Patient-reported outcomes after simple dislocation of the elbow.

BACKGROUND: The current study was designed to investigate the epidemiology and long-term clinical and patient-reported outcomes following simple dislocation of the elbow in adults. METHODS: We identified all adult patients treated at our trauma center for a simple dislocation of the elbow during a ten-year period. One hundred and forty patients were eligible for review, and 110 (79%) were reviewed at a mean of eighty-eight months (range, sixteen to 171 months) after the injury. This review included clinical examination, the Disabilities of the Arm, Shoulder and Hand (DASH) questionnaire, the

Medial elbow torque during baseball hitting: considerations for return to play.

BACKGROUND: Injuries to the ulnar collateral ligament are increasing among baseball players at all levels. Little is known about the loads experienced by the medial elbow during baseball hitting. The purpose of this study was to describe the stress experienced by the medial elbow of the trail arm during baseball hitting. We hypothesized that the biomechanical comparison of hitting will demonstrate greater elbow varus torque of the trail arm for wood vs. aluminum bat and inside vs. other swing locations. METHODS: Thirteen healthy National Collegiate Athletics Association Division 1 baseball pl

[Elbow dislocation fractures].

The treatment of complex elbow dislocation fractures is a challenge to both the treating surgeon as well as to the patient due to the complex bony and soft tissue anatomy of the joint. In order to establish an expedient treatment algorithm, all osseous and ligamentous injuries need to be thoroughly assessed. Furthermore, a detailed knowledge of the joint-stabilizing structures, practicable surgical approaches as well as the possible techniques for fracture fixation and/or arthroplasty are essential to facilitate early rehabilitation of the elbow and avoid injury-related complications. 'A torn shoulder': an emergency department case study.

BACKGROUND: Rupture of the biceps is not a very common presentation to the emergency departments; as a result, it is poorly documented, especially in nursing literature. It is important that these patients are appropriately managed by registered advanced nurse practitioners. As the diagnosis of rupture is usually made on clinical grounds with imaging assisting in the diagnosis, left untreated these injuries can lead to considerable functional deficits. METHODS: The following paper is a case study based on a patient who presented to the emergency department with a proximal biceps rupture follo

Surgical treatment of brachial plexus traction injuries in children, excluding obstetric palsy.

Traumatic brachial plexus injuries in children, excluding birth palsy, are seldom reported. In this study, we report on 11 cases operated upon between 1995-1998, and followed for at least 30 months. All patients were males with an average age of 11 years (range, 3-16 years). The denervation time averaged 3.8 months (range, 1-8 months). Eight patients had two or more root avulsions; two had additional severe infraclavicular injuries. In total, 6 grafting and 25 extraplexal neurotization procedures were used. Donor nerves included the intercostal nerves, phrenic nerve, spinal accessory nerve, an

Ultrasound Applications in Adaptive Sports Medicine.

The use of ultrasound in adaptive sports has not been well studied. There is more literature for use in wheelchair athletes who suffer overuse upper extremity injuries, including shoulder and biceps tendon injuries, lateral epicondylosis, ulnar neuropathy at the elbow, and carpal tunnel syndrome. A few key lower extremity injury phenotypes have been studied with ultrasound in this population, including HO and skin complications. There are also unique applications of ultrasound, including boosting, respiratory muscle training, and evaluation of deep tissue injuries. Further ultrasound studies f

The Radial Forearm Flap in reconstruction of upper limb injuries: A case series.

BACKGROUND: The Radial Forearm Flap is a fasciocutaneous flap which has established its role in soft tissue reconstruction in the head and neck region as a free flap. Its value as a useful flap in soft tissue reconstruction in upper limb injuries has also been recognized. However there has been a dearth of report in our sub region on the use of this flap. OBJECTIVE: To report the use of Radial Forearm Flap in the reconstruction of traumatic defects in the elbow and hand in our centre. METHODS: Four cases in which pedicled island Radial Forearm Flaps were successfully used to reconstruct diff

Distinct Corticospinal and Reticulospinal Contributions to Voluntary Control of Elbow Flexor and Extensor Muscles in Humans with Tetraplegia.

Humans with cervical spinal cord injury (SCI) often recover voluntary control of elbow flexors and, to a much lesser extent, elbow extensor muscles. The neural mechanisms underlying this asymmetrical recovery remain unknown. Anatomical and physiological evidence in animals and humans indicates that corticospinal and reticulospinal pathways differentially control elbow flexor and extensor motoneurons; therefore, it is possible that reorganization in these pathways contributes to the asymmetrical recovery of elbow muscles after SCI. To test this hypothesis, we examined motor-evoked potentials (M

A Prospective Randomized Comparison of Median and Ulnar Fascicle Transfers to the Biceps Motor Branch in Upper Brachial Plexus Injury.

