Lateral Malleolus Reconstruction After Tumor Resection in Children: A Case Report and Literature Review.

BACKGROUND: Pediatric reconstruction of lateral malleolus was necessary and challengeable. Up to now, vascularized fibular was the optimal graft to reconstruct epiphyseal defection. However, the sophisticated microvascular operation has limited the wide application of this technique. CASE PRESENTATION: We present the case of a 9-year-old boy with Ewing sarcoma in left distal fibula. In order to restore the growth capacity, we used reverse-flow vascularized fibular epiphyseal graft with tibialis anterior artery to reconstruct the bone defect after tumor resection with no microvascular anastomo

Morphologic Features of the Contralateral Femur in Patients With Unilateral Slipped Capital Femoral Epiphysis Resembles Mild Slip Deformity: A Matched Cohort Study.

BACKGROUND: Hip osteoarthritis has been reported in the contralateral hip in patients who had been treated for unilateral slipped capital femoral epiphysis (SCFE) during adolescence. Although this might be related to the presence of a mild deformity, the morphologic features of the contralateral hip in unilateral SCFE remains poorly characterized. QUESTIONS/PURPOSES: Do measurements of (1) femoral head-neck concavity (α angle and femoral head-neck offset), (2) epiphyseal extension into the metaphysis (epiphyseal extension ratio and epiphyseal angle), and (3) posterior tilt of the epiphysis (e

The arrow and the target-description of the "Robin Hood" technique for fixation of tibial spine avulsion fractures: a technical note.

Avulsion fractures of the tibial spine, also known as the tibial eminence, are predominantly described in the pediatric population and uncommon in adults. In the presence of a tibial spine fracture associated with a fracture of the proximal meta-epiphyseal tibia, management of the spine fragment remains controversial. The objective of this technical note is to describe the Frosch technique termed the "Robin Hood technique", which consists of fixation of the tibial spine fracture with screws inserted in a retrograde fashion in cases of combined fractures of the tibial spine (due to avulsion of

The Galeazzi-equivalent lesion in children revisited.

A fracture at the medial end of the distal third of the radius with an epiphyseal separation of the distal ulna in a 16-year-old boy is described. This injury, known as the Galeazzi-equivalent lesion in children, is characterized by complete distal ulnar epiphyseal separation without rupture of the distal ligamentous stabilizing system between the radius and ulna, which includes the triangular fibrocartilage complex, interosseous ligaments and periosteal tube of the ulnar. The Galeazzi fracture-dislocation and the Galeazzi-equivalent lesion appear to be completely dissimilar in their pathologi

[Osteoarticular complications of organ transplantations].

Osteoarticular complications can develop after organ transplantation and are responsible for high morbidity. In addition to infectious complications due to common or opportunistic germs, those that are the best known are complications related to glucocorticoid treatment, namely osteoporosis and epiphyseal osteonecrosis. More recently, other complications such as the effect of cyclosporine on bone metabolism and epiphyseal impaction have been observed. Still others are iatrogenic, due to the multiple drugs required following transplantation, and may affect the joints, the tendons or the muscula

[Congenital partial gigantism of the upper extremities in children].

Angio-and phlebography, as well as pathomorphological studies were made on epiphyseal cartilaginous plate, diaphyseal bone tissue and skin of patients with partial gigantism of upper extremities. Significant developmental anomalies were found in the formation of palmar arches and digital arteries, as well as hypoplasia and aplasia of deep brachial and antebrachial veins. Light and electron microscopy revealed epiphyseal plate dystrophy and dysplasia, tubular bone osteoporosis, subcutaneous fat hyperplasia. The data obtained suggest a pathogenetic relationship between the vascular formation and

Slipped capital femoral epiphysis: a pathological study. I. A light microscopic and histochemical study of 21 cases.

Core biopsy specimens of slipped proximal femoral epiphyseal growth plates and of normal controls were examined by light microscopy and histochemistry. Slipped growth plates showed diminished cellularity with an overall excess of matrix as well as severe disorientation and misalignment of chondrocytes. Decrease in number of argyrophilic fibers in the longitudinal septa suggested deficiency in collagen. Histochemical staining of slipped plates did not deviate from normal except in severely deformed areas. Anatomic and histochemical observations did not conclusively support or exclude biochemica

Carpal length in children--a useful measurement in the diagnosis of rheumatoid arthritis and some concenital malformation syndromes.

The distance between the midpoint of the distal radial epiphyseal growth plate and the proximal end of the third metacarpal offers a useful measure for determining wrist size. Standards of this distance as compared to intermetacarpal width and second metacarpal length are presented. These measures (or ratios) should be useful in evaluating patients with juvenile rheumatoid arthritis and a number of congenital malformation syndromes, particularly when the carpals are still not fully ossified. Shortening of the carpus occurs in multiple epiphyseal dysplasia, the otopalatodigital syndromes, Turne

Habitual patellar dislocation in extension in children with immature epiphysis: a report of three cases.

