PURPOSE: The brain produces intense heat as a result of cerebral metabolism and cerebral blood flow, and the generated heat is removed mainly through circulation of the intracranial blood vessels and cerebrospinal fluid (CSF). Because magnetic resonance (MR) images are constructed from analysis of the spin of various molecules, the diffusion coefficient can be used as a parameter that reflects the temperature of water molecules. We used diffusion-weighted imaging (DWI)-based MR imaging to measure the temperature of the CSF around the lateral ventricles in patients with idiopathic normal pressu

In this case report, neuroleptic malignant syndrome (NMS) in an elderly patient with normal pressure hydrocephalus overlapping corticobasal degeneration was reported. The case highlights the need for clinicians to be cautious when using dopaminergic medication in the elderly patients, since these agents have risks for NMS which is a life-threatening complication. Additionally, co-occurrence of primary and secondary parkinsonian dementia syndromes should be kept in mind to avoid additional complications in the elderly patients.

INTRODUCTION: The diagnosis and management of idiopathic normal pressure hydrocephalus (INPH) remains unclear despite the development of guidelines. In addition, the role of cerebrospinal fluid (CSF) aqueductal stroke volume (ASV) remains unspecified.
OBJECTIVES: The aim of this study was to compare the results of the tap test (TT) and ASV in patients with possible INPH.
MATERIALS AND METHODS: Among 21 patients investigated with both TT and phase-contrast (PC) MRI, we identified two groups, with either (1) a positive TT (PTT) or (2) a negative one (NTT), and we compared their ASV as measured

PURPOSE: This study was designed to investigate the specific cerebral blood flow (CBF) pattern in patients with idiopathic normal-pressure hydrocephalus (iNPH) and a predictive value for shunt responsiveness in a multicenter study (Study of Idiopathic Normal-Pressure Hydrocephalus on Neurological Improvement: SINPHONI).
METHODS: Eighty-four iNPH patients underwent shunt operations using MRI selection criteria from the SINPHONI and were subjected to CBF single photon emission computed tomography (SPECT). The perfusion patterns on SPECT were classified: anterior-dominant CBF reduction type (A t

BACKGROUND AND PURPOSE: Because DTI can provide good markers of white matter pathology, it could be useful in differentiating white matter changes of INPH from those of other dementias. The aim of this study was, by using DTI, to compare the characteristic white matter changes in INPH with those in AD, subcortical vascular dementia, and healthy control subjects.
MATERIALS AND METHODS: Sixteen patients with presurgical INPH, 10 with AD, 10 with subcortical vascular dementia, and 20 healthy control subjects underwent DTI. All patients with INPH showed clinical improvement after shunt surgery, a

To determine the impact of cortical Alzheimer disease pathology on shunt responsiveness in individuals treated for idiopathic normal pressure hydrocephalus (iNPH), 37 patients clinically diagnosed with iNPH participated in a prospective study in which performance on neurologic, psychometric, and gait measures before and 4 months after shunting was correlated with amyloid β plaques, neuritic plaques, and neurofibrillary tangles observed in cortical biopsies obtained during shunt insertion. No complications resulted from biopsy acquisition. Moderate to severe pathology was associated with worse

OBJECTIVES: To explore diagnostic differences in periventricular white matter (PWM) and deep white matter (DWM) diffusion patterns in patients diagnosed with Binswanger disease (BD) and in patients diagnosed with probable idiopathic normal pressure hydrocephalus (INPH) using diffusion-weighted imaging (DWI).
MATERIALS AND METHODS: Apparent diffusion coefficient (ADC) values were calculated in the PWM and DWM in patients with INPH (n = 14) and BD (n = 9) and in controls (n = 10) using an spin echo echo planar imaging single-shot diffusion sequence and region of interest (ROI) analysis.

PURPOSE: In the older patient with dilated ventricles, it is often difficult to differentiate normal pressure hydrocephalus (NPH) from cerebral atrophy caused by Alzheimer disease (AD). This study was undertaken to see if dilatation of the perihippocampal fissures (PHFs) could be used as a distinguishing characteristic of these two disorders.
METHODS: MR images of 17 patients with AD were compared with those from an equal number of patients with NPH who improved after ventriculoperitoneal shunting. The PHFs, lateral ventricles, third ventricle, and temporal horns were graded subjectively. Obj

Auditory brainstem responses (ABRs) were studied in 15 adults with suspected normal pressure hydrocephalus (NPH) before and after shunting. The patients were divided into Groups A (shunt-ineffective) and B (shunt-effective). The pre- and postoperative ABRs of each patient were compared with those of 20 normal volunteers, and the relationships between ABRs and certain clinical findings were investigated. Preoperatively, nine patients (60%) showed prolonged central conduction time (CCT) (interpeak latency of wave I-V or neural-axonal conduction time) relative to the mean control value plus 2 SD.

