Coronavirus Disease 2019-Associated Invasive Fungal Infection.

Coronavirus disease 2019 (COVID-19) can become complicated by secondary invasive fungal infections (IFIs), stemming primarily from severe lung damage and immunologic deficits associated with the virus or immunomodulatory therapy. Other risk factors include poorly controlled diabetes, structural lung disease and/or other comorbidities, and fungal colonization. Opportunistic IFI following severe respiratory viral illness has been increasingly recognized, most notably with severe influenza. There have been many reports of fungal infections associated with COVID-19, initially predominated by pulmo

Gut microbiota in chronic inflammatory disorders: A focus on pediatric inflammatory bowel diseases and juvenile idiopathic arthritis.

The gut microbiota is integral to human health, including maintaining the delicate balance between tolerance and protection against potentially harmful pathogens. A growing body of evidence implicates the intestinal microbiome in immune-mediated inflammatory disorders; these data span the spectrum from genetic and environmental disease risk factors, to animal studies (particularly germ-free and gnotobiotic models) and human studies, including evidence of dysbiosis in diseased individuals compared to healthy populations. In this review, we summarize both animal and human data supporting a link

Does Complement Have a Role in the Pathogenesis of Alopecia Areata?

Alopecia areata (AA) is an autoimmune disorder in which immune attack of the anagen follicle causes hair loss in approximately 2% of the population. Although the pathogenesis of AA has not been fully determined, most likely it is mediated by a variety of factors including cellular/humoral immunity and genetic predisposition. Researchers have been interested in the possible role of the complement pathway in AA since the 1970s. Given recent evidence suggesting that complement plays a role in many immunologic and inflammatory dermatologic diseases including systemic lupus erythematosus, bullous d

Maternal HIV and Paediatric Lung Health.

With improved prevention of mother to child transmission of HIV, paediatric HIV disease is less common. However, the number of HIV exposed but uninfected infants is growing. Exposure to maternal HIV impacts infant respiratory health through an increase in known risk factors such as increased preterm birth and low birth weight, suboptimal breastfeeding, increased psychosocial stressors and increased exposure to infective pathogens. Exposure to the HIV virus and altered maternal immune environment result in immunologic changes in the infant that may contribute to respiratory disease risk. HIV ex

Inflammation and Human Ovarian Follicular Dynamics.

Inflammation is a biologic process that mediates tissue effects including vasodilation, hyperemia, edema, collagenolysis, and cell proliferation through complex immunologic pathways. In regard to the ovary, inflammation has key physiologic roles in ovarian folliculogenesis and ovulation. On the other hand, inflammatory processes are subject to underlying pathology and, if pushed, proinflammatory conditions may have a negative impact on ovarian follicular dynamics. Obesity and polycystic ovary syndrome (PCOS) serve as examples of conditions associated with chronic endogenous production of low-g

Determinants for the development of visceral leishmaniasis disease.

Leishmaniasis is a vector-borne neglected tropical disease associated with a spectrum of clinical manifestations, ranging from self-healing cutaneous lesions to fatal visceral infections. Among the most important questions in Leishmania research is why some species like L. donovani infect visceral organs, whereas other species like L. major remain in the skin. The determinants of visceral leishmaniasis are still poorly understood, although genomic, immunologic, and animal models are beginning to provide important insight into this disease. In this review, we discuss the vector, host, and patho

Advances in acquired pediatric heart disease.

Acquired heart disease in children may result in significant morbidity and mortality. Advances continue to be made in understanding Kawasaki disease, acute and chronic rheumatic heart disease, infective endocarditis, myocarditis, and dilated cardiomyopathy. The role of superantigens, particularly bacterial toxins, in the pathogenesis of Kawasaki disease continues to be defined. Intravascular ultrasound promises to improve the assessment of coronary arteries in Kawasaki disease. Current recommendations for the long-term management of Kawasaki disease are discussed. Light on the persistent light reaction-photosensitivity dermatitis-actinic reticuloid syndrome.

In this article are reviewed the various hypotheses concerning the etiology of the persistent light reaction and photosensitivity dermatitis, which occurs among older people and is characterized by an extreme photosensitivity that sometimes evolves into a pseudolymphoma, that is, actinic reticuloid. An etiologic agent can sometimes be demonstrated, but the precise pathogenetic mechanism is unknown. Clinical experience and experimental research indicate that the cause is most probably multifactoral: contact allergenic, photoallergenic, phototoxic, immunologic, and metabolic factors are involved

Necrotizing or cavitating pneumonia due to Streptococcus Pneumoniae: report of four cases and review of the literature.

