7 for 11: Updates in pediatric anaphylaxis.

Historically, one of the biggest barriers to understanding and diagnosing anaphylaxis has been a lack of consensus definition and clinical tools to support the standardization of anaphylaxis recognition and management. This review details important updates to the field on the subject of pediatric anaphylaxis in the past year. In 2025, a new consensus definition and clinical tool for anaphylaxis was published by an international expert panel with endorsement from 46 international, multidisciplinary medical and patient advocacy organizations. There have also been important changes to management

Plan-related inequalities in costs, disease progression and mortality among hypertensive patients in China.

Medical payment plans shape financial protection and access to care, but their associations with costs, disease progression and survival in hypertension remain poorly characterised. We analyse 8,004,039 inpatient admissions among 4,675,738 patients in Henan, China, from 2019 to 2024 using mixed-effects, multistate and survival models, and externally validate the non-mortality findings in Xinjiang. Compared with self-pay, urban employee, urban-rural resident and commercial or other insurance are associated with 24.95%, 8.71% and 16.64% higher total inpatient expenditure, respectively, while all

Comparison of acute toxicities between contemporary forward-planned 3D conformal radiotherapy and inverse-planned intensity-modulated radiotherapy for whole breast radiation.

The use of inverse-planned intensity-modulated radiation therapy for whole breast radiation treatment has become more prevalent, but this may impose an increased cost on the health system. We hypothesized that when applied with the same treatment planning goals, tangential forward-planned field-in-field 3D conformal radiotherapy and tangential inverse-planned intensity-modulated radiotherapy would be associated with comparable toxicities. Women who underwent tangential whole breast irradiation at our institution from 2011 to 2015 planned using either forward-planned field-in-field 3D conformal

Assessment of Patient Functioning in Transitional Care: An Observational Study.

BACKGROUND: Transitional care requires effective coordination between hospital and community services to ensure continuity of care and appropriate post-acute rehabilitation pathways. In this context, physiotherapists may contribute to discharge planning through standardized assessment of patients' functional status and rehabilitation needs. OBJECTIVE: This study aimed to assess patients' functional status at hospital discharge, investigate variability in post-acute care allocation across three hospitals, and explore the contribution of physiotherapist-led functional assessment to hospital-to-

Coverage of Nonpharmacologic Treatments for Low Back Pain Among US Public and Private Insurers.

IMPORTANCE: Despite epidemic rates of addiction and death from prescription opioids in the United States, suggesting the importance of providing alternatives to opioids in the treatment of pain, little is known regarding how payers' coverage policies may facilitate or impede access to such treatments. OBJECTIVE: To examine coverage policies for 5 nonpharmacologic approaches commonly used to treat acute or chronic low back pain among commercial and Medicare Advantage insurance plans, plus an additional 6 treatments among Medicaid plans. DESIGN, SETTING, AND PARTICIPANTS: Cross-sectional study

Content, timing, participants and documented outcomes of family meetings in a specialist palliative care unit in Portugal: a retrospective descriptive study.

BACKGROUND: Family meetings are a core communication strategy in palliative care, but their content in routine clinical practice remains insufficiently described. This study aimed to characterize the timing, participants, goals, problems identified, and decisions reached in family meetings conducted in a specialist palliative care unit. METHODS: We conducted a retrospective descriptive study of all formally documented family meetings held in a certified palliative care unit in Portugal between October 2012 and December 2019. Data were extracted from standardized meeting records and electronic

The river meeting the sea: A qualitative exploration of the healthcare transition experiences of adolescents and young adults living with rare renal disorders and their parents.

BACKGROUND: Rare renal disorders are a group of complex conditions that can lead to progressive kidney failure and lifelong multi-system complications. Upon reaching young adulthood, adolescents and young adults must navigate the healthcare transition between paediatric and adult services. This process serves to bridge the gap between health services and provide adolescents and young adults with developmentally appropriate support to manage adult life with their condition. However, this process can prove challenging for adolescents and young adults with rare renal disorders, a research area th

A Machine Learning-Based Clinical Decision Support System to Improve End-of-Life Care.

