Gastric mucus alterations associated with murine Helicobacter infection.

The C57BL/6 mouse has been shown to develop gastric adenocarcinoma after Helicobacter felis infection. This model was used to determine whether mucin and trefoil factor (TFF) expression after infection was altered in a similar fashion to the changes seen in the protective gastric mucus layer of the human stomach after H. pylori infection. Our results indicate that this mouse model mimics many of the changes seen after human H. pylori infection, including increased expression of muc4 and muc5b and loss of muc5ac. These alterations in mucin expression occurred as early as 4 weeks postinfection, …

Helicobacter pylori infection, host genetics and gastric cancer.

Helicobacter pylori infects half the world's population and is responsible for a considerable global health burden, including peptic ulcer disease and gastric cancer. The infection causes a chronic gastritis, the severity and distribution of which determine the clinical outcome. Bacterial, environmental and host genetic factors combine to define the degree of gastric damage. Most patients have a limited mild pan-gastritis with no significant clinical consequences. Antral-predominant gastritis is associated with high gastric acid output and an increased risk of duodenal ulcers. Corpus-predomina…

Review article: Helicobacter pylori-negative duodenal ulcer disease.

BACKGROUND: Helicobacter pylori infection rates in duodenal ulcer (DU) patients may be lower than previously estimated. AIM: To review the real prevalence of H. pylori-negative DUs and its possible causes. METHODS: Bibliographical searches in MEDLINE looking for the terms 'H. pylori' and 'duodenal ulcer'. RESULTS: Mean prevalence of H. pylori infection in DU disease, calculated from studies published during the last 10 years including a total of 16 080 patients, was 81%, and this figure was lower (77%) when only the last 5 years were considered. Associations with H. pylori-negative DU were:…

Helicobacter pylori infection and childhood idiopathic thrombocytopenic purpura.

BACKGROUND: Several studies showed some chronic idiopathic thrombocytopenic purpura (ITP) patients with complete platelet recovery after Helicobacter pylori (H. pylori) eradication and cited the cause of persistent thrombocytopenia as inability to eradicate H. pylori. So we studied H. pylori infection status in pediatric ITP patients at diagnosis and address whether such infection played a role in the development of childhood ITP. METHODS: We compared H. pylori infection status by stool H. pylori antigen test of an ITP group including 32 childhood ITP patients at diagnosis from September 2004…

Childhood functional abdominal pain and Helicobacter pylori infection.

BACKGROUND/AIMS: Like functional gastrointestinal disorders in adults, the exact etiology of childhood functional abdominal pain (FAP) is unclear. Among those proposed etiologies, chronic Helicobacter pylori (HP) infection plays an important role. Therefore, we conducted this study to investigate the relationship of childhood FAP and chronic HP infection. METHODOLOGY: From 1998 to 2000, pediatric patients referred to our outpatient department under the impression of recurrent abdominal pain were carefully evaluated, and only subjects with FAP were recruited for study. Endoscopic examinations …

Molecular markers in Helicobacter pylori-associated gastric carcinogenesis.

Helicobacter pylori infection is a known risk factor of gastric carcino-genesis. This article presents early molecular alterations associated with H. pylori chronic gastritis and advances in the molecular characterization of preneoplastic intestinal metaplasia (IM) and premalignant gastric mucosal lesions. H. pylori infection induces changes in gene expression, genomic instability and accumulation of gene mutations in the stomach epithelium. Mutations, including LOH and microsatellite instability, and gene hypermethylation are seen not only in gastric cancer, but are already detectable in IM a…

Helicobacter pylori outer membrane proteins and gastroduodenal disease.

BACKGROUND AND AIMS: A number of Helicobacter pylori outer membrane proteins (OMPs) undergo phase variations. This study examined the relation between OMP phase variations and clinical outcome. METHODS: Expression of H pylori BabA, BabB, SabA, and OipA proteins was determined by immunoblot. Multiple regression analysis was performed to determine the relation among OMP expression, clinical outcome, and mucosal histology. RESULTS: H pylori were cultured from 200 patients (80 with gastritis, 80 with duodenal ulcer (DU), and 40 with gastric cancer). The most reliable results were obtained using …

Potent gastric acid inhibition in Helicobacter pylori eradication.

At present, antisecretory drugs--foremost among them the proton pump inhibitors (PPIs)--represent a keystone in Helicobacter pylori eradication therapy. The present article shall first compare the role of PPIs as compared with histamine H2 receptor antagonists, both of them in the role of antibiotic-associated antisecretory therapy, and shall then address the contribution of each of the various PPIs that have been developed until the present time to the H. pylori eradication therapies. In summary, it may be concluded that PPIs are more effective overall than H2 receptor antagonists when the tw…

Helicobacter pylori impairs iron absorption in infected individuals.

