Selective posterior cerebral artery amobarbital test in patients with temporal lobe epilepsy for surgical treatment.

PURPOSE: To evaluate whether the selective posterior cerebral artery amobarbital test (PCAAT) can predict postoperative memory function in patients with temporal lobe epilepsy (TLE) for surgical treatment and its clinical safety. METHODS: The database included adult patients with intractable TLE undergoing presurgical evaluation at Taipei Veterans General Hospital between 1995 and 2002. The PCAAT was carried out on patients who had failed conventional intracarotid amobarbital test (IAT) or had IAT results that evoked a concern of postoperative memory function decline. A battery of learning an

Subtle microscopic abnormalities in hippocampal sclerosis do not predict clinical features of temporal lobe epilepsy.

PURPOSE: Subtle microdysplastic features are found in some patients with hippocampal sclerosis (HS) and refractory temporal lobe epilepsy. The significance of these findings is unknown. We investigated their frequency, relation to the pattern of HS, and clinical associations. METHODS: One-hundred forty patients with histologically confirmed HS (mean age at operation, 35 years; 85 women) were analyzed. The presence of HS and subtle structural abnormalities (SSAs) in the mesial temporal lobe and in the lateral neocortical tissue was assessed in detail. Antecedents, seizure characteristics, two

Quantitative magnetic resonance imaging in consecutive patients evaluated for surgical treatment of temporal lobe epilepsy.

We present the results of quantitative Magnetic Resonance Imaging (MRI) in 55 consecutively referred patients with clinical evidence of temporal lobe epilepsy (TLE). The Cavalieri method was used in combination with point counting to provide unbiased estimates of the volume of the left and right hippocampus, amygdala, temporal lobe, lateral ventricles and cerebral hemisphere, and pixel by pixel maps of the T2 relaxation time were computed for both central and anterior sections of the hippocampus. The 99th centiles of hippocampal volume, hippocampal volume asymmetry and T2 relaxation times in 2

Comparison of localizing values of various diagnostic tests in non-lesional medial temporal lobe epilepsy.

Though the surgical treatment for medial temporal lobe epilepsy yields a high success rate, more studies are needed in order to determine the most efficacious pre-operative algorithm. The authors studied the relationship between surgical outcome and the localization results of various pre-operative diagnostic tests to assess the predictive value. Seventy-one consecutive patients who had undergone anterior temporal lobectomy with amygdalohippocampectomy with the diagnosis of non-lesional medial temporal lobe epilepsy, who had been followed up more than 24 months, were analyzed retrospectively.

Correlation of neuropsychological and metabolic changes after epilepsy surgery in patients with left mesial temporal lobe epilepsy with hippocampal sclerosis.

BACKGROUND: Epilepsy surgery often causes changes in cognition and cerebral glucose metabolism. Our aim was to explore relationships between pre- and postoperative cerebral metabolism as measured with 18F-fluorodeoxyglucose positron emission tomography (FDG-PET) and neuropsychological test scores in patients with left mesial temporal lobe epilepsy with hippocampal sclerosis (MTLE-HS), who were rendered seizure-free after epilepsy surgery. RESULTS: Thirteen patients were included. All had neuropsychological testing and an interictal FDG-PET scan of the brain pre- and postoperative. Correlation

Progressive Hippocampal Volume Reduction Beyond Normal Aging in Temporal Lobe Epilepsy: A Longitudinal MRI Study.

Recent neuroimaging studies have suggested that epilepsy may show progressive brain structural changes over time. However, longitudinal hippocampal volume changes in temporal lobe epilepsy (TLE) have not been adequately compared with those observed in normal aging. In this study, we enrolled 46 patients with unilateral TLE and 25 healthy controls with longitudinal structural MRI scans using the same scanner. Hippocampal volumes were quantified using the Hipposeg software, and the annual percentage volume reduction relative to baseline (%Reduction/year) was calculated for each hippocampus. Comp

Face-name associative memory in temporal lobe epilepsy: Region-specific insights in right-hemisphere onset.

