The role of modeling in evaluation of maternal and child health programs: using the lives saved tool to help answer core evaluation questions.

This paper explains how The Lives Saved Tool (LiST), a computer-based model that estimates the impact of scaling up interventions on stillbirths, maternal, neonatal and child health, can contribute to evaluations of programs being delivered at scale to improve maternal and child health. LiST can be used to estimate the impact of a program in advance, allowing planners to refine, streamline and set appropriate program targets. LiST can also be used to estimate the impact of a program, which is particularly useful given the high costs of measuring changes in population health. Finally, LiST can

A Selected Review of the Mortality Rates of Neonatal Intensive Care Units.

INTRODUCTION: Newborn babies in need of critical medical attention are normally admitted to the neonatal intensive care unit (NICU). These infants tend to be preterm, have low birth weight, and/or have serious medical conditions. Neonatal survival varies, but progress in perinatal and neonatal care has notably diminished mortality rates. In this selected review, we examine and compare the NICU mortality rates and etiologies of death in different countries. METHODS: A literature search was conducted in Ovid MEDLINE, OLDMEDLINE, EMBASE Classic, and EMBASE. The primary endpoint was the mortality

Etiologies of NICU deaths.

BACKGROUND AND OBJECTIVES: Infant mortality is an indicator of overall societal health, and a significant proportion of infant deaths occur in NICUs. The objectives were to identify causes of death and to define potentially preventable factors associated with death as areas for quality improvement efforts in the NICU. METHODS: In a prospectively defined study, the principal investigator in 46 level III NICUs agreed to review health care records of infants who died. For each infant, the principal investigator reviewed the medical record to identify the primary cause of death and to look for pr…

Neonatal outcome following cesarean birth: a prospective study.

Neonatal morbidity and mortality in 573 singleton cesarean born infants was recorded. In 70% of women, it was a primary cesarean section. The neonatal mortality was 3.6% in babies born to primigravida mothers and 9.8% in infants of multigravida. Mortality in male and female infants was 5.73 and 9.65%, respectively. Preterm infants registered more than 7 times higher mortality than term infants. Birth weight below 2000 g and above 3500 g were associated with higher asphyxia rate and neonatal death. Neonatal morbidity frequently encountered was birth asphyxia (19.19%), septicemia (13.43%), jaund

Beyond coverage: Can vaccination reduce child mortality despite structural inequality? A global ecological analysis of routine childhood immunization (2010-2023).

Routine childhood immunization is still a critical topic to global public health, since it prevents an estimated 3.5 to 5 million deaths annually. However, its access remains uneven, especially in low-income countries, where structural inequalities limit the reach of immunization programs, and the Covid-19 pandemic disrupted routine services, worsening the existing disparities. This ecological study (2010-2023) examined associations between vaccine coverage and infant mortality using the WHO/UNICEF data for six childhood vaccines: BCG, DTP3, HepB3, Hib3, MCV2, and Pol3. Countries were stratifi…

The association of early postnatal weight loss with outcome in extremely low birth weight infants.

BACKGROUND: To compare outcomes of extremely low birth weight (ELBW) infants having different weight losses in the first 3 days of life. METHODS: One hundred and twenty six ELBW infants were evaluated retrospectively for weight loss percentages on the third day of life compared to their birth weight. We examined the weight loss on the third day of life compared to the birth weight for the ELBW infants and tested its association with mortality and morbidities. The mortality was subgrouped as overall mortality and mortality in the first 7 days of life. The morbidities were patent ductus arterio

Artificial intelligence-powered models in predicting mortality in maternal, newborn, and children under five: a systematic review protocol.

BACKGROUND: Maternal, newborn, and under-five mortality remain significant global health challenges, with 287,000 maternal deaths reported in 2020 and 4.9 million under-five deaths in 2022. The use of artificial intelligence in prediction models has shown great potential in predicting these mortalities. Artificial intelligence (AI)-powered models are capable of processing complex datasets to identify non-linear relationships and patterns, potentially outperforming traditional statistical methods in predictive accuracy, thereby enabling early detections and interventions more specifically. As A

Pulmonary interstitial emphysema in infants less than 1000 g at birth.

Eighty (32%) of 249 infants weighing less than 1000 g at birth developed pulmonary interstitial emphysema (PIE); its incidence was 42% in infants at 500-799 g, 29% at 800-899 g and 20% at 900-999 g. Fifty-five per cent occurred in the first 24 h and 43% were associated with other forms of air leak. The incidence and mortality of PIE did not improve between 1977 and 1984. The mortality rates in infants with or without PIE were not significantly different in the first 4 years of the study period (53% vs 45%). As the mortality improved in infants without PIE during the second 4 years, the differe

Medical care and preterm infants of normal birth weight.

Preterm infants of normal birth weight (born before 37 completed weeks of gestation and weighing more than 2,250 g) experience a neonatal mortality risk almost four times higher than do term infants in the same weight range. In an analysis of the effect of hospital level of birth on neonatal mortality, such preterm normal weight infants were found to experience higher mortality if born outside of a Level 3 (tertiary care) center. For all singleton infants in this weight-gestation category born in New York City maternity services during a 3-year period (N = 23,257), the relative mortality risk

Improving program targeting to combat early-life mortality by identifying high-risk births: an application to India.

BACKGROUND: It is widely recognized that there are multiple risk factors for early-life mortality. In practice most interventions to curb early-life mortality target births based on a single risk factor, such as poverty. However, most premature deaths are not from the targeted group. Thus interventions target many births that are at not at high risk and miss many births at high risk. METHODS: Using data from the second wave of Demographic and Health Surveys from India and a hierarchical Bayesian model, we estimate infant mortality risk for 73.320 infants in India as a function of 4 risk facto…

Effects of environmental enrichment and stereotypic behavior on maternal behavior and infant viability in a model carnivore, the American mink (Neovison vison).

In several species, stress compromises maternal behaviors that are important for infant viability (e.g. licking and grooming). Understanding how stress in captivity affects maternal behavior could therefore be beneficial, especially for carnivores in zoos and breeding centers where infant mortality is often high. We used a model carnivore--American mink--to test two hypotheses, namely that maternal investment and/or behavior is i. improved by environmental enrichment; and ii. compromised by stereotypic behavior. We observed 22 females raised in an indoor facility, 9 enriched, 13 non-enriched. …

Early postnatal assessment of the newborn in the developing country: Malawi.

The high rate of infant mortality, defined as the number of deaths in the first year of life for every 1000 babies born alive (WHO, 1981), is recognized as an extremely serious problem in the developing world. Worldwide decreases in the number of infant deaths are occurring; however, the gap continues to increase in countries with high socioeconomic baselines and those with low per capita income. This article discusses ways in which relevant health care policies and the use of newborn assessment tools can decrease infant mortality in Malawi Central Africa. The description of the tool, Newborn …
