Effects of general anaesthetics on the pH of gastric contents in man during surgery: a survey of halothane, fluoroxene and cyclopropane anesthesia.

The effect of premedication and three general anaesthetics on gastric content pH was investigated. Neither premedication with pentobarbital-atropine nor morphine-scopolamine given 1-2 h prior to anaesthesia appeared to affect the acidity of the gastric contents. Halothane invaribly increased the pH of the gastric contents; none of the seven patients studied had a gastric pH of less than 2.5 (mean 5.1) after 1 h of anesthesia. Cyclopropane uniformly maintained the acidity of the gastric contents; only one out of seven patients had a gastric content pH above 2.5 (mean 1.7) after 1 h of anaesthes

Defining failure to rescue in emergency general surgery on the Eve of Centers for Medicare and Medicaid Services adoption: A scoping review.

BACKGROUND: Failure to rescue is an established surgical quality metric. The Centers for Medicare and Medicaid Services will replace Patient Safety Indicator-04 with a new failure-to-rescue measure in 2027. However, no standardized failure-to-rescue definition exists for emergency general surgery, and evidence supporting complication inclusion is lacking. We sought to examine whether failure-to-rescue definitions vary across emergency general surgery studies, how these definitions align with national frameworks, and characterize complication-mortality associations. METHODS: A scoping review w…

Effects of Desflurane and Propofol General Anesthesia on Postoperative Recovery Quality in Elderly Patients Undergoing Spinal Surgery: A Randomized Non-Inferiority Trial.

PURPOSE: Postoperative recovery quality is a key concern for elderly patients undergoing spinal surgery, yet evidence comparing different anesthetic techniques in this population remain limited. This study aimed to compare the quality of recovery between desflurane-based inhalational anesthesia and propofol-based total intravenous anesthesia in this population. PATIENTS AND METHODS: This randomized non-inferiority trial was conducted from July 1 to October 31, 2025. A total of 109 elderly patients scheduled for spinal surgery were randomly assigned to two groups: the desflurane group (group D

Effect of Ultrasound-Guided Sciatic and Saphenous Nerve Blocks on Analgesia During General Anaesthesia for Ankle Fracture Surgery: A Randomised Controlled Trial.

Background and Objectives: Ultrasound-guided sciatic and saphenous nerve blocks are widely used as analgesic alternatives for multimodal analgesia. However, how to objectively quantify the opioid- and anaesthetic-sparing effects of an additional peripheral nerve block in patients undergoing ankle fracture surgery under general anaesthesia remains unclear. This study aimed to evaluate their efficacy in managing postoperative pain and opioid-sparing effects following ankle fracture surgery. Materials and Methods: Patients scheduled for ankle fracture surgery were randomly assigned to the nerve b

BIS Versus End-tidal Anaesthetic Gas Concentration-guided General Anaesthesia for Prevention of Intraoperative Awareness in Elective Surgery: A Randomised Controlled Trial.

BACKGROUND: Awareness during general anaesthesia (GA) is a rare but serious complication, affecting 0.15%-0.2% of patients, with potential psychological consequences such as post-traumatic stress disorder (PTSD). Bispectral index (BIS) monitoring, which uses electroencephalography (EEG)-derived metrics to assess anaesthesia depth, has shown promise in reducing intraoperative awareness, though its universal adoption remains limited due to mixed evidence. PURPOSE: This study aimed to compare BIS-guided and end-tidal anaesthetic gas concentration (ETAG)-guided techniques in a randomised controll

A Step Beyond Mortality: Identifying Factors of Prolonged Hospital Stay for Emergency General Surgery Conditions in a Low- and Middle-Income Country.

INTRODUCTION: While various factors leading to prolonged length of stay (LOS) have been identified for emergency general surgery (EGS), there is limited literature on specific factors for individual emergent specialties. This study aimed to identify patient factors and in-hospital complications associated with prolonged LOS for gastrointestinal (GI) and non-GI-related EGS presentations in a low-resource setting. METHODS: Data from 2010 to 2019 were retrieved from one of the largest tertiary care centers in Pakistan. We included adult patients (≥18 y) with index admissions for primary EGS cond…

The Recovery of Ciprofol, Remimazolam and Remimazolam-Flumazenil for General Anesthesia Undergoing Fundus Surgery: A Single-Center, Prospective, Randomized, Controlled Clinical Study.

