Effect of medication and psychotherapy on heart rate variability in panic disorder.

BACKGROUND: Panic disorder (PD) patients have been shown to have reduced heart rate variability (HRV). Low HRV has been associated with elevated risk for cardiovascular disease. Our aim was to investigate the effects of treatment on heart rate (HR) in patients with PD through a hyperventilation challenge. METHODS: We studied 54 participants, 43 with Diagnostic and Statistical Manual of Mental Disorders (DSM-IV) PD and 11 controls. Subjects lay supine with their heads in a plastic canopy chamber, resting for 15 min and then breathing at a rate of 30 breaths per minute for 10 min. HRV was sampl…

The relationship between heart rate variability and inflammatory markers in cardiovascular diseases.

INTRODUCTION: Recent evidence implicates a cholinergic anti-inflammatory pathway. Because vagus nerve activity mediates some heart rate variability (HRV), this qualitative review examines the literature concerning circulating cytokines and HRV in cardiovascular function in humans. This qualitative review examines the literature concerning circulating cytokines and HRV in cardiovascular function in humans. METHODS: Thirteen studies on HRV, inflammation, and cardiovascular function were located by electronic library search and descriptively reviewed. RESULTS: The relationship between HRV and i…

Combined effect of cigarette smoking and sulfur dioxide on heart rate variability.

Cigarette smoking (CS) and air pollution can both alter autonomic cardiac function, yet little has been reported on their combined effect on cardiovascular dysfunction. Therefore, we assessed the potential effect of smoking on the association between sulfur dioxide (SO(2)) and heart rate variability (HRV) in community residents. We found evidence that SO(2) induced short but dramatic decreases in HRV indices, the standard deviation of the NN interval (SDNN), low frequency (LF), and high frequency (HF), in smokers compared with non-smokers. We suggest that CS influences the decrease in HRV dire…

Sympathetic control of short-term heart rate variability and its pharmacological modulation.

The static relationship between heart rate (HR) and the activity of either vagal or sympathetic nerves is roughly linear within the physiological range of HR variations. The dynamic control of HR by autonomic nerves is characterized by a fixed time delay between the onset of changes in nerve activity and the onset of changes in HR. This delay is much longer for sympathetically than for vagally mediated changes in HR. In addition, the kinetics of the HR responses shows the properties of a low-pass filter with short (vagal) and long (sympathetic) time constants. These differences might be second…

Longitudinal changes in heart rate-corrected measures of exercise performance in children.

Interpretation of change in exercise performance over time in children with repaired congenital heart disease is often hampered by poor effort that limits the maximum heart rate; this is often difficult to distinguish from chronotropic impairment, a common finding in these children. In an attempt to address this limitation, we sought to examine measures of exercise performance that are corrected for heart rate in healthy children and to determine if these change with somatic growth. We studied two serial graded exercise tests in 24 healthy children at an interval of >3 years. Paired comparison…

Determinants of heart rate recovery in patients with suspected coronary artery disease.

BACKGROUND: Heart rate recovery after exercise is an independent prognostic indicator for cardiovascular and all-cause mortality. The purpose of this study was to clarify the clinical determinants of heart rate recovery. METHODS AND RESULTS: We examined 114 consecutive male patients who underwent exercise myocardial perfusion single-photon emission computed tomography and echocardiography for the evaluation of suspected coronary artery disease. Heart rate recovery was obtained from the subtraction of heart rate in the first minute of recovery after exercise from maximal heart rate during exer…

The use of intrapartum electronic fetal heart rate monitoring: a national survey.

This study describes the use of intrapartum electronic fetal monitoring in Ireland. Electronic fetal monitoring (EFM) has become routine in the assessment of fetal wellbeing during labour. Current evidence indicates that the routine use of EFM leads to an increased caesarean section and operative vaginal delivery rate and a reduction in the rate of neonatal seizures. Practices and service provision related to the use and interpretation of and educational provision for electronic fetal monitoring have not been investigated in Ireland. A national survey of all (n = 22) maternity units in Ireland

Autonomic changes during wake-sleep transition: a heart rate variability based approach.

Autonomic function during sleep and wakefulness has been extensively investigated, however information concerning autonomic changes during t … logically different states; this transition starts with a decrease in the very slow oscillations in heart rate that anticipates a step-change resetting of autonomic function, followed by a decrease in sympathovagal balance towards the end of the process.

Effect of chronic maternal methadone therapy on intrapartum fetal heart rate patterns.

OBJECTIVE: Treatment of maternal opioid dependence with methadone is associated with a delay in fetal heart rate (FHR) accelerations in nonstress tests. The objective of this investigation was to determine the effect of methadone maintenance therapy on intrapartum FHR patterns. METHODS: This retrospective cohort study compared intrapartum FHR tracings from 56 methadone-treated patients > or =36 weeks gestation with a control group of nonsubstance using patients matched for maternal age, parity, gestational age, and ethnicity. Blinded FHR interpretation included the recording of baseline, vari…

Heart rate variability and turbulence in hyperthyroidism before, during, and after treatment.

