A case report of frontal spontaneous epidural hematoma associated with cranial osteomyelitis and epidural abscess due to paranasal sinusitis.

BACKGROUND: Intracranial epidural hematoma (EDH) is frequently secondary to trauma, but in some rare cases, spontaneous EDH (SEDH) could develop without trauma. Cranial osteomyelitis is an uncommon osseous infection that most frequently presents as a postoperative complication but also rarely originates from paranasal sinusitis and can develop extracranially to form a subperiosteal abscess or intracranially to form an epidural, subdural, or cerebral abscess. Intracranial epidural abscess (EDA) is an uncommon infection that forms in the space between the cranial bone and dura mater. It is rare

Correction of proptosis and zygomaticomaxillary asymmetry using orbital wall decompression and zygoma reduction in craniofacial fibrous dysplasia.

Craniofacial fibrous dysplasia is associated with facial asymmetry, orbital dystopia, and orbital proptosis. Although radical excision of the affected bone with immediate craniofacial reconstruction is usually the best treatment option, complete excision may be impossible in some patients because of involvement of the skull base, including the sphenoid and ethmoidal bones. In adult patients without functional loss or rapid disease progression, a shaving procedure is the optimal alternative for fibrous dysplasia affecting the maxilla and zygoma areas. Lesions in the skull base causing exophthal

[A case of pycnodysostosis--observation of the skull by CT scan].

A 13-year-old boy was presented to the Department of Neurosurgery, Saiseikai Fukuoka General Hospital for further examinations concerning abnormal findings in the skull radiogram taken when he struck his head. His physical features showed some characteristics the same as those of pycnodysostosis as follows--proportionate dwarfism, prominent forehead, short spoon-shaped fingers, bilateral exophthalmos. A skull radiogram revealed widely open cranial sutures with no healing of the fracture and craniotomy which was performed for an acute epidural hematoma 6 years ago. Furthermore, the mandible was

Use of a navigation system in endonasal surgery: Impact on surgical strategy and surgeon satisfaction. A prospective multicenter study.

OBJECTIVES: Surgical navigation systems (SNS) are now widely used in endoscopic endonasal surgery. Benefit, however, has not been fully studied. The objective of this study was to evaluate the impact of an SNS in terms of performance of the surgical procedure and of surgeon satisfaction, in a prospective multicenter study. MATERIALS AND METHODS: A multicenter prospective study included patients undergoing endoscopic endonasal surgery using the electromagnetic DigiPointeur® (DGP) SNS in 16 French hospitals. An observation form, completed by the surgeon immediately at end of procedure, included

Death related to nasal surgery: case report with review of therapy-related deaths.

The investigation of deaths that are suspected to be related to medical therapy present several challenges for the forensic pathologist. In all such cases, it is important for the pathologist to understand the therapy/procedure that may have played a role in death. As such, review of medical records and possible consultation with various medical specialists before autopsy can provide valuable information to ensure proper evaluation in any given case.Herein, we describe a rare death occurring as a complication of septoplasty, nasal polypectomy, and intranasal endoscopic ethmoidectomy, which are

Nuances of Olfactory Groove Meningioma Surgery: 2-Dimensional Operative Video.

Olfactory groove meningiomas frequently present as large or giant-size tumors associated with marked frontal lobe edema and significant frontal lobe dysfunction. Simpson grade I removal was rare in early reports due to their invasion of the ethmoid sinuses and skull base bone,1 which resulted in high recurrence rates.2,3 Indeed, recurrence occurred in the most celebrated case of olfactory groove meningioma.4,5  To achieve Simpson grade I removal (tumor, dura, bone), protect the frontal lobes from additional injury, and provide the best chance for recovery, we demonstrate a few nuances for olfa

The reconstruction of skull base defects in infants using pedicled nasoseptal flap-a review of four cases.

INTRODUCTION: Benign lesions of the skull base are common in the paediatric population, and are usually congenital in aetiology. Majority of these lesions are treated transcranially exposing the patients to a number of risks. Although endoscopic endonasal surgery (EES) helps avoid many of these potential morbidities, CSF leak with its attendant complication remains a big concern. This study reports the use of the Hadad flap in the reconstruction of skull base defects in infants to prevent this problem. The study was conducted on four infants with a mean age of 7 months, who underwent repairs f

A rare case of dedifferentiated liposarcoma of the sinonasal cavity: A case report.

Sarcoma is an uncommon histopathological presentation of sinonasal tumors, comprising ~15% of all cases; liposarcoma is particularly uncommon. An analysis of the available medical literature revealed no prior reports of dedifferentiated liposarcoma (DDLPS) of the sinonasal cavity. This case report presents a rare case of DDLPS of the sinonasal cavity. A 40-year old six-week pregnant female was admitted with a left nasal obstruction. Endoscopic evaluation of the left nasal cavity revealed a polypoid lesion. A computed tomography scan indicated a mass invading the left nasal cavity, maxillary si

Cytopathologic characteristics of SMARCB1 (INI-1) deficient sinonasal carcinoma: A potential diagnostic pitfall.

