Pregnancy Outcomes of Women Admitted to a Tertiary Care Centre with Short Cervix.

OBJECTIVE: Our aim was to characterize the management and outcome of women admitted to a tertiary high-risk obstetrics unit with a short cervix (≤25 mm), measured on transvaginal ultrasound (TVS), and to determine the latency period from diagnosis to delivery. METHODS: A retrospective chart review of women admitted to the High Risk Obstetrics Unit at Sunnybrook Health Sciences Centre between 2005-2011 with an ultrasound-identified short cervix was done. RESULTS: A total of 110 women admitted for a short cervix between 2005-2011 (N = 56 singletons; N = 54 twin gestations) met the inclusion cr

Contribution of the second stage of labour to pelvic floor dysfunction: a prospective cohort comparison of nulliparous women.

OBJECTIVE: Maternal expulsive efforts are thought to damage the pelvic floor. We aimed to compare pelvic floor function and anatomy between women who delivered vaginally (VB) versus those with caesarean delivery (CD) prior to the second stage of labour. DESIGN: Prospective cohort. SETTING: University Hospital Midwifery practice. POPULATION: Nulliparas. METHODS: Pregnant nulliparas were recruited during pregnancy and women who underwent CD prior to the 2nd stage of labour at birth were recruited immediately postpartum. Both groups were followed prospectively to 6 months postpartum. MAIN OU

A Clinical Risk Model to Predict Obstetric Anal Sphincter Injuries in Laboring Patients.

OBJECTIVE: Our objective was to develop a risk stratification tool for predicting obstetric anal sphincter injury risk in women during labor. METHODS: In this retrospective cohort study of singleton deliveries within Kaiser Permanente Northern California, a predictive model for anal sphincter injury was developed within the 2013 birth cohort and validated in the 2014 birth cohort of 22,741 births. Predictors of obstetric anal sphincter injury were identified using multiple regression analysis and used to create a risk calculator tool based on effect size and clinical judgment. RESULTS: Durat

The role of episiotomy in prevention of genital lacerations during vaginal deliveries--results from two European centers.

OBJECTIVES: There is an ongoing debate regarding the routine versus restrictive use of episiotomy The study aim was to investigate if episiotomy during vaginal deliveries can reduce both, the number and severity of genital lacerations. MATERIAL AND METHODS: The study included all women who gave vaginal birth at AOU. "G. Martino" Messina (n=382) and the Clinic for Ob/Gyn Clinical Center of Serbia, Belgrade (n=4227) during 2011. Lacerations during birth were recorded and divided according to location and severity Women with lacerations were subdivided into two groups: with or without mediolater

[Delivery during time of shift change is not a risk factor for obstetric complication: a historical cohort study].

BACKGROUND: The time of shift change is a unique time because the continuity of routine care is interrupted. The association between delivery during time of shift change and obstetric complications has not been evaluated. OBJECTIVE: We hypothesized that delivery during time of shift change is at risk for obstetric complications. METHODS: A historical cohort study was performed of all women with a singleton pregnancy undergoing a trial of labor at term during 2006-2010. Data was extracted from a computerized database that is continuously updated during Labor. The hour of delivery was divided

The significance of peripartum fever in women undergoing vaginal deliveries.

We investigated whether patients undergoing vaginal delivery who developed peripartum fever (PPF) had increased rates of other gestational complications. A retrospective study was undertaken comparing pregnancy complications of patients who developed PPF with those who did not. A multivariable logistic regression model was constructed to control for confounders. To avoid ascertainment bias, the year of birth was included in the model. Women who underwent cesarean delivery and those with multiple pregnancies were excluded from the study. During the study period, there were 169,738 singleton vag

Is fetal heart rate variability a good predictor of fetal outcome?

