This study aimed to define the number and type of complications associated with the Wada test at an academic medical center for comparison to previous reports. PURPOSE: The intracarotid amobarbital procedure (IAP) is widely used to help predict who might be at risk for postoperative amnesia after unilateral temporal lobectomy for intractable seizures. Noninvasive MR acquisition of blood flow and stationary tissue provides three-dimensional display of flow and of the vascular and brain surfaces. The calculated motion of a simulated bolus injection is derived from the measured velocity vector field and is animated to resemble cine angiography. Simulation of a bolus injection into the basilar artery of a healthy volunteer shows the blood flow into the posterior cerebral arteries.

We describe two patients who underwent intracarotid amobarbital procedure (IAP) postoperatively following temporal lobectomy (one right temporal and one left temporal lobectomy), prompted by consideration of reoperation for persistent, intractable seizures. INTRODUCTION: The use of plasminogen activators (PAs) in acute stroke patients has dual effects. PAs are beneficial by dissolving thrombi occluding an artery, but may be harmful by their proteolytic and excitotoxic actions.
METHODS AND RESULTS: We report two patients with a fatal brain herniation who rapidly deteriorated despite the successful recanalization and the sustained arterial patency or blood flow. Imaging studies taken after the thrombolytic treatment suggested early occurrence of cellular damage and profound breakdown of the blood-brain barrier.