Fourteen patients with normal pressure hydrocephalus had the autoregulation of cerebral blood flow (CBF) and intracranial pressure (ICP) investigated. In 8 of the patients the effect of Captopril on ICP and CBF was also investigated. The mean arterial blood pressure (MABP) was 109 mmHg (intra-arterially), and ICP was 11 mmHg (intraventricularly). Changes in global CBF were estimated by the arterio-venous oxygen difference method. The autoregulation of CBF was present in 13 of the patients (p less than 0.01). The lower limit of CBF autoregulation was 86% of the baseline perfusion pressure. One

Because diagnostic criteria for normal pressure hydrocephalus have not been clearly determined, it is often difficult to differentiate patients with this potentially treatable condition from those with Alzheimer-type dementia. We have studied three patients with normal pressure hydrocephalus, 17 patients with Alzheimer-type dementia, and seven healthy elderly controls using positron emission tomography and [18F]Fluorodeoxyglucose (FDG). Both Alzheimer-type dementia and normal pressure hydrocephalus groups showed lower cortical rates of FDG utilisation than controls. However, the patterns of me

Analysis of 25 patients who fulfilled clinical and radiographic criteria for the diagnosis of "normal pressure" hydrocephalus (NPH) demonstrated (1) a significant relationship between presence of motor signs with good outcome and absence of motor signs with poor outcome following ventricular shunting, (2) symptoms and signs of parkinsonism in 40% of patients in whom the diagnosis of NPH was made, and (3) no reliable relationship between radiographic measurements or cisternogram appearance and outcome following shunting. The clinical picture is the most important factor in selection of NPH pati

The authors question the current validity of the traditional concept of normal pressure hydrocephalus (NPH) as it was described by Adams and Hakim in 1965. The classic features of the disease are addressed. It is concluded that most of the historical statements made three decades ago need to be revised. Especially, the term "normal pressure" hydrocephalus probably does not match the actual manometric profile of patients with NPH. Similarly, the terms"curable"and "reversible" dementia are inadequate to designate the mental alterations of NPH. That NPH is non-specific to the adult population is

Follow-up study on ICP was carried out in 6 cases of hydrocephalus within normal head circumference for 8-9 months after birth. There were no cases with shunting operation. Abnormal plateau-like wave was recorded in 2 cases with myelomeningocele during follow-up period. In other cases of postemeningitic hydrocephalus, almost normal baseline pressure without abnormal pressure wave was recorded and all showed favorable morbidity (function). These results showed the importance of follow-up study on ICP for 9 months after birth in cases of hydrocephalus without large head circumference and shuntin

BACKGROUND: The diagnosis of normal-pressure hydrocephalus (NPH) is often complicated due to deficiencies of the objective measures currently used after test drainage of CSF. We used Arterial Spin Labeled Magnetic Resonance Imaging (ASL-MRI)-a novel, simplified, completely non-invasive, radiation-free method-to measure global cerebral blood flow (CBF) before and after performing a large-volume lumbar puncture (LVLP) in patients suspected of NPH.
METHODS: We compared baseline ASL-CBF in 20 patients (65-91 years old, mean: 75 years; 11 men) with history of recurrent falls from unsteady gait, ur

Knowledge of the genetic factors in normal pressure hydrocephalus (NPH) is rapidly evolving, with significant advances in recent years. We conducted a systematic review examining genetic contributions to NPH risk. Ovid Embase, Ovid Medline, Web of Science, and Cochrane Central were searched from inception through October 14, 2024, for human studies in English reporting familial NPH cases, genetic variants associated with NPH, and associations with other neurogenetic disorders and exploring transcriptomics. Studies on secondary, obstructive, and congenital hydrocephalus were excluded, and findi

BACKGROUND: Shunted hydrocephalus patients are at an inherent risk of developing chronic subdural hematomas (cSDHs) from overdrainage. The preliminary results of three much-anticipated clinical trials (STEM, EMBOLISE, MAGIC-MT) have recently been presented, showing efficacy of middle meningeal artery embolization (MMA-E) in reducing observational failure and the need for reoperation in non-shunted patients, but the effect of this procedure in shunted patients remains to be further investigated. In this series, we present our institution's experience with MMA-E in shunted normal pressure hydroc

Idiopathic normal pressure hydrocephalus (iNPH), the leading cause of reversible dementia in older adults, is characterized by urinary incontinence, gait impairment, cognitive decline, and ventriculomegaly. Neuroradiological features rely on visual assessment, including sulcal characteristics. This study applies automatic sulcal-based morphometry to characterize the sulcal phenotype in iNPH and to distinguish responders from non-responders. We analyzed the sulcal morphology in 32 iNPH patients and 41 healthy controls. Patients were categorized as responders (Resp) or non-responders (nResp) bas