OBJECTIVE: The purpose of this study was to investigate, in a prospective manner, whether fetal heart rate (FHR) variability serves as a reliable single predictor of fetal outcome. METHODS: We undertook a prospective study of 2,200 consecutive deliveries. Preterms < 37 weeks, twins, stillbirths and fetal malformations were excluded from the study. FHR tracings were analyzed for variability in every delivery. Three windows were evaluated: 1) early in labor (30 min); 2) in the active phase of labor - 1 h prior to complete cervical dilatation (30 min); and 3) throughout the second stage of deliv

Quality of Labor Analgesia with Dural Puncture Epidural versus Standard Epidural Technique in Obese Parturients: A Double-blind Randomized Controlled Study.

BACKGROUND: The dural puncture epidural technique may improve analgesia quality by confirming midline placement and increasing intrathecal translocation of epidural medications. This would be advantageous in obese parturients with increased risk of block failure. This study hypothesizes that quality of labor analgesia will be improved with dural puncture epidural compared to standard epidural technique in obese parturients. METHODS: Term parturients with body mass index greater than or equal to 35 kg · m-2, cervical dilation of 2 to 7 cm, and pain score of greater than 4 (where 0 indicates no

Intrathecal injection of morphine for obstetric analgesia.

Intrathecal injection of morphine was used to provide obstetric analgesia in 20 primiparous women in labor. When the cervix was at least 3 cm dilated, morphine, 1 or 2 mg, was injected intrathecally. In all parturients, labor pains were completely relieved after 15-60 min and analgesia lasted as long as eight to 11 hours. The analgesia was not associated with any alteration of pin-prick sensation or motor power, and there was no change in the arterial blood pressure or heart rate. All infants were delivered vaginally by use of episiotomy annd a low forceps, except two infants of mothers in the

Features of shoulder dystocia in a busy obstetric unit.

OBJECTIVE: To assess the incidence and complications of shoulder dystocia and whether those complications could be avoided. STUDY DESIGN: Retrospective analysis of shoulder dystocia between 1996 and 1999 to determine whether macrosomia, diabetes, height of head at full dilatation, length of second stage or instrumental delivery could predict shoulder dystocia. Fetal asphyxia, brachial plexus injury, maneuvers used to free the shoulders and experience of the attendant were also assessed. RESULTS: There were 56 cases of shoulder dystocia in 24,000 births, 59% after spontaneous delivery. McRobe

Point-of-care ultrasound for assessing fetal head descent during labor.

OBJECTIVES: Ultrasound assessment of fetal head station is more accurate than digital examination, but its use is limited by the lack of equipment dedicated to the labor ward. Point-of-care ultrasound (POCUS) may overcome this barrier. We assessed the agreement between a handheld POCUS device and a conventional high-end system in measuring fetal head descent. METHODS: Seventy singleton pregnancies were examined in the second stage of labor. Two experienced operators, blinded to each other's results, performed translabial scans in randomized sequence, one with a handheld wireless device and on

Postpartum febrile morbidity following vacuum-assisted birth: A retrospective cohort study.

OBJECTIVE: To evaluate whether vacuum-assisted birth (VAB) independently increases the risk of postpartum febrile morbidity, identifying a potentially modifiable contributor to maternal infectious morbidity. METHODS: This retrospective cohort study was conducted at a university-affiliated tertiary maternity center between 2011 and 2023. We included women with singleton pregnancies ≥37 weeks of gestation who underwent vaginal birth. Women who underwent VAB formed the study group, while those experiencing spontaneous vaginal birth (SVB) served as the comparison group. To focus on new-onset post

Maternal and Neonatal Complications Resulting From Vacuum-Assisted and Normal Vaginal Deliveries.

Background Operative vaginal delivery is a procedure that is performed using forceps or vacuum to extract an infant from the birth canal. It has many indications, one of which is prolonged second stage of labor. Although rare, vacuum extraction (VE) can lead to various neonatal and maternal complications. The objective of this study was to compare the rates of different neonatal and maternal complications between vacuum-assisted deliveries and spontaneous vaginal deliveries. Methods This is a retrospective cohort study that was conducted in King Abdulaziz Medical City, Jeddah (KAMC-J), Saudi A