PURPOSE: Ciprofol (HSK3486 injectable emulsion) is a 2.6-disubstituted phenol derivative that acts similarly to propofol with a fast onset, quick, and stable recovery. Moreover, remimazolam is a new benzodiazepine with a faster onset and recovery than midazolam, and more stable hemodynamics than propofol. We found no relevant literature that directly compared these two drugs. The primary objective of this study was to compare the recovery of HSK3486 with that of remimazolam during successful general anesthesia. PATIENTS AND METHODS: This prospective, randomized, double-blind study included 93

Predictors of postoperative delirium in paediatric patients undergoing surgery under general anaesthesia at Amhara Regional State Tertiary Hospitals: a multicenter prospective study.

INTRODUCTION: Postoperative delirium in paediatric patients is a recognised issue. Nevertheless, in low- and middle-income nations, researchers have had luck in determining its extent and predictors. Identifying predictors of postoperative delirium in paediatric patients having general anaesthesia at Tertiary Hospitals in Ethiopia was the aim of this study. METHODS: A multicenter, prospective follow up study was conducted from April 15 to June 15, 2023 at the study settings. During the study period a total of 424 paediatric surgical patients treated under general anaesthesia in all study loca

Combined General and Regional Anesthesia for a Patient With Duchenne Muscle Dystrophy With an Implanted Left Ventricular Assisted Device Undergoing Orthopedic Surgery.

Duchenne muscular dystrophy (DMD) is an X-linked inherited dystrophinopathy, with an incidence of 1 in 3,600 - 5,000 male live-born infants. The leading cause of death is often cardiomyopathy-related heart failure. Given the progressive nature of the disorder with involvement of skeletal muscle, respiratory and cardiac function, perioperative care remains challenging with an increased incidence of perioperative morbidity and mortality. Perioperative care can be challenging due to life-threatening perioperative adverse events related to associated end-organ effects, as well as sensitivity to va

The Effect of Brief Warming during Induction of General Anesthesia and Warmed Intravenous Fluid on Intraoperative Hypothermia in Patients Undergoing Urologic Surgery.

Background and Objectives: Transurethral urologic surgeries frequently lead to hypothermia due to bladder irrigation. Prewarming in the preoperative holding area can reduce the risk of hypothermia but disrupts surgical workflow, preventing it from being of practical use. This study explored whether early intraoperative warming during induction of anesthesia, known as peri-induction warming, using a forced-air warming device combined with warmed intravenous fluid could prevent intraoperative hypothermia. Materials and Methods: Fifty patients scheduled for transurethral resection of the bladder

An audit of secondary prevention for peripheral arterial disease in primary care - scope for improved collaboration between vascular surgery and general practitioners.

BACKGROUND: Symptomatic peripheral arterial disease (PAD) is a common cause for referral from primary care to vascular surgery. Best medical therapy (BMT), encompassing anti-platelets, statins, smoking cessation, blood pressure and glycaemic control, is a cornerstone of PAD management. However, these easily modifiable risk factors are often left unaddressed between referral and clinic review. METHODS: A prospective audit of electronic 'Healthlink' referrals by GPs to the vascular department for symptomatic PAD between July 2021 and June 2022 was performed. Referrals were individually reviewed

Construction of a computational mechanical model of bronchi for practical simulation of the optimal positive intrathoracic pressure conditions during general thoracic surgery.

BACKGROUND: Thoracic CO2 insufflation with positive intrathoracic pressure is usually effective during thoracoscopic surgery, however, lung collapse is sometimes insufficient. We hypothesized that inappropriate bronchial collapse might cause this unsuccessful lung collapse. OBJECTIVE: The objective of this study was to construct a computational mechanical model of bronchi for practical simulation to discover the optimal conditions of positive intrathoracic pressure during thoracoscopic surgery. METHODS: Micro-focus high-resolution X-ray computed tomography measurements of lungs from just-sla
