Pulmonary embolization in patients with implanted pacemakers is rare and an intracardiac thrombus attached to the distal end of a newly implanted ventricular endocardial electrode following removal of a previously implanted lead is a curiosity. We have documented such a case by digital subtraction angiography, a minimally invasive procedure that demonstrates both an intracardiac thrombus and secondary pulmonary embolization in a single procedure, which also allows simplified follow-up evaluation following therapy.

Cardiac disease is a leading cause of death worldwide. Disturbance in the conduction system of the heart may trigger or aggravate heart dysfunction, affecting the efficiency of the heart, and lead to heart failure or cardiac arrest. Patients may require implantable cardiac rhythm management devices (ICRMDs) to maintain or restore the heart rhythm. ICRMDs have undergone important improvements, yet limitations still exist, presenting important technological challenges. Most ICRMDs consist of a subcutaneous control unit and intracardiac electrodes. The leads, which connect the electrodes to the c

Despite the increasing use of stereoelectroencephalography, subdural electrode (SDE) implantation remains valuable to identify the epileptogenic network. Extra-axial hemorrhage, including subdural and epidural, is one of the most frequent complications of SDE implantation. This study aimed to identify risk factors associated with extra-axial hemorrhage post–SDE implantation in patients with focal drug-resistant epilepsy. We retrospectively reviewed consecutive patients who underwent SDE implantation via craniotomy at Hiroshima University Hospital between 2008 and 2022. Multivariate logistic re

BACKGROUND: Auditory brainstem implants (ABIs) can be used to restore hearing in subjects with no functional auditory nerve. Not infrequently, channels on the ABI give rise to nonauditory stimulation. The aim of the present study was to explore the prevalence of side effects for different portions of the implant and to investigate the number of active channels.
METHODS: Patients aged ≥ 15 years implanted with an ABI at Uppsala University Hospital were investigated. The number of active channels was extracted from the ABI programming software and reported side effects were extracted from the m

Neural implants help understand neurological disorders and are actively used to study deep and cortical brain surface regions. Dealing with cortical surface regions is less complicated in clinical therapy than deep brain regions. Researchers are interested in identifying cortical surface region/s for a particular neurological disorder. Rodent models are extensively used in preclinical studies. Usually, microwires, screws, and grid-type implants are used for such studies, but they are not designed for specific rodent brain regions. Since the grids are typically standard in size, in some cases,

The implantable cardioverter-defibrillator (ICD) was a breakthrough in the prevention of sudden cardiac death. After years of technical development in the USA, Michel Mirowski succeeded in proving reliable automatic defibrillation of ventricular tachyarrhythmias through initial human implantations in 1980, despite many obstacles. Nearly 4 years later, the first patients received ICDs at multiple centers in Germany. Subsequently, outside the USA, Germany became the country with highest implantation rates. The absolute number of implantations remained small as long as implantations required epic

INTRODUCTION: Assessing cochlear implantation's impact on cell loss and preventing post-implant cochlear damage are key areas of focus for hearing preservation research. The preservation of auditory neuronal and sensory neural hearing cells has a positive impact on auditory perception after implantation. This study aimed to provide details on a semi-automated spiral ganglion neuronal cell counting method, developed using whole implanted gerbil cochlea acquisitions with light-sheet microscopy.
METHODS: Mongolian gerbils underwent right cochlear implantation with an electrode array whose silico

In 2002 an increased number of cochlear implant related meningitis cases was reported by the U. S. Food and Drug Administration (FDA). The most commonly identified causative agent was Streptococcus pneumoniae. Although most cases of meningitis were related to a special electrode design, the risk for post-operative pneumococcal meningitis might nonetheless be enhanced by opening of the cochlea during implantation. In the present study, a threshold model for middle ear inoculation of S. pneumoniae was established in the guinea pig after cochlear implantation to assess the post-operative risk of

OBJECTIVE: Sacral root neuromodulation is an effective technique for the treatment of Chronic Micturition Dysfunction (CMD) refractory to conventional therapy. New indications such as chronic pelvic pain and interstitial cystitis are currently making their way within the urologic setting. Several groups of gastric surgeons are now choosing this technique for the management of rectal diseases (constipation, sphincter dysfunction). This paper contributes our experience in the treatment of patients with CMD and the comparative results at one year from implant in all patients.