Tumors of the head and neck are extremely diverse and a subset are poorly differentiated and difficult to classify. Recently, a new entity has been described with rhabdoid and/or plasmacytoid cytologic features and a characteristic genetic signature-inactivation of the SMARCB1 (INI-1) tumor suppressor gene. To date, only 16 cases of SMARCB1 (INI-1) deficient sinonasal carcinoma have been described, and there are currently no reports of the cytopathologic features by fine needle aspiration (FNA) cytology. A case of a 77-year-old man who presented with a posterior ethmoid sinus lesion with invas

Pythiosis in the Nasal Cavity of Horses.

Two cases of nasal pythiosis are reported in horses from the semi-arid region of northeastern Brazil. From January 1986 to December 2015, the Laboratory of Animal Pathology, Federal University of Campina Grande received 830 equine samples, 156 (18.79%) of which were diagnosed with pythiosis. Of these, two horses (1.28%), a male and a female adult cross-breed, had lesions in the nasal cavity. Both horses had access to water reservoirs. Clinically, they had swelling in the rhinofacial region and a serosanguineous nasal discharge. Macroscopically, in case 1, the lesion affected the nasal vestibul

Endoscopic endonasal approach to the maxillary strut: anatomical review and case series.

OBJECTIVES/HYPOTHESIS: The maxillary strut is the bone that separates the foramen rotundum and superior orbital fissure. Tumors involving the lateral wall of the sphenoid sinus, posterior ethmoid, or posterior maxillary sinus may invade this region. The authors detail the anatomy of the strut and present a case series that emphasizes the importance and utility of this useful landmark during an endoscopic endonasal approach to lesions in this region. STUDY DESIGN: Cadaveric dissections and retrospective case series. METHODS: Endoscopic endonasal dissections were performed on six formalin-fixe

Attachment-oriented endoscopic surgical strategy for sinonasal inverted papilloma.

BACKGROUND: It is well documented that inverted papillomas (IP) have a localized attachment site. Still, instead of concentrating on the attachment site, endoscopic surgeons often perform an extended resection similar to the one achieved after external surgery. OBJECTIVE: Our objective was to evaluate an attachment-oriented endoscopic surgical strategy and to determine IP attachment diameter and location. METHODS: A prospective study was conducted. Thirty-three consecutive patients who underwent endoscopic IP excision (2001--2007) were enrolled. Thirty patients had adequate follow-up. Attach

Preservation of olfaction in anterior craniofacial approaches.

Through the combined efforts of neurosurgeons, head and neck surgeons, and craniofacial surgeons, the standard transbasal approach to the frontal fossa has been modified to include removal of the orbital roofs, nasion, and ethmoid sinuses. This approach has been combined further with facial disassembly procedures to provide extensive midline exposure to the midface and clival region. Extended frontal approaches, however, necessitate removal of the crista galli and sectioning of the olfactory rootlets with the associated risk of anosmia, cerebrospinal fluid (CSF) leak, and the need for complex

Bony Septal Grafts Enhance Aesthetic Subunits and Ventilation in Unilateral Cleft Lip Nasal Deformity.

BACKGROUND: East Asian patients with unilateral cleft lip and nasal deformity (UCLND) have severe septal deviated. An innovated septal reconstruction method using both perpendicular plate of ethmoid (PPE) and cartilage septum may improve ventilation and achieve similar aesthetic improvements as septal reconstruction using costal cartilage. METHODS: Patients with UCLND who underwent septal reconstruction using PPE or costal cartilage in the same period were retrospectively included in a 1:1 ratio. Comparison of preoperative and postoperative differences in NOSE and ROE scales and changes in sc

AI-based three-dimensional identification of safe harvesting area of perpendicular plate of ethmoid in rhinoplasty.

BACKGROUND: In rhinoplasty, using the perpendicular plate of the ethmoid (PPE) as a graft offers advantages such as strong support and avoiding costal cartilage use, but identifying a safe harvesting area (inferior one-third portion) is challenging for surgeons. This study aimed to develop an AI-based 3D segmentation model for precise identification of PPE and safe harvesting area from computed tomography (CT) images. METHODS: CT data from 200 patients were manually segmented in Mimics 20.0 to define PPE and its safe harvesting area, with boundaries based on anatomical landmarks. The robust m

Frontoethmoidal mucocele following pediatric craniofacial surgery.

BACKGROUND: Mucoceles occur as a result of accumulation and retention of mucous secretions in a paranasal sinus and are uncommon in the pediatric age group. Persistent or intermittent closure of its ostium through a variety of causes, including previous surgery, is implicated in etiology. The authors report 2 cases of frontoethmoidal mucocele that followed box osteotomies for the treatment of orbital dystopia, with medical literature review and discussion of possible causal factors and events. METHODS: Case histories and radiological imaging are presented on 2 patients presenting with frontoe
