Nodular fasciitis with intra-articular presentation within the knee.

A female in her early 30s presented with worsening right knee pain with atraumatic onset. Imaging revealed a heterogeneous, contrast-enhancing, T1 hypointense mass in the retropatellar fat pad. Initial differential diagnosis favoured localised tenosynovial giant cell tumour. Malignancy was also considered. These alternative pathologies require more aggressive treatment modalities and demonstrate significant recurrence rates. The patient ultimately underwent open excision and histological analysis which revealed intra-articular nodular fasciitis (NF). Detection of ubiquitin-specific protease 6 …

Case Report on Necrotizing Fasciitis after Liposuction and Abdominoplasty.

Liposuction and abdominoplasty are common cosmetic procedures, but they can lead to severe complications such as necrotizing fasciitis (NF), which is a life-threatening infection with rapid progression and significant mortality rates. NF can develop after surgical procedures due to bacterial colonization or inadequate postoperative care. In the case shown here, a 33-year-old woman presented with skin flap fascial necrosis with abdominal wall infection 20 days after undergoing abdominoplasty with liposuction at another private clinic. Initial management included incision and drainage of an abdo…

Pediatric eosinophilic fasciitis: similarities and differences with adult forms.

The purpose of this study was to identify pediatric eosinophilic fasciitis, which is an extremely rare condition, in order to describe their clinical, paraclinical, and therapeutic characteristics. We made a call for observations via societies for pediatric rheumatology in France and surrounding countries and collected clinical and paraclinical data of the cases fulfilling the diagnostic criteria. Eight patients under 18 years of age with confirmed eosinophilic fasciitis followed between April 2004 and July 2022 in France, Germany, Italy, and Spain were included. The median age of onset of sym…

[Fatal retroperitoneal necrotizing fasciitis due to Aeromonas caviae septicaemia].

Aeromonas is well-recognized for causing diarrhea and post-traumatic wound infections. The most common Aeromonas species include Aeromonas hydrophila, Aeromonas caviae, and Aeromonas sobria. In cases of immunocompromise and malignancy, Aeromonas infections can prove fatal. Instances of deadly necrotizing fasciitis in the extremities due to Aeromonas infection have been documented. Herein, a case of previously unreported fatal retroperitoneal necrotizing fasciitis involving Aeromonas caviae in a patient with a history of gastric cancer is presented.

Necrotizing Fasciitis Post-Cesarean Section Leading to Transabdominal Hysterectomy.

Necrotizing fasciitis (NF) is a rare but life-threatening disease characterized by rapidly spreading inflammation and subsequent necrosis of the fascial planes and surrounding tissues. Limited literature has described NF as involving an adjacent solid organ beyond fascial planes that has required its removal. We present a case of a 25-year-old white female who underwent a cesarean section and subsequently developed NF involving her uterus and abdominal wall that necessitated a total abdominal hysterectomy, serial surgical debridement of necrotic tissue, and wound vacuum assisted closure (VAC) …

Monomicrobial Gram-Negative Necrotizing Fasciitis of the Lower Extremity.

Unlike other skin and soft tissue infections, necrotizing fasciitis (NF) is a rare and potentially life-threatening condition. It is usually caused by polymicrobial infections or monomicrobial gram-positive organisms, mainly Staphylococcus and Streptococcus. Monomicrobial gram-negative Escherichia coli (E. coli) NF is a rare form of NF, primarily reported in patients with underlying comorbidities or immunocompromised states. Here, we present a novel case of monomicrobial gram-negative NF caused by E. coli in an otherwise healthy patient. This case highlights that E. coli NF cannot be solely at…

Emphysematous cystitis revealed by necrotizing fasciitis: An unusual presentation.

Emphysematous cystitis (EC) is a severe infection of the bladder that usually affects older women with diabetes mellitus. In rare situations, EC can lead to serious complications such as necrotizing fasciitis. We report the case of a 77-year-old woman who developed emphysematous cystitis complicated with rapidly progressive, gas-producing, necrotizing inflammation located on the right lower abdominal wall. The patient had aggressive drainage and debridement in order to control the extensive necrosis. To our knowledge, this is the third case report that describes a possible association between …

Acute necrotic arachnidism with necrotising fasciitis and recurrent pneumothorax.

A male infant presented with swelling of the left leg and fever. Over the next 2 days, the area developed fasciitis extending to the left thigh, abdomen, and lower chest. Meanwhile, the parents found a giant brown spider within the infant's cot belonging to the genus Loxosceles, otherwise called the brown recluse spider. The dermo-myonecrosis progressed to deeper tissues involving the lung parenchyma requiring invasive ventilation. CT of the thorax showed multiple pneumatoceles, and lung biopsy showed alveolar necrosis. The infant was treated with intravenous antibiotics and corticosteroids. W…

Therapeutic considerations for patients with chronic plantar fasciitis (Review).

Heel pain is a common complaint in the population, leading to a limitation in daily activities and a poor quality of life. Chronic plantar fasciitis is the most common cause of heal pain. Despite its name, which suggests inflammation, the underlying process is rather a degenerative one. The clinical course is often chronic or relapsing, as a challenge to physicians to find the most effective therapeutically approach. First-line treatment consists of rest, shoe modification, orthosis and physiotherapy. Physical exercise is an important tool which can be combined with other therapeutic options. …

MRSA Causing Necrotizing Fasciitis in 18-Month-Old Boy.

Necrotizing fasciitis is a rare and fulminant infection of the superficial fascia and subcutaneous cellular tissue. It is commonly polymicrobial, with the combination of aerobic and anaerobic bacteria, which contributes to the rapid progression and severity of the disease. The microbes commonly involved include group A streptococcus, Enterobacteriaceae, anaerobes, and Staphylococcus aureus. Over the past few years, skin and soft tissue infections, including necrotizing fasciitis, due to methicillin-resistant Staphylococcus aureus are increasing.

[Comprehensive treatment of 25 cases of acute necrotizing fasciitis].

From May 2013 to March 2020, 25 patients with acute necrotizing fasciitis were admitted to Beijing Chaoyang Emergency Rescue Center, including 18 males and 7 females, aged 7 to 78 years. The lesions were mainly located in the lower extremities, with the original lesion areas ranging from 20 cm×15 cm to 83 cm×42 cm. After admission, comprehensive systemic treatment was performed, the lesion area was cut open and drained as soon as possible, and the necrotic tissue was removed. Vacuum sealing drainage was performed when the necrotic tissue in the wound was reduced. The wounds were directly sutur…

Cervicofacial necrotizing fasciitis after topical application of herbal medicine.

Cervical necrotizing fasciitis represents an aggressive form of deep neck space infection with a high mortality rate. The origin is generally odontogenic, in most cases, resulting from a dental abscess. A series of three cases developed after local application of herbal medicine in patients with no co-morbidities and with a history of a toothache in the lower quadrants for >2 weeks is presented. All patients were managed with antibiotics, extraction of offending tooth and serial wound debridement. Two patients recovered with a resultant skin defect on the neck and one died due to multiple orga…