End-of-life care (EoLC), such as advance care planning, advance directives, hospice, and palliative care consults, can improve patient quality of life and reduce costs, yet such interventions are underused. Machine learning-based prediction models show promise in identifying patients who may be candidates for EoLC based on increased risk of short-term (less than 1 year) mortality. Clinical decision support systems using these models can identify candidate patients at a time during their care when care teams can increase the provision of EoLC.Evaluate changes in the provision of EoLC with imple

Predictors of caesarean intent and effect of unmet health conditions: evidence from Kerala, India.

BACKGROUND: India's rising dependence on institutional childbirth through Caesarean sections has intensified both the health-related risks and the financial pressures experienced by women and their households. The escalating costs of Caesarean deliveries have become a significant source of out-of-pocket spending, with the burden falling most heavily on low-income families in Kerala. OBJECTIVE: This study aims to identify the cognitive, emotional, and healthcare related predictors that influence women's intentions to opt for elective Caesarean delivery. It seeks to understand the underlying fa

Applying the Behavior Change Wheel to design physical activity interventions for women undergoing the menopause transition.

OBJECTIVE: Physical activity (PA) is critical for mitigating cardiovascular disease risk during the menopause transition, yet participation often declines during this life stage. This study applied the Behavior Change Wheel (BCW), a comprehensive framework for designing evidence-based interventions, to identify determinants of PA and develop tailored strategies to support women in meeting UK PA guidelines. METHODS: We conducted a behavioral diagnosis by integrating primary data from three focus groups with perimenopausal and postmenopausal women (n=14, 51.5±2.2 years) and findings from six qu

Maternal attachment and perception of motherhood in relation to third-trimester antenatal depression: a cross-sectional analysis.

Antenatal depression is a common complication of pregnancy, with consequences spanning maternal mental health, obstetric outcomes, and early mother-infant adaptation. Effective early identification requires integrating psychological and contextual information alongside validated screening. This study examined whether adult attachment style and the perception of motherhood are associated with antenatal depressive severity in late pregnancy, beyond socio-demographic factors. In a cross-sectional analysis of 140 third-trimester women, adult attachment (Revised Adult Attachment Scale, R-AAS) and d

Icing or cake? Grant competitions as a model for funding chronic disease prevention in Tasmania, Australia.

Competitive grant funding is a well-established mechanism for generating activity and interventions in the field of chronic disease prevention. Yet grant competitions may be burdensome for organizations, and money may not be enough to bring about lasting change in communities. In this study, we explore the dynamics of awarding and receiving money in the context of a state-level government grant competition to support community organizations and promote community-driven action for health and well-being in Tasmania, Australia. Drawing on reflections of successful grant recipients and real-time o

Data-driven decision-making for district health management: a cluster-randomised study in 24 districts of Ethiopia.

BACKGROUND: Use of local data for health system planning and decision-making in maternal, newborn and child health services is limited in low-income and middle-income countries, despite decentralisation and advances in data gathering. An improved culture of data-sharing and collaborative planning is needed. The Data-Informed Platform for Health is a system-strengthening strategy which promotes structured decision-making by district health officials using local data. Here, we describe implementation including process evaluation at district level in Ethiopia, and evaluation through a cluster-ran

' … they just take one pill, so it is easy to use, more convenient … ': South African health care provider perspectives on a Dual Prevention Pill.

The Dual Prevention Pill (DPP) is a daily oral pill containing oral pre-exposure prophylaxis (PrEP) for protection against HIV and a combined oral contraceptive (COC) for the prevention of unintended pregnancy, currently under development. However, little is known about healthcare providers' (HCPs) perspectives on this potential new multipurpose prevention technology (MPT). A more granular understanding of HCP views about the DPP may support efforts to ensure equitable introduction of and access to this innovative technology. We conducted in-depth interviews with 17 HCPs working at family plan
