extraosseous and intraosseous vascular anatomy, which were organized into three vascular arcades: medial, lateral, and posterior. The medial arcade was formed by the superior …

[Ulnar neuropathy at the elbow. Diagnosis and therapeutic criteria].

Ulnar neuropathy at the elbow (UNE) is described as a nerve injury arising from trauma. For practical management purposes, two general types of UNE have been defined. One group is comprised of those cases in which identifiable bone deformity is present at the elbow and the other comprises cases in which only nerve dysfunction is found. The concept of cubital tunnel syndrome is discussed, as is the importance of differential diagnosis and the use of diagnostic electrophysiology. Nerve transposition is considered the therapeutic measure of choice when UNE is a consequence of anomalous elbow, and…

The role of MR imaging in the management of elbow problems.

In the past several years, the role of MR imaging in diagnosing pathologic conditions of the elbow has dramatically increased. Aside from imaging soft-tissue tumors, it can accurately visualize partial and complete tears of tendons and ligaments, as well as displacement of epiphyseal fractures in children. Its role in identifying loose bodies, particularly when they are nonosseous, and areas of osteochondritis dissecans has also increased. The use of MR imaging for diagnosing neuropathies, particularly when electrodiagnostic studies are negative, offers exciting possibilities as additional tec…

Compressive neuropathies and tendinopathies in the athletic elbow and wrist.

Overuse syndromes of the upper extremity in the athletic population are a common and often difficult problem for physician and patient alike. Optimal function of the upper extremity is tied intimately to success in many sporting activities. Correct diagnosis and proper care require a thorough knowledge of the pertinent anatomy, pathophysiology, and pathomechanics involved in each disorder. Conservative care with rest, activity modification, and medication is adequate for most athletic injuries. Surgical intervention may be indicated for continuing pain, decreased performance, or to prevent chr

Modified in situ decompression of the ulnar nerve at the elbow.

Controversy continues to surround the treatment of ulnar nerve compression at the elbow in the cubital tunnel. By performing extensive segmental electrodiagnostic studies of the ulnar nerve through the cubital tunnel, accuracy of diagnosis is improved. In those patients with documented compression where the ulnar nerve enters the flexor carpi radialis, decompression of this area alone yields excellent results with rapid recovery.
