Neuromuscular electrical stimulation for pre- and post-knee replacement surgery: a qualitative study.

Total knee replacement effectively relieves pain and restores function in end-stage osteoarthritis, but rehabilitation is essential to optimize outcomes. Postoperative pain, quadriceps weakness, and limited mobility can restrict participation in conventional exercises. Neuromuscular electrical stimulation (NMES) may support muscle activation when voluntary exercise is difficult, yet its use in practice remains limited. This qualitative study explored perspectives of patients and healthcare professionals on NMES use in pre- and postoperative knee rehabilitation. Twelve participants, including s

Neuromotor modules revealed by direct electrical stimulation of the human primary motor cortex.

Muscle synergies extracted from multimuscle electromyographic (EMG) signals are widely interpreted as fundamental building blocks of motor control that coordinate groups of muscles during movement. Whether EMG-derived muscle synergies represent genuine neurally encoded modules or motor regularities arising from task or biomechanical constraints has been hotly debated because neurophysiological data that definitively demonstrate the neural basis of muscle synergies in humans have been lacking. Here, we seek to validate the potential neural origin of upper limb muscle synergies by exploiting dir

Divergent Behavioral and Circuit-Level Adaptations to Acute and Chronic Gastric Electrical Stimulation.

Anxiety disorders are highly prevalent and often refractory to existing treatments, motivating the development of alternative neuromodulatory strategies. Peripheral bioelectronic approaches targeting the gut-brain axis such as vagus nerve stimulation (VNS) demonstrate that modulation of visceral afferent pathways can influence central emotional circuits. Gastric electrical stimulation (GES) is a clinically established therapy for gastrointestinal motility disorders. While the stomach is densely innervated by vagal afferents, the effect of GES on anxiety-related behavior has not been systematic

Gastric electrical stimulation for children with refractory nausea and vomiting: A sham-controlled trial.

OBJECTIVES: Gastric electrical stimulation (GES) improves symptoms in children with refractory nausea and vomiting, but pediatric evidence is limited to observational studies. We conducted the first controlled trial of GES in children. METHODS: We performed a single-blind, sham-controlled trial of temporary GES in children using a nasogastric lead. After endoscopic lead placement on study day 1, the stimulator was OFF for 4 days and then ON for 4 days. Participants were blinded to stimulation status. Symptom Monitor Worksheet (SMW) scores and nutrient drink test volumes were collected at base

Electrical stimulation of the vagus nerve improves amyloid pathology in delirium superimposed on dementia.

BACKGROUND: Delirium and delirium superimposed on dementia (DSD) are common complications affecting patients suffering from ongoing neurodegenerative pathologies. Peripheral surgical trauma can trigger neuroinflammation and ensuing DSD via mechanisms that remain poorly understood. Given the multifactorial therapeutic effects of neuromodulation, including vagal nerve stimulation, we have tested a minimally invasive approach to combat DSD following orthopedic surgery. METHODS: We performed orthopedic surgery on 5xFAD and CVN-AD mice and tested the efficacy of minimally invasive percutaneous vagu

Biphasic Electrical Stimulation of Schwann Cells on Conducting Polymer-Coated Carbon Microfibers.

Electroactive biomaterials are a key emerging technology for the treatment of neural damage. Conducting polymer-coated carbon microfibers are particularly useful for this application because they provide directional support for cell growth and tissue repair and simultaneously allow for ultrasensitive recording and stimulation of neural activity. Here, we report in vitro experiments investigating the biology of Schwann cells (SCs), a major player in peripheral nerve regeneration, on electroconducting microfibers. The optimal molecular composition of the cell substrate and cell culture medium wa

Modeling and Simulation of Spatial Characteristics in Epidural Electrical Stimulation for Targeted Neural Activation.

Neuromodulation via epidural neural interfaces holds promise as a treatment approach for various neurological conditions affecting the human central nervous system. Epidural electrical stimulation has been shown to be effective in suppressing chronic pain and restoring motor functions. However, the quantitative understanding of the efficacy and spatial selectivity of epidural electrical stimulation (EES) through epidural neural interfaces remains limited. In this study, we employed computational modeling and simulation techniques to investigate the spatial characteristics of epidural neuromodu

Preconditioning Effects of Neuromuscular Electrical Stimulation in Patients with Symptomatic Peripheral Arterial Disease.

AIMS: Intermittent claudication is a major barrier to establishing an exercise routine in patients with peripheral artery disease (PAD). This study investigated the preconditioning effects of neuromuscular electrical stimulation (NMES) on walking capacity in patients with PAD and intermittent claudication. Additionally, it aimed to determine the optimal NMES settings and the underlying mechanisms of its effects. METHODS: A total of 15 patients with PAD (Fontaine II) participated in a crossover study. Each patient underwent 10-min sessions of NMES at two frequencies (4 and 20 Hz) and two inten

Acute electrical muscle stimulation effects on strength and anaerobic power in soccer players.