Streptococcus pneumoniae is seldom considered as an etiologic agent of necrotizing or cavitating pneumonia. However, during a 5-month period we encountered four patients, bacteremic with S. pneumoniae, with such a pulmonary process. Review of the older literature indicates that this association may be more frequent than is commonly assumed. Anatomic, physiologic, and immunologic alterations of the pulmonary defense mechanisms prior to and during the infection as well as virulence factors of S. pneumoniae (i.e., rapid multiplication, accumulation of capsular polysaccharides, and inhibition of p

Effect of cow's milk on the gastrointestinal tract: a persistent dilemma for the pediatrician.

The confusing area of cow's milk intolerance is explored in an attempt to define the various mechanisms whereby milk affects gastrointestinal function, resulting in clinical symptoms (diarrhea, vomiting, gastrointestinal bleeding, etc.). The adverse reaction of infants to cow's milk ingestion may relate to lactose intolerance (enzymatic), a direct toxic reaction to the mucosal surface resulting in epithelial damage, or it may be immunologically mediated. Factors such as increased intestinal permeability to milk proteins during the newborn period may also contribute to susceptibility of young i

An update on Vitiligo pathogenesis.

Vitiligo, the most common depigmenting disorder of the skin, is undergoing a period of intense advances in both disease understanding and therapeutic possibilities leading the way to the beginning of a new era for the disorder. Its pathophysiology has gathered the attention of researchers for years, and many advances have been made in the clarification of the interaction between different factors that result in depigmented macule formation. The complex interplay between non-immunological and immunological factors in vitiligo is key for the development of the disease, and the participation of c

Eliminating Schistosomes through Vaccination: What are the Best Immune Weapons?

The successful development of vaccines depends on the knowledge of the immunological mechanisms associated with the elimination of the pathogen. In the case of schistosomes, its complex life cycle and the mechanisms developed to evade host immune system, turns the development of a vaccine against the disease into a very difficult task. Identifying the immunological effector mechanisms involved in parasite attrition and the major targets for its response is a key step to formulate an effective vaccine. Recent studies have described some promising antigens to compose a subunit vaccine and have p

Immunohistologic evidence that villitis in human normal term placentas is an immunologic lesion.

Villitis of unestablished origin is a lesion in placentas from normal and high-risk pregnancies. We have studied villitis areas in 25 normal term placentas for immune cells, coagulation components, and endothelial markers. Villitis areas were filled with activated (HLA-DR, HLA-DP, and HLA-DQ reactive) macrophages. B lymphocytes were not identified, and T lymphocytes were of the helper (CD4) phenotype. Antibodies to coagulation components revealed perivascular and trophoblastic basement membrane deposits of factor IX, increased numbers of platelets, and fetal stem vessels that did not react wit

[Atopic neurodermatitis. Clinical course--pathophysiology--therapy].

Atopic dermatitis is one of the most frequent dermatological diseases. It is a symptom in the framework of the atopic syndrome. Although it is not characteristic for infants and newborns, in respect to localization and form, typical lesions (seropapules, lichenification) and localizations (bends of the arm and knee, face and wrists) may begin at the age of four years and later. Regarding the pathophysiological aspects, non-immunological factors (decrease in the sebaceous gland secretion, reduced barrier function of skin, disturbances in sweat gland secretion) as well as immunological factors (

Immunology of the gut: role of the eosinophil.

The gut wall is one of the conspicuous sites of eosinophil accumulation, presumably because of local chemotactic stimuli. It is reasonable to assume that one chemotactic factor is released by the mast cell, which is often found in proximity to the eosinophil. The association of eosinophils and eosinophilia with allergic disorders has long been recognized, and recent work has shown that increased eosinophil production is mediated by the lymphocyte. That process shares characteristics with other immunological actions. An increased rate of eosinophil tissue accumulation and destruction may be the

Differentiating Peanut Allergy from Sensitization in Polish Children: A Real-Life Diagnostic Model.

Peanut allergy (PA) remains a major diagnostic challenge in pediatric allergy, largely due to the frequent discrepancy between immunological sensitization and clinically relevant disease. This study aimed to develop a real-life diagnostic prediction model to distinguish true peanut allergy from asymptomatic peanut sensitization in children referred for evaluation of suspected PA. In this cross-sectional study, 80 children aged 1-18 years were assessed in a tertiary allergy center in Poland. Sixty-five children with peanut sensitization underwent detailed clinical history assessment, skin prick