BACKGROUND/OBJECTIVES: Implantable cardioverter defibrillators are used to prevent sudden cardiac death. The subcutaneous implantable cardioverter-defibrillator was newly developed to overcome the limitations of the conventional implantable cardioverter defibrillator-transvenous device. The subcutaneous implantable cardioverter defibrillator is indicated for young patients with heart disease, congenital heart defects, and poor venous access, who have an indication for implantable cardioverter defibrillator without the need for anti-bradycardic stimulation. We aimed to compare the efficacy and

Treating diseases nonpharmacologically, using targeted neurostimulation instead of systemic drugs, is a hallmark of the burgeoning field of bioelectronic medicine. In this review, we provide a brief overview of the discovery and function of the prototypical neuroimmune reflex, the "inflammatory reflex." We discuss various biomarkers developed and used to translate early physiological discoveries into dosing parameters used in experimental settings, from the treatment of animal models of disease through a proof-of-concept clinical study in rheumatoid arthritis (RA). Finally, we relate how uniqu

During treatment with deep brain stimulation (DBS), electrical current is delivered into the brain parenchyma through implanted electrodes. Although this technique is routinely used in the treatment of Parkinson's disease, essential tremor, and dystonia, a growing number of neuropsychiatric applications for DBS are being investigated. Investigators can use animal models of these diseases to study the mechanisms through which DBS exerts its effects, to explore new applications of this therapy, and to identify and characterize alternative stimulation targets. Here, we discuss preclinical DBS res

The use of stimulation electrodes implanted in the brain to control severely disabling neurological and psychiatric conditions is a fast emerging area of clinical neuroscience. For instance, high-frequency stimulation of the subthalamic nucleus (STN) has become the surgical therapy of choice for advanced Parkinson's disease. This therapy improves motor disability substantially and also the quality of life, but some patients show postoperative behavioral changes such as depression and mania. These behavioral effects can be explained on the basis of the anatomical data. The STN is interconnected

A 48-year-old man with severe, lifelong Tourette's syndrome (TS) characterized by forceful self-injurious motor tics and obsessive-compulsive disorder was treated with bilateral deep brain stimulation (DBS). The decision to treat was based on his progressive neurological impairment (left sided weakness secondary to spinal cord injury) because of his relentless, violent head jerks. Electrodes were implanted at the level of the medial part of the thalamus (centromedian nucleus, the substantia periventricularis, and the nucleus ventro-oralis internus). DBS resulted in a substantial reduction of t

In the experiments on conscious rabbits with the electrodes implanted in the brain it was found by the methods of hydrogen clearance and polarography that piracetam (50 and 500 mg/kg) and phenibut (50 mg/kg) induced a decrease of the local blood flow in the cortex of the frontal, temporal and occipital regions and the total cerebral blood flow. The administration of the drugs before tilting prevents the occurrence of vasodilatation in the brain and results in a decrease of the blood flow in the cortex and in the confluence of the cerebral venous sinuses that is thought to be connected with the

Numerous changes continue to occur in regard to cochlear implant candidacy. In general, these have been accompanied by concomitant and satisfactory changes in surgical techniques. Together, this has advanced the utility and safety of cochlear implantation. Most devices are now approved for use in patients with severe to profound rather the prior requirement of a bilateral profound loss. In addition, studies have begun utilizing short electrode arrays for shallow insertion in patients with considerable low frequency residual hearing. This technique will allow the recipient to continue to use ac

The antiepileptic medication and surgical treatment had brought many patients with epilepsy to be seizure free, however, one third of the patients continue to experience seizures. There has recently been an explosion of research into brain stimulation for treating these intractable epilepsy patients. This is largely due to the success of deep brain stimulation of movement disorders. The intelligent cardiac pacemakers also stimulated the neurosurgeons to utilize the implantable devices. In this paper, brain stimulations with vagus nerve stimulator (VNS), depth electrodes, subdural electrodes, e