Anaerobic power and lower limb muscle strength are of great importance in soccer, and various preloading strategies are used to improve these abilities. We investigated the acute effects of electrical muscle stimulation on muscle strength and anaerobic power in soccer players. Nineteen healthy male soccer players (age=21.1±1.6 y, training experience=10.1±3.2 y, height=178.1±4.0 cm, and body mass=8.9±3.7 kg) participated in the study. A placebo effect was created without telling the participants that which current (intensity) was applied. After the current applied to the quadriceps muscles, str

Transcranial electrical stimulation as a therapeutic strategy for Alzheimer's disease: Current uses and challenges.

Alzheimer's disease (AD) is an age-related neurodegenerative disorder for which there are currently rarely effective drug treatments available to halt or slow down its progression. With the aging of the world population, AD as the primary cause of dementia, is rapidly becoming one of the most expensive, lethal, and burdening diseases of this century. In recent years, the new method used to treat nervous system diseases including AD is transcranial electrical stimulation (tES) with non-invasive and for regulating the flexibility of neural circuits operation and behaviors. The rationale of tES f

[Sixteen-Channel Multimodal High-Precision Transcranial Electrical Stimulation System Development].

This paper introduces a 16-channel multimodal high-precision transcranial electrical stimulation system specifically for non-invasive brain stimulation. This system added TMCS mixed four traditional stimulation modes with TACS, TDCS, TPCS and TRNS. By designing a compensated high-precision constant current source, the constant stimulation current with an accuracy of 0.03 mA in the range of ±2 mA and the stimulation frequency of 50~200 kHz with low frequency of 0~4 kHz (high frequency of 0.1 Hz) are realized. In TACS stimulation mode, there are five adjustable wave forms: triangular wave, sine

Isometric Exercise Combined With Transcranial Electrical Stimulation Improves Conditioned Pain Modulation.

Exercise and primary motor cortex (M1) stimulation may alleviate pain by enhancing the endogenous pain-inhibitory system, with its efficacy assessed through conditioned pain modulation (CPM). This study examined whether combining exercise with M1-targeted transcranial electrical stimulation enhances CPM more effectively than either intervention alone. Two randomized, sham-controlled experiments were conducted. In Experiment 1 (N = 70), participants completed a 3-min isometric handgrip exercise or quiet rest. In Experiment 2 (N = 140), participants received 20 min of M1-targeted transcranial ra

Role of transcutaneous electrical nerve stimulation in alleviation of tinnitus in normal hearing subjects.

BACKGROUND: Subjective tinnitus is characterized by perception of sound in the absence of any external or internal acoustic stimuli. Many approaches have been developed over the years to treat tinnitus (medical and nonmedical). However, no consensus has been reached on the optimal therapeutic approach. Electrical nerve stimulation targeting peripheral auditory pathways presents a promising area of investigation for the treatment of tinnitus. Non-invasive transcutaneous vagal nerve stimulation of the auricular branch of the vagus nerve has been introduced and studied but its success rate varies

The simulation and experimental validation of a novel noninvasive multi-target electrical stimulation method.

The brain is a complex system of structure and function. Brain diseases and brain functional abnormalities often involve multiple functionally connected regions, including the deep brain. Studies have shown that multi-target electrical stimulation is more effective than single-target electrical stimulation. However, non-invasive multi-target electromagnetic stimulation, such as multi-target transcranial magnetic stimulation (TMS), transcranial direct current stimulation (tDCS) and transcranial alternating current stimulation (tACS) cannot meet the needs of synchronous multi-target accurate ele

Motion sickness susceptibility modulates the impact of electrical vestibular stimulation on postural control.

Motion sickness is defined as a sensation of uneasiness that occurs during physical motion, such as transportation by bus, plane, car or train. Postural control is one of the multisensory processes that has been examined in individuals susceptible to motion sickness. Indeed, postural control relies on visual, somatosensory and vestibular information. While studies suggest normal-like postural control during quiet standing, others suggest that individuals with motion sickness show increased postural instability during sensory perturbations, namely during visual and somatosensory disturbances. Electrical stimulation promotes peripheral nerve regeneration by upregulating glycolysis and oxidative phosphorylation.

Peripheral nerve injury (PNI) frequently results in motor and sensory dysfunction due to the limited regenerative capacity of axonal neurons and Schwann cells. Electrical stimulation (ES) has emerged as a promising strategy to enhance nerve regeneration; however, the underlying mechanisms, particularly those related to energy metabolism, remain poorly understood. This study aimed to investigate whether ES could promote nerve regeneration in a mouse model of PNI by modulating energy metabolism. ES was applied to the gastrocnemius and posterior thigh muscles post-sciatic nerve injury. Motor func

Early Results of Optimized Nerve Management With Electrical Stimulation for Lower Extremity Neuroma.

BACKGROUND: Nerve injury is the most common complication following foot and ankle surgery, with painful neuroma reported in up to 10% of procedures. Current treatment often yields varying degrees of pain relief. Electrical stimulation (ES) through peripheral neuromodulation is an emerging technology associated with improvement in nerverelated pain and acceleration of neural regeneration. This study assessed the short-term outcomes of combining nerve reconstruction techniques with ES in providing early pain relief for patients with symptomatic lower extremity neuromas. METHODS: We describe a s
