Serial electroencephalograms (EEGs) and multimodality evoked potentials (EPs) were performed along with neurological and neuropsychological evaluation, cerebrospinal fluid assessment and magnetic resonance imaging at 6 month intervals in 73 neurologically asymptomatic HIV infected subjects. The results were compared with 50 age- and sex-matched controls. EEG was abnormal in 2 subjects (3%) initially and was abnormal in 7 (9%) subjects by the last examination. EEG abnormality (diffuse slowing) correlated significantly with slowed reaction time in neuropsychological testing (P < 0.05). VEP and B…

Per-operative use of S.E.P.s for vascular disease is based on: 1. The relationship between electrical cortical responses and cerebral blood flow. 2. The existence of a reversible threshold of cerebral ischemia. Intra-operative monitoring S.E.P.s were used during 30 procedures for aneurysms of the middle cerebral artery (M.C.A.). In 18 cases, a temporary occlusion of M.C.A. was necessary. Occlusion times ranged from 1 to 30 minutes. The central conduction time delay (C.C.T.), i.e. the delay N14-N20 and the cortical peak (N20) amplitude, elicited by median nerve stimulation was bilaterally monit

We examined brainstem auditory evoked potentials (BAEPs) in 28 children with cerebrovascular diseases and 38 normal control subjects and investigated the neuroanatomic correlations of the late waves (VI and VII) of the BAEPs. The patients included 19 patients with moyamoya disease and 9 with other cerebrovascular accidents. Wave VI was more consistently seen (only missing in 2 cases, 5%-2 ears, 3%) than wave VII (missing in 26 cases, 68%-40 ears, 53%) in the control group. Absent, depressed or prolonged wave VI correlated well with cerebral lesions involving internal capsule and basal ganglia,…

We investigated the somatosensory evoked potentials (SEPs) produced by posterior tibial nerve (PTN) stimulation in 8 infants and children with focal spinal cord disorders. The spinal responses of the PTN-SEPs were considered to assist in the localization of spinal lesions because their abnormalities were consistent with the neurologic and/or radiologic findings in all 6 examinations that revealed abnormal spinal SEPs. The cortical responses correlated significantly with proprioception in the lower limbs because proprioception was only disturbed when the cortical SEPs were absent (3 examination…

Preoperative recording of evoked potentials after stimulation of the tibial nerve was followed by similar intraoperative recording in 6 patients with cauda and conus terminalis tumours and 2 patients with intervertebral disc prolapse at lumbar level. The shape and latency of these potentials were sensitive indicators of the level and degree of damage to the nervous elements in this area. The selectivity of the method was significantly higher during intraoperative recording. Monitoring of these potentials may serve for control of the integrity of sensory pathways during neurosurgical interventi…

An animal model of lumbar spinal stenosis was developed in which the pathophysiology of this condition could be examined. Four experimental groups, each containing six dogs, were studied. One group had a laminectomy of the sixth and seventh lumbar vertebrae only; these animals served as controls. In the three other groups, a laminectomy was performed and the cauda equina was constricted by 25, 50, or 75 per cent to produce chronic compression. Cortical evoked potentials were recorded preoperatively, immediately after constriction, and at one, two, and three months postoperatively. Daily neurol…

Informativeness of different methods used for registration of optic generated potentials in response to light flash (FOP) has been studied in 27 healthy persons and 21 patients (32 eyes) with partial atrophy of the optic nerve. It is shown that the most informative one for diagnosing optic nerve atrophy and determining visual acuity in such patients is a method of investigation of the critical incidence of rythm reproduction of FOP light flickerings. Considering the complexity and difficulty of the mentioned method for its practical usage, a simplified method is proposed; a registration of FOP

Noninvasive sensory evoked potentials (SEP) performed at bedside in the Intensive Care Unit for patients in coma can be helpful in establishing both a diagnosis and a prognosis. Based on a more than 6-year experience on this subject, the authors discuss general aspects concerning these EP, their probable known generators, and propose a classification depicting different aspects observed for flash visual EP (F-VEP), brainstem auditory EP (BAEP), and median nerve somato-sensory EP (SSEP). Isolated, SSEP shows the best diagnostic and prognostic performance. Nevertheless, the authors consider that…

Very unusual scalp recorded SEPs were seen in a case after removal of a large intraventricular meningioma. One day before death (ten days after surgery) when the patient was moribund, SEPs from the clinically and radiologically affected hemisphere in which a hematoma and thalamic infarct were found, had a normal latency and were decreased in amplitude. SEPs from the clinically and radiologically nonaffected hemisphere, where only minimal findings were seen, had an abnormally short latency (16.9 msec) and the CCT was only 3.1 msec. We do not offer any logical explanation of these unusual SEP fi

19 patients with clinically definite, probable or possible multiple sclerosis were examined by means of visual evoked potentials and magnetic resonance imaging. The patients were classified in three groups according to the criteria of Mac Alpine. 11 patients had definite multiple sclerosis: 8 of these patients, who had visual disorders evoking an optic neuropathy, had abnormal visual evoked potentials and 7 of these 8 patients had a pathological magnetic resonance imaging. In the other 3 patients of this group without optic neuropathy. 2 of them had abnormal visual evoked potentials and the 3 …

Pattern visual, somatosensory and brainstem auditory evoked potentials (EPs) of 14 patients with definite multiple sclerosis, 222 patients suspected of having multiple sclerosis, 26 patients with isolated optic neuritis and 40 patients with a chronic not diagnosed neurologic disorder, were compared with their clinical diagnoses on 2 1/2-year follow-up. In the MS suspects, an EP abnormality demonstrating a clinically silent lesion in any modality (65 patients) was associated with a 71% chance of clinical deterioration (48% chance of definite MS within the follow-up period). Normal EPs (121 pati…

Twenty-one eyes of thirteen patients suffering from optic neuritis were examined with the Octopus automated perimeter and pattern visually evoked cortical potentials. All eyes had visual acuity of 0.2 or better. Visual field examination was measured by program 31 or 33, which tests the central 30-degree fields in a 6-degree grid. Pattern evoked potentials were obtained by reverse checkerboard stimulation of 3 rev/sec. A prolonged P100 latency was related to at least one abnormal point in the central nine points of program 31. All seven eyes with one abnormal point in the center had a delayed p…

Arthroscopy : the journal of arthroscopic & related surgery : official publication of the Arthroscopy Association of North America and the International Arthroscopy Association1988eng
With the increase in the use of shoulder arthroscopy in the past decade, there has been an increased awareness of complications. Reports of the occurrence of transient neuropraxia indicate an incidence of 10%-30%. The recording of somatosensory evoked potentials (SEP) for the study and functional monitoring of the sensory pathway is well accepted as a reproducible method of monitoring peripheral nerve and spinal cord function during surgery. SEPs were recorded during shoulder arthroscopy in 20 patients to monitor the musculocutaneous nerve, ulnar nerve, and either the median or radial nerve. I…

Flash-evoked visual potentials (VEPs) recorded from the scalp were used in a series of 35 patients with tumors along the visual pathway: 3 orbital tumors, 25 perisellar tumors, 4 intraventricular tumors, and 3 occipital lesions. Preoperatively, various combinations of impaired visual fields and visual acuity were observed in over 90% of the patients. A postoperative decrease in visual function was observed in 3 cases. Of the 25 perisellar lesions, 13 were operated through a standard frontotemporal craniotomy and 12 were operated through a transnasal-transsphenoidal approach. VEPs were highly s