BACKGROUND: Infection with Helicobacter pylori is recognised as a major risk factor for chronic gastritis, peptic ulcer disease and gastric cancer. The association between H. pylori infection and iron deficiency anaemia has been established. Multiple mechanisms have been advocated to explain the relationship between H. pylori and iron status and their association might reduce iron deposit. AIM: Aim of this study was to investigate whether H. pylori infection affects iron absorption. METHODS: The study was designed on a prospective basis. Fifty-five subjects underwent upper gastrointestinal e…

Helicobacter Pylori eradication therapy: getting research into practice.

Helicobacter Pylori (H. Pylori) is the primary cause of duodenal ulcer (DU). Guidelines recommend that all patients with DU be considered for Helicobacter Pylori Eradication Therapy (HPET). However, the proportion of patients with DU on long term anti-ulcer medication receiving HPET is small. This study examined the effectiveness of the continuing medical education (CME) network of the Irish College of General Practitioners (ICGP) in promoting best practice in DU treatment among GPs in an eastern region of Ireland. Ninty eight GPs recruited from the CME network of the ICGP were randomised in t…

Mast cells in Helicobacter pylori associated antral gastritis.

Mast cells are known to be effector cells in various inflammatory reactions, but their role in gastritis is unclear. The present study was undertaken to investigate the extent of mast cell involvement in antral gastritis with and without Helicobacter pylori (H. pylori) infection and thus evaluate the possible role of mast cells in the pathogenesis of H. pylori-associated gastritis. Antral mucosal biopsies were taken from 212 subjects with symptoms suggestive of acid peptic disease. Sections were assessed for inflammation. Modified Giemsa stain was used to detect H. pylori infection and 1% tolu…

[How to treat after Helicobacter pylori eradication failure?].

Helicobacter pylori eradication failure generally concerns between 10 and 30% of the patients. When eradication failure occurs, it is necessary a) to try to identify the potential causes, b) to confirm the indication of eradicating H. pylori, and c) to choose a second line therapeutic strategy. Among the factors most frequently associated with failed eradication, are poor compliance, younger age, smoking, and weak gastric inflammatory activity. The major factor of resistance is related to the sensitivity of the bacterial strain to antibiotics used. In France, resistance to clarithromycin is ar…

Helicobacter pylori in patients receiving long-term dialysis.

BACKGROUND: Helicobacter pylori (H. pylori) is considered to cause gastritis and gastric ulcer. In dialysis patients, digestive tract hemorrhage is sometimes fatal. However, in regard to H. pylori infection in patients with end-stage renal disease (ESRD), many issues remain to be clarified. METHODS: This study included 76 symptom-free patients with ESRD. The subjects consisted of 25 patients with chronic renal failure who had not received dialysis and 51 patients receiving dialysis. On upper digestive tract endoscopy, specimens were taken for analysis of H. pylori. Urease test, culture, and m…

Helicobacter pylori-induced activation of human endothelial cells.

Helicobacter pylori infection causes active chronic inflammation with a continuous recruitment of neutrophils to the inflamed gastric mucosa. To evaluate the role of endothelial cells in this process, we have examined adhesion molecule expression and chemokine and cytokine production from human umbilical vein endothelial cells stimulated with well-characterized H. pylori strains as well as purified proteins. Our results indicate that endothelial cells actively contribute to neutrophil recruitment, since stimulation with H. pylori bacteria induced upregulation of the adhesion molecules VCAM-1, …

[Helicobacter pylori--does it only cause gastroduodenal disease?].

Helicobacter pylori is a human pathogen that can be found all over the world. It is responsible for the following diseases of gastrointestinal tube: gastritis, gastric ulcer, duodenal ulcer, gastric cancer, gastric lymphomas, Menetier disease. Some research has been done recently trying to identify the connection between H. pylori infection and idiopathic Parkinson's Disease morbidity. Some of them show that people with this neurological disease are more likely to have ulcers and also seropositivity in the direction of H. pylori. The direct influence of H. pylori infection on Parkinson Disease…

Allelic Variation within Helicobacter pylori babA and babB.

Helicobacter pylori strains show both geographic and disease-associated allelic variation. We investigated the diversity present in two genes, babA and babB, which are members of a paralogous family of outer membrane proteins. Eleven family members within a single H. pylori strain, predicted to encode proteins with substantial N- and C-terminal similarity to each other, were classified as babA paralogues. In their central regions, most are less than 54% related to one another. Examining the babA and babB central regions in 42 H. pylori strains from different geographic locales, we identified f…
