Early concepts of fever as a major feature of illness range from those of Hippocrates to those present in the Bible and have influenced cultural attitudes during several major European pandemics of both black plague and tuberculosis, the "white plague" of the early 19th century. Evolving ideas of thermoregulation and fever in the 19th century followed the first extensive use of the clinical thermometer by Wunderlich. Experimental studies on the pathogenesis of fever during the last 30 years suggest that fever and certain aspects of both immunoregulation and inflammation are produced by a singl…

This study attempted to find the incidence of scarlet fever in the Oxford region, including the proportion of patients from whom Streptococcus pyogenes could be isolated. General practitioners collected throat swabs from patients with suspected scarlet fever. The swabs were examined for viral and bacterial pathogens. Hepatic lesions were studied for the first time in 13 cases of boutonneuse fever (Mediterranean exanthematous fever). The glutamic-oxalacetic transaminases were raised in eight patients, the glutamic-pyruvic transaminases showed an increase in 10 patients, alkaline phosphatases in seven of the 10 patients investigated, and conjugate bilirubin showed moderate increases in three patients. Five patients were studied histologically; this study showed lesions of a granulomatous type, similar to those described in Q fever, in three patients, fatty degeneration with marked alcoholism in another patie…

Q fever was first described in Australia and has since been reported from many countries. Rickettsia burneti infects numerous species of mammals and has been isolated from ticks. Infected cattle, sheep and goats remain healthy but excrete the organism, which is present in high concentration in products of conception and survives in dust for long periods. The ways in which humans may become infected are considered. Many of these infections do not cause symptoms, but some are fatal and the prevention of this disease remains a challenge.