Patients with subclinical and treated overt hyperthyroidism have an excess vascular mortality rate. Several symptoms and signs in overt hyperthyroidism suggest abnormality of cardiac autonomic function that may account in part for this excess mortality rate, but few studies have examined cardiac autonomic function in untreated and treated hyperthyroidism. We assessed heart rate turbulence (HRT) and time-domain parameters of heart rate variability in a large, unselected cohort of patients with overt hyperthyroidism referred to our thyroid clinic (n = 259) and compared findings with a group of n…

Heart rate reduction: a potential target for the treatment of myocardial ischaemia.

Because most of myocardial perfusion takes place during diastole, reducing the heart rate is likely to have anti-ischaemic effects. The present paper reviews the role of heart rate in relation to coronary artery disease outcome, as well as ways to reduce heart rate in the clinical setting. Nonpharmacological intervention, and particularly exercise training, has a definite effect on both heart rate reduction and the prevention of myocardial ischaemia. The protective role of beta-blockers after acute myocardial infarction is amply documented; their anti-ischaemic efficacy appears to be attributa…

Increased nonstationarity of neonatal heart rate before the clinical diagnosis of sepsis.

The clinical diagnosis of neonatal sepsis is preceded by abnormal heart rate (HR) characteristics of transient decelerations and reduced variability, which intuitively appear to be more nonstationary than normal HR variability. Our goals were to investigate stationarity of HR, and to devise measures useful for early diagnosis of neonatal sepsis. In this context, we define non-stationarity to be present when the observed data differ from surrogate data generated by stationary Gaussian noise with arbitrary linear correlations. We devised statistical methods for determining stationarity of HR dat…

Heart rate response is longer after negative emotions than after positive emotions.

Summary. Recent ambulatory findings showing comparable cardiovascular effects of positive and negative emotions are not consistent with the assumed etiological role of negative affect in stress-related diseases. We tested the hypothesis that regardless of initial reactivity, responses associated with negative emotions would be prolonged compared to responses associated with positive emotions. During 8 h, 33 healthy subjects from a general population reported their emotional arousal, emotional valence and physical activity and recorded their heart rates (HR) after a beep at each 60th min ('init…

Air pollution and heart rate variability among the elderly in Mexico City.

BACKGROUND: Suspended particles and ozone have been associated with varying degrees of cardiac autonomic dysfunction. METHODS: In Mexico City, residents from a nursing home underwent heart rate variability analysis every other day for 3 months. Indoor and outdoor PM2.5 (particulate matter less than 2.5 mm in diameter) were measured daily at the nursing home. Levels of ozone and other atmospheric pollutants were obtained from a nearby automated monitoring station. RESULTS: Of the initial 42 screened participants, 34 (81%) were followed during the study period. The 24-hour average levels of in…

Spectral analysis of fetal heart rate variability in fetuses with supraventricular extrasystoles.

OBJECTIVE: Fetal heart rate (FHR) variability is an important indicator of fetal well-being. In fetal tachyarrhythmias, however, visual analysis of FHR variability is limited. We therefore applied power spectral analysis of FHR to evaluate the fetal state. METHODS: Fetal R-R intervals were detected by means of an external ECG in 3 fetuses with supraventricular extrasystoles after cardiac malformations had been excluded by fetal echocardiography. Using an autoregressive model, power spectral densities were calculated from 20 consecutive 256-beat segments for the following frequency bands: <0.0…

Hopelessness is associated with decreased heart rate variability during championship chess games.

OBJECTIVE: Clinical observations suggest that negative affects such as helplessness/hopelessness (HE/HO) may induce autonomic duration; affects were assessed for every move after reconstruction of the games. In all games compiled, 18 situation of intense confidence/optimism and 20 of intense helplessness/hopelessness were observed. RESULTS: Intense affects of HE/HO were associated with decreasing HF-HRV (Fisher exact test, p =.003), increasing "nervousness" (p =.0005), decreasing "optimism" (p =.0005), and decreasing "calmness" (p =.0005). CONCLUSIONS: Investigation of championship chess gam

Respiratory modulation of heart rate and blood pressure during Cheyne-Stokes respiration.

Cheyne-Stokes respiration (CSR) is a form of periodic breathing associated with oscillations in heart rate (HR) and blood pressure (BP), which have previously been attributed to the effects of intermittent hypoxia and arousals from sleep. We herein review the major findings from a series of experiments, in which we explored the possibility that the ventilatory oscillations of CSR can independently modulate HR and BP. Using frequency spectral analysis, we showed that CSR in patients with heart failure causes oscillations in HR and BP that are eliminated by abolition of ventilatory oscillations,…

Baroreflex control of heart rate by oxytocin in the solitary-vagal complex.

Previous work demonstrated that oxytocinergic projections to the solitary vagal complex are involved in the restraint of exercise-induced tachycardia (2). In the present study, we tested the idea that oxytocin (OT) terminals in the solitary vagal complex [nucleus of the solitary tract (NTS)/dorsal motor nucleus of the vagus (DMV)] are involved in baroreceptor reflex control of heart rate (HR). Studies were conducted in male rats instrumented for chronic cardiovascular monitoring with a cannula in the NTS/DMV for brain injections. Basal mean arterial pressure and HR and reflex HR responses duri…