Mapping memory function in temporal lobe epilepsy (TLE) is important for understanding surgical risk, yet practical noninvasive tools remain limited. We tested whether a face-name associative memory fMRI task could detect subregional dysfunction within the amygdala-hippocampal complex (AHC) in TLE. Forty-nine individuals with unilateral TLE (29 left-, 20 right-onset; 51% MTS-negative) and 25 controls from three centers were scanned during a face-name encoding task and completed delayed recognition and neuropsychological tests of memory. The hippocampal head, body, tail, and amygdala were anato

CPEB1 drives ferroptosis-neuroinflammation crosstalk in temporal lobe epilepsy via the SIRT1-NRF2 acetylation axis.

BACKGROUND: Temporal lobe epilepsy (TLE) is a common neurological disorder frequently resistant to pharmacological treatment, yet its molecular mechanisms remain incompletely understood. Cytoplasmic polyadenylation element-binding protein 1 (CPEB1) is a post-transcriptional regulator implicated in neuronal stress responses; however, its role in epilepsy and redox-inflammatory signaling remains unclear. METHODS: An integrative multi-omics strategy combining single-cell transcriptomics, bulk RNA sequencing, and bioinformatics analyses was employed, followed by validation in human epileptic hipp

Assessing Executive Function in Patients With Temporal Lobe Epilepsy Through a Game-Based Assessment Task.

OBJECTIVES: Temporal lobe epilepsy (TLE) is often accompanied by executive function (EF) impairment, highlighting the need for accessible and quantitative assessment approaches. In this study, we applied our previously developed game-based executive function assessment tool (EFAT) to evaluate EF performance in TLE patients. METHODS: Thirty-two TLE patients and 29 healthy controls (HCs) completed a comprehensive battery of standardized tests, including the Stroop Color-Word Test (SCWT) and the Trail Making Test (TMT), as well as the EFAT. Metrics from the EFAT (number of touches, number of cor

Biophysically informed large-scale circuit modeling reveals region-specific microscale dynamics in temporal lobe epilepsy.

Objective. Temporal lobe epilepsy (TLE) is frequently accompanied by hippocampal sclerosis (HS) and involves widespread alterations in brain structure and function. However, most neuroimaging studies rely on single-modality data and provide limited insight into the microscale neural mechanisms underlying these changes. The relationship between structural connectivity (SC) and functional connectivity also remains insufficiently understood. This study aims to integrate structural and functional data to investigate microscale neural dynamic alterations in TLE.Approach. A biophysically informed re

Cognitive and Biomarker Signatures of Late-Onset Temporal Lobe Epilepsy: Toward Non-Alzheimer Neurodegenerative Mechanisms.

BACKGROUND AND OBJECTIVES: Late-onset unexplained epilepsy (LOUE) represents a substantial proportion of epilepsies with onset after 50 years and often manifests as temporal lobe epilepsy (LO-TLE). Although a link with Alzheimer disease (AD) has been suggested, only a subset of LO-TLE shows AD-related biomarkers, indicating biological heterogeneity. This study aims to characterize the cognitive and CSF phenotype of LO-TLE and compare it with healthy controls (HCs) and patients with mild cognitive impairment due to AD (MCI-AD). METHODS: This Italian cross-sectional cohort study included LO-TLE

CA1 parvalbumin interneurons display alterations of myelin patterning driven by microgliosis in temporal lobe epilepsy.

Temporal lobe epilepsy (TLE) is one of the most common forms of epilepsy, characterized by significant reorganization of hippocampal neuronal circuits. While changes in GABAergic inhibitory circuits are a major feature of this reorganization, other prominent alterations include hippocampal myelination disruption, neuronal loss, and pronounced glial and inflammatory responses. Recent studies have established that parvalbumin (PV +) interneurons in the hippocampus are partially myelinated under normal conditions, representing a notable structural feature of this interneuron subtype. However, whe