BACKGROUND: Pediatric habitual patellar dislocation in extension (E-HPD) is a rare and specific form of patellar dislocation with limited reports in the literature. For children with immature epiphyses, the gold standard for E-HPD surgery has not been established, and the appropriate treatment is controversial. We report three cases of pediatric E-HPD with epiphyseal immaturity and explore the etiology, risk factors, and treatment of E-HPD in children. METHODS: This article describes three cases of children with E-HPD whose epiphyses were not closed because of skeletal immaturity. The children…

Growth and shaping of metacarpal and carpal cartilage anlagen: application of morphometry to the development of short and long bone. A study of human hand anlagen in the fetal period.

A histological and morphometric analysis of human metacarpal and carpal anlagen between the 16th and 22nd embryonic weeks was carried out with the aim of studying the establishment of the respective anlage architecture. No differences in the pattern of growth were documented between the peripheral and central zones of the metacarpal epiphyses and those of the carpals. The regulation of longitudinal growth in long bone anlagen occurred in the transition zone between the epiphysis and the diaphysis (homologous to the metaphyseal growth plate cartilage in more advanced developmental stage of the …

Dysplasia epiphysealis hemimelica: a report of four cases.

UNLABELLED: Dysplasia epiphysealis hemimelica (DEH), also known as Trevor's disease, is a rare congenital skeletal developmental disorder in childhood. It is characterized by an asymmetric osteocartilaginous overgrowth arising from epiphyses or epiphyseal equivalents. Lesions have hemimelic topography, usually occur in the lower limbs, most commonly in the ankle and the knee and affect either the medial or lateral part of one epiphysis. OBJECTIVE: The purpose of this study is to describe the imaging features of DEH by reporting four cases. MATERIALS AND METHODS: We present four cases of DEH …

Standardizing ordinal subadult age indicators: Testing for observer agreement and consistency across modalities.

Skeletal and dental data for subadult analyses obtained from dry bones or various types of medical images, such as computed tomography (CT) scans or conventional radiographs/x-rays, should be consistent and repeatable to ensure method applicability across modalities and support combining study samples. The present study evaluates observer agreement of epiphyseal fusion and dental development stages obtained on CT scans of a U.S. sample and the consistency of epiphyseal fusion stages between CT scans and projected scan radiographs/scout images (U.S. CT sample), and between dry bones and convent

Altered postnatal expression of insulin-like growth factor-I (IGF-I) and type X collagen preceding the Perthes' disease-like lesion of a rat model.

The spontaneously hypertensive rat (SHR) is a widely used animal model for the study of hypertension. It also exhibits an osteonecrosis of the femoral epiphysis that resembles the clinical features of Perthes' disease in humans. In this rat model, occlusion of the epiphyseal vessels occurs as a result of a breakdown of the mechanically vulnerable epiphysis. The postnatal development of the epiphysis recapitulates the serial events of the endochondral ossification (i.e., cartilage formation), chondrocyte hypertrophy, cartilage mineralization, vascularization, and introduction of osteoblasts tha

Osteochondritis dissecans of the femoral condyles.

Because the results of treatment of osteochondritis dissecans of the femoral condyles are still not consistently satisfactory, we reviewed the cases of eighty-three patients (ninety-five knees) with osteochondritis dissecans who were followed for two to thirty-one years to identify factors that may influence treatment and long-term prognosis. Of the eighty-three patients, sixteen had only non-surgical treatment, sixty-five had only surgical treatment, and two had non-surgical treatment of one knee and surgical treatment of the other. Of the twenty-two knees (eighteen patients) that were treate

Trichorhinophalangeal syndrome type II without the chromosome 8 deletion that resembled metachondromatosis.

We describe a 5-year-old girl with features resembling Trichorhinophalangeal syndrome, type I (sparse scalp hair, bushy eyebrows, bulbous nose, long philtrum, cone-shaped epiphyses, clinobrachydactyly, epiphyseal changes in the femoral head and short stature), and appendicular exostoses similar to trichorhinophalangeal syndrome, type II. However, despite physical resemblance to the trichorhinophalangeal syndrome variants, cytological analysis showed a structurally normal chromosome 8 and no mental deficiency was apparent. In addition, morphological congruities between multiple exostoses and me…

The Distribution of Paediatric Forearm Fractures: A Five-Year Retrospective Cohort Study of 4546 Forearm Fractures in Children.

BACKGROUND: Forearm fractures are the most common fractures in children, accounting for 41% of all paediatric fractures. Most research focuses on distal forearm fractures, but studies encompassing the entire forearm are limited. OBJECTIVE: This retrospective study describes the distribution and patterns of paediatric forearm fractures over a five-year period. METHODS: We conducted a retrospective cohort study of children aged 0-15 years who received a radiograph of the forearm, wrist or elbow between March 2019 and December 2023 in the study region. Fractures were manually identified and reg

Vascular safe zones for surgical dislocation in children with healed Legg-Calvé-Perthes disease.

BACKGROUND: Legg-Calvé-Perthes disease consists of idiopathic osteonecrosis of the femoral head, causing proximal femoral growth deformity. Recent advances in surgical technique have permitted safe surgical dislocation of the hip, allowing for correction of femoracetabular impingement. The purpose of this study was to characterize the location and number of lateral epiphyseal arteries supplying the femoral head in children with healed Legg-Calvé-Perthes disease. METHODS: This retrospective study included nineteen children (twenty-two hips) with a diagnosis of Legg-Calvé-Perthes disease (the L
