BACKGROUND: In twin-twin transfusion syndrome (TTTS), the donor and recipient fetus are exposed to differing volume loads and show discordant intertwin vascular compliance in childhood despite identical genotype. We hypothesized that discordance is prevented by intrauterine endoscopic laser ablation of placental anastomoses, which abolishes intertwin transfusion. We tested this by examining pulse wave velocity (PWV) in brachial arteries of twin survivors of TTTS treated with and without laser therapy.

OBJECTIVE: To determine effects of cyclophotocoagulation via administration of 100 J with a neodymium:yttrium aluminum garnet (Nd:YAG) laser on corneal touch threshold (CTT), intraocular pressure (IOP), aqueous tear production, and corneal nerve morphology in eyes of dogs.
ANIMALS: 15 dogs.
PROCEDURE: Noncontact Nd:YAG laser was transsclerally applied (10 applications; 25 W for 0.1 seconds for each application to each of 4 quadrants) to the ciliary body of the left eye of 15 dogs; the right eye was the control eye. Pseudophakic patients that require surgical management of glaucoma have many of the same options as phakic patients. Considerations when performing incisional glaucoma surgery (trabeculectomy with adjunctive antimetabolites and glaucoma drainage device implantation) and laser glaucoma surgery (laser trabeculoplasty and laser cyclophotocoagulation) in pseudophakic patients are reviewed here. The decision to operate and the choice of surgical procedure must be individualized for each patient.

Syringomas are benign lesions most commonly appearing around the eyelids and in malar areas, but they have also been described on other body sites. Depending on the site they may cause either aesthetic disturbance (e.g. face, neck, arms) or pruritus (e.g. vulva). This is the first report of argon laser treatment of syringoma in a patient with multiple vulvar syringomas in association with pruritus vulvae and carcinophobia.

Eight infants with stage 3+ retinopathy of prematurity (ROP) were treated using the diode laser through a binocular indirect ophthalmoscope. Two infants with zone I disease were treated prethreshold as soon as the signs of plus disease developed. Successful regression was achieved in all cases, though 3 infants required repeat treatment. All infants were followed up for a minimum of 3 months. Keeler acuity scores were in the normal range in 4 of the 8 infants.

BACKGROUND: Corneal neovascularization is a common clinical entity. Although visual acuity is usually impaired and corneal function compromised, there has been only limited success in the clinical management of this condition. Laser photocoagulation and intubation are effective measures for palliation of the unresectable esophago-cardial malignancy. Laser therapy is especially useful for short strictures and noncircular and proximal lesions. The energy levels needed for cyclophotocoagulation using either transscleral contact krypton or cw-YAG lasers were compared in pigmented rabbits. The same transscleral laser probe was used for both lasers. With the krypton laser, 0.25 J of energy caused macroscopically detectable lesions, and 1.5 J led to widespread destruction of the ciliary body. Diode laser treatment for retinopathy of prematurity was successful in 81% of 21 eyes with 'threshold' (zone 2, stage 3+) disease. This compares favourably with cryotherapy and argon laser photocoagulation. The retinal outcome and technique are discussed.
