Hemiarthroplasty versus nonoperative treatment of displaced 4-part proximal humeral fractures in elderly patients: a randomized controlled trial.

BACKGROUND: The aim of the study was to report the 2-year outcome after a displaced 4-part fracture of the proximal humerus in elderly patients randomized to treatment with a hemiarthroplasty (HA) or nonoperative treatment. PATIENTS AND METHODS: We included 55 patients, mean age 77 (range, 58-92) years, 86% being women. Follow-up examinations were done at 4, 12, and 24 months. The main outcome measures were health-related quality of life (HRQoL) according to the EQ-5D and the DASH and Constant scores. RESULTS: At the final 2-year follow-up the HRQoL was significantly better in the HA group c

Posterior arm compartment syndrome after a combined supracondylar humeral and capitellar fractures in an adolescent: a case report.

BACKGROUND: Supracondylar humeral fractures in children are common, with associated bony injuries typically being fractures of the forearm or distal radius. To our knowledge, a combined supracondylar humeral fracture associated with a Hahn-Steinthal capitellar fracture has not been reported earlier. Similarly, posterior arm compartment syndrome is extremely rare, with most instances having occurred after chronic compression and bleeding into the compartments from anticoagulation, direct trauma including crush, or tendon ruptures. Only 2 cases have been reported after a fracture of the humerus.…

Heat-induced segmental necrosis after reaming of one humeral and two tibial fractures with a narrow medullary canal.

In three cases referred to our clinic (a simple fracture of the humeral shaft, a simple, closed fracture, and a wedge fracture of the mid-third of the tibia), bone necrosis had resulted from excessive heat produced by reaming extremely narrow medullary cavities (5-5.5 mm diameter) with the 9 mm front-cutting reamer as part of a reamed nailing procedure. In any one case, different degrees of damage can occur from the metaphysis to the diaphysis. Based on the clinical course and the histological evaluation, we postulate that heat-induced damage can be divided into four degrees of severity (0-3):

Does age affect patient outcomes after humeral head replacement in the treatment of acute proximal humeral fractures? A comparative cohort study with a minimum 10 years long-term follow-up.

BACKGROUND: Humeral head replacement (HHR) is now rarely recommended for complex proximal humeral fractures (PHFs) in older patients. However, in relatively young and active patients with unreconstructable complex PHFs, controversy still exists regarding the treatment options of reverse shoulder arthroplasty and HHR. The goal of this study was to compare the survival, functional, and radiographic outcomes of HHR in patients aged <70 years and those aged ≥70 years after a minimum 10 years follow-up. METHODS: Eighty-seven out of 135 patients undergoing primary HHR were enrolled and then divided…

Use of locking humeral stems in reverse total shoulder arthroplasty for complex proximal humerus fractures: a scoping review.

The use of locked humeral stems in reverse total shoulder arthroplasty (rTSA) has emerged as an alternative fixation strategy for treating complex proximal humerus fractures (PHFs). This scoping review synthesizes the available literature evaluating clinical outcomes, radiographic findings, and complications associated with interlocking humeral stems in rTSA for PHFs. Nine studies, encompassing 434 patients, were identified, of whom 308 underwent rTSA with a locking humeral stem. The mean patient age was 75.7 ± 5.1 years, with a mean follow-up of 20.0 ± 8.9 months, and at least 54% of patients

Open reduction can be a reasonable, safe and effective choice in complex paediatric supracondylar humeral fractures operative treatment.

BACKGROUND: Supracondylar humeral fractures are among the most common paediatric injuries. Displacement and consequent need for reduction impose operative treatment. Restoration of the anatomy of the distal part of the humerus prevents deformities and functional disorders of the elbow and upper limb. Stable K-wire fixation can be succeeded either by closed or open reduction method. AIM: To investigate the safety and efficacy of open reduction method in the operative treatment of Gartland type III and IV supracondylar humeral fractures in children. METHODS: We retrospectively studied 131 case…

The effect of different internal fixation materials on Jakob type III lateral humeral condyle fractures in pediatric patients.

BACKGROUND: Humeral lateral condyle fractures are common pediatric elbow injuries, with Jakob type III representing the most severe form. While open reduction and internal fixation is the standard treatment, consensus is lacking on the optimal fixation method. Inadequate fixation can lead to serious complications. The efficacy of absorbable internal fixation materials remains debated. This study aimed to evaluate the outcomes of different internal fixation materials for treating these fractures in children. METHODS: A retrospective review was conducted on 51 children (39 male, 12 female; mean

Correlation of Radiological Parameters with Functional Outcomes in Proximal Humeral Fractures Treated with PHILOS Plates: A Retrospective Analysis.

INTRODUCTION: Proximal humerus fractures are prevalent, especially among the elderly, and pose significant challenges in treatment, particularly for displaced fractures. Despite the availability of various surgical techniques, outcomes remain inconsistent, often due to poor anatomical reduction and fixation. This study examines the correlation between specific radiographic parameters and functional outcomes, aiming to identify key predictors of recovery. By focusing on these parameters, we aim to refine surgical approaches and improve follow-up strategies, ultimately enhancing patient outcomes