BACKGROUND: Transfer of an ulnar nerve fascicle to the motor branch of the biceps has been widely adopted in patients with upper brachial plexus injuries, with consistently favorable outcomes. Transfer of a median nerve fascicle has also been reported to yield satisfactory results. This study aimed to compare the outcomes of median and ulnar nerve fascicle transfer to the biceps in patients with C5-C6 and C5-C7 brachial plexus injuries. METHODS: A total of 50 consecutive patients underwent surgery between June 2013 and December 2018. The donor nerve was randomized before surgery. After exclud

Difficult elbow fractures: pearls and pitfalls.

Complex elbow fractures are exceedingly challenging to treat. Treatment of severe distal humeral fractures fails because of either displacement or nonunion at the supracondylar level or stiffness resulting from prolonged immobilization. Coronal shear fractures of the capitellum and trochlea are difficult to repair and may require extensile exposure. Olecranon fracture-dislocations are complex fractures of the olecranon associated with subluxation or dislocation of the radial head and/or the coronoid process. The radioulnar relationship usually is preserved in anterior but disrupted in posterio

Relationship Between Glenohumeral Internal Rotation Deficit and Medial Elbow Torque in High School Baseball Pitchers.

BACKGROUND: Alterations in throwing mechanics have been identified as a risk factor for overuse injuries in baseball players. Glenohumeral internal rotation deficit (GIRD) has been found to adversely affect throwing mechanics, but the effect of GIRD on medial elbow torque is unclear. PURPOSE: To investigate the relationship between GIRD and medial elbow torque in high school-aged baseball pitchers. STUDY DESIGN: Descriptive laboratory study. METHODS: High school baseball pitchers (14-18 years old) were recruited for participation in this study. Players' height, weight, body mass index, and

Risk factors for decreased range of motion and poor outcomes in open periarticular elbow fractures.

INTRODUCTION: The purpose of this study was to identify risk factors present at the time of injury that predict poor functional outcomes and heterotopic ossification (HO) in open periarticular elbow fractures. MATERIALS AND METHODS: We performed a retrospective review of 136 combat-related open elbow fractures from 2003 to 2010. Patient demographics, injury characteristics, treatment variables, and complications were recorded. Functional outcomes were analyzed to determine range of motion (ROM) and Mayo Elbow Performance Score (MEPS). Secondary outcome measures included the development of HO,

[Arthroscopic capsular release of the elbow].

Limited mobility of the elbow joint can be due to various factors, the most common of which is trauma to the elbow. In addition, limited elbow movement can be a direct cause of decreased functionality and interfere with everyday life activities. The majority of the patients can recover a functional range of motion if physical therapy is combined with a splinting regimen. However, some patients will experience continued elbow stiffness. Traditionally, open techniques have been successful in releasing the elbow joint capsule and regaining functional range of motion. Early results of arthroscopic

Fractures of the Humeral Ulnar Condyle in Children - a Rare but Complicated Injury.

BACKGROUND: Ulnar humeral condyle fractures are rare paediatric elbow fractures, classified as Salter-Harris IV paediatric elbow injuries. Due to constant radiological changes in the elbow with varying manifestation of ossification centres as well as late ossification of the trochlea, diagnosis of these injuries is challenging. To avoid long-term complications, the treating surgeon should be familiar with the rare injury picture, diagnostics and adequate therapeutic measures. MATERIAL AND METHODS: The present retrospective study includes data on all paediatric cases from 2002 to 2019 with pri…

Optimal management of ulnar collateral ligament injury in baseball pitchers.

The ulnar collateral ligament stabilizes the elbow joint from valgus stress associated with the throwing motion. During baseball pitching, this ligament is subjected to tremendous stress and injury if the force on the ulnar collateral ligament during pitching exceeds the physiological limits of the ligament. Injuries to the throwing elbow in baseball pitchers result in significant time loss and typically surgical intervention. The purpose of this paper is to provide a review of current information to sports medicine clinicians on injury epidemiology, injury mechanics, injury risk factors, inju

"How the Wrightington classification of traumatic elbow instability can simplify the algorithm for treatment".

There are numerous injury patterns of elbow-fracture dislocation that can lead to confusion about the best surgical management. The Wrightington classification aims to provide a simple categorization based on the injury to the coronoid process and the three column concept of the elbow osseous stability that describes a medial column consisting of the anteromedial coronoid facet and sublime tubercle, the middlecolumn is the anterolateral coronoid facet, and the lateral column is the radial head and lateral ligament complex with a fulcrum for varus/valgus stability between the two coronoid facet

A review of the Liverpool elbow prosthesis from 1974 to 1982.

Eighty elbows in 65 patients with an average age of 57 years have had two-part non-constrained Liverpool elbow arthroplasties performed since 1974. Fifty-five had rheumatoid arthritis, eight osteoarthritis or ankylosis secondary to injury, one osteochondritis dissecans and one pyknodysostosis. The average preoperative range of movement was 42 degrees to 112 degrees with 47 degrees of pronation and 42 degrees of supination. There was significant gain in the arc of movements at follow-up: 32 degrees in the extension-flexion range (average range 32 degrees to 134 degrees of flexion) and 42 degree
