Noise exposure and its effect on the labyrinth, Part I.

According to the author, the assumption is made that in Ménière's disease, compensation should be paid in acoustic trauma cases. Ménière's disease is an aftereffect of noise exposure, even if the time lapse exceeds 10 years. A historical review of existing literature (from 1872 to 1958) shows no convincing evidence of this matter. Ten patients were chosen at random. All were exposed to noise and presented with vestibular dysfunction at a certain time.

Otolith Membrane Herniation, not Semicircular Canal Duct Dilation, Is Associated with Decreased Caloric Responses in Ménière's Disease.

Ménière's disease (MD) is a debilitating disorder with unclear pathophysiology whose diagnosis often relies on clinical judgment rather than … ears with a normal response had this herniation. Moreover, we evaluated the semicircular canals for endolymphatic hydrops, which had been hypothesized to contribute to the dissociation, and found no evidence of duct dilation/hydrops. We did, however, note a potentially novel morphologic finding-smaller bony labyrinth cross-sectional diameters/areas in some MD ear canal …

Understanding and Managing Ménière's Disease: A Comprehensive Case Report.

Ménière's disease (MD) is a chronic vestibular disorder characterized by recurrent vertigo, fluctuating sensorineural hearing loss (SNHL), tinnitus, and aural fullness, with an unpredictable course that can significantly impair quality of life. Endolymphatic hydrops (EH) remains the hallmark histopathological finding; however, its underlying pathophysiology is not fully understood and likely reflects a multifactorial process involving immune, viral, allergic, genetic/familial, and vascular mechanisms. Emerging evidence suggests that EH may represent a secondary manifestation rather than the pr…

Advanced Magnetic Resonance Imaging Sheds Light on the Distinct Pathophysiology of Various Types of Acute Sensorineural Hearing Loss.

OBJECTIVE: To compare the findings of magnetic resonance imaging (MRI) with advanced protocols in patients with various types of acute senso … earing loss (ISSNHL) but was rarely observed in definite Ménière's disease (MD, 2.6%). In contrast, endolymphatic hydrops (EH) was frequently observed in patients with definite MD (79.5%) but was observed much less frequently in patients with ISSNHL (11.0%). In patients with cochlear MD and ALHL, detection rates of cochlear EH were similar to those with definite MD, wh …

Three-Tesla magnetic resonance imaging of the vestibular endolymphatic space: A systematic qualitative description in healthy ears.

BACKGROUND AND PURPOSE: A detailed knowledge of the normal Magnetic Resonance (MR) anatomy of the vestibular endolymphatic space (ES) could be useful to understand the linkage between endolymphatic hydrops (EH) and Ménière's disease (MD). Our aim was to describe the MR anatomy of the vestibular ES as depicted by MR imaging in healthy ears. METHODS: This report describes a single-center retrospective study. Three readers analyzed the healthy ears of 22 consecutive patients who had undergone MRI for unilateral sudden hearing loss. The readers described the vestibular ES based on a delayed post-…

Bilateral Vestibular Atelectasis as a Fluctuating Bilateral Vestibulopathy: A Case Report Care.

The vestibular atelectasis (VA) is a very rarely reported clinical and radiological condition characterized by the collapse of the labyrinthine membrane. Clinical features include symptoms which mimic endolymphatic hydrops; bilateral forms are rarely reported in the literature. Tullio's sign was reported in a few cases. In this paper, we report an original case of bilateral fluctuating VA associated with a rare posterior semicircular canal (SSC) dehiscence variant. A 54-year-old patient presented with evolving and fluctuating bilateral vestibulopathy associated with a pressureinduced nystagmus…

Charles Skinner Hallpike and the beginnings of neurotology.

Although the symptom of vertigo had been well-recognized for several thousand years, it was not until the pioneering work of Prosper Ménière in the mid-19th century that it was appreciated that vertigo could originate from damage to the inner ear. Before that time, patients with vertigo (regardless of the cause) were said to have "cerebral congestion," a condition resulting from excessive blood filling the brain. Bloodletting and leeches to relieve the congestion were the treatment of choice. The discovery of endolymphatic hydrops in temporal bone specimens from patients with Ménière's disease…

Cyclophosphamide inhibits the progression of Meniere's disease by reducing the generation of circulating immune complex.

Endolymphatic hydrops is a characteristic pathological manifestation of Meniere's disease (MD) that has been previously associated with autoimmunity. Interest in the circulating immune complex (CIC) has increased due to its reported role in the occurrence of MD. The present study aimed to investigate the potential value of serum CIC concentration in the diagnosis of MD and the therapeutic potential of cyclophosphamide (CTX) for the treatment of MD. In the present study, guinea pigs were immunized with isologous crude inner ear antigens to establish an autoimmune MD model. Pure tone audiometry,…

Ménière's Disease: A Tri-Decade Scoping Review of Treatment Trends and Evidence Quality.

INTRODUCTION: Ménière's disease (MD) is a complex inner ear disorder defined by endolymphatic hydrops and a triad of episodic vertigo, fluctuating hearing loss, and tinnitus. Despite AAO-HNS diagnostic guidelines, disease heterogeneity and lack of treatment consensus persist. OBJECTIVE: This study conducts a tri-decade scoping review (1994-2024) to evaluate the MD evidence base, shifts in management, and methodological rigor regarding AAO-HNS and CONSORT compliance. MATERIALS AND METHODS: A systematic search of PubMed, CINAHL, and Cochrane (2014-2024) identified English-language RCTs and pro…

Immunohistochemical localization and mRNA expression of aquaporins in the macula utriculi of patients with Meniere's disease and acoustic neuroma.

Meniere's disease is nearly invariably associated with endolymphatic hydrops (the net accumulation of water in the inner ear endolymphatic space). Vestibular maculae utriculi were acquired from patients undergoing surgery for Meniere's disease and acoustic neuroma and from autopsy (subjects with normal hearing and balance). Quantitative immunostaining was conducted with antibodies against aquaporins (AQPs) 1, 4, and 6, Na(+)K(+)ATPase, Na(+)K(+)2Cl co-transporter (NKCC1), and alpha-syntrophin. mRNA was extracted from the surgically acquired utricles from subjects with Meniere's disease and aco…

The relationship between clinical characteristics and magnetic resonance imaging results of Ménière disease: a prospective study.

Ménière disease (MD) is an idiopathic inner ear disorder, and endolymphatic hydrops (EH) being considered to be its pathological basis. Currently, there is no gold standard for diagnosing MD. Previous study has reported visualized EH using MRI by intratympanic gadolinium-based contrast media (GBCM) administration (IT-Gd) in patients with MD, and this technique was gradually established for MD diagnosis. However, few studies reported their diagnostic sensitivity and specificity in clinical application. This prospective study aimed at investigating the clinical characteristics and magnetic reson…

Pediatric subjects with vertigo or dizziness are rare in Japan, although considerable statistical data accumulated, mostly indicating that orthostatic hypotension is the most frequent clinical symptoms in Japan, as opposed to Benign paroxysmal vertigo of childhood (BPVC), reported to be most frequent abroad. We studied BPVC incidence and clinical features. Subjects were 5 boys and 7 girls aged 4 to 15 years old (average +/- sd 9.5 +/- 3.1 years). The predominant diagnosis was BPVC in 8, orthostatic hypotension in 6, labyrinthitis in 2, and delayed endolymphatic hydrops, and conversion disorder…
