Pulmonary metastasis from uterine leiomyosarcoma in a patient with limited cutaneous systemic scleroderma.

A 51-year-old woman, who was diagnosed as having limited cutaneous systemic scleroderma, presented with pulmonary nodules incidentally detected in a chest radiograph. The patient had surgical biopsy of the nodules. In microscopic examination of the specimens, proliferation, mitotic activity, and cellular anaplasia of spindle cells were present. Fluorodeoxyglucose-positron emission tomography showed tumors in lungs as well as uterus. The diagnosis of the tumor was pulmonary metastases from uterine leiomyosarcoma. We should be on alert the possibility of developing malignant disease in patient w…

Synchronous uterine adenocarcinoma and leiomyosarcoma: A rare case report causing a clinical conundrum.

INTRODUCTION: Synchronous gynecologic primary cancers are uncommon. When present, the most frequent malignancies consist of endometrial and ovarian carcinomas. Here we report an exceedingly rare case of concurrent uterine adenocarcinoma and leiomyosarcoma. CASE PRESENTATION: A 60 year-old female presented with four years of postmenopausal bleeding. An endometrial sampling showed grade 2 endometrioid adenocarcinoma. She proceeded with hysterectomy that contained an anterior endometrial mass and a posterior myometrial mass. The final pathology demonstrated concurrent uterine adenocarcinoma and …

MED12 and HMGA2 mutations: two independent genetic events in uterine leiomyoma and leiomyosarcoma.

Recent identification of somatic MED12 mutations in most uterine leiomyomas brings a new venue for the study of the tumorigenesis of leiomyomas. We are particularly interested in the correlation of MED12 and HMGA2 gene products in leiomyomas and leiomyosarcomas with and without MED12 mutations. To address these issues, in this study we examined MED12 mutations in a large cohort of usual type leiomyomas (178 cases) and uterine leiomyosarcomas (32 cases). We found that 74.7% (133/178) of leiomyomas had MED12 mutations, which was consistent with several independent studies. In contrast, only 9.7%…

Concomitant aortic leiomyosarcoma and takayasu arteritis in a 55-year-old male patient.

Takayasu arteritis is a form of large vessel granulomatous vasculitis affecting often young or middle-aged women, especially of Asian descent. It mainly affects the aorta and its branches. Primary malignancies, such as leiomyosarcoma of the aorta are extremely rare. This report discusses the exceptional and concomitant association of Takayasu arteritis and aortic leiomyosarcoma in a 55-year-old male. The patient suffered from systemic signs and symptoms related to arteritis together with claudication of left upper limb due to left artery subclavian stenosis. After instrumental evaluation, an i…

Radiation-induced leiomyosarcoma of the maxillofacial region: facial reconstruction with implant-retained prosthesis.

Radiation-induced sarcoma represents an exceptional rarity in the head and neck region. The treatment is surgical excision with negative margins and follow-up. After surgery, reconstruction of the defects can be accomplished either surgically or prosthetically. Implant-retained maxillofacial prostheses may offer an acceptable solution. In this report, a large facial defect resulting from radiation-induced leiomyosarcoma was reconstructed with implant-retained prosthesis. A patient with a large facial defect resulting from tumor surgery presented for reconstructive treatment. The primary tumor …

Asymptomatic right atrial leiomyosarcoma with tricuspid valve obstruction in a young female patient.

Primary cardiac leiomyosarcomas of the heart are rare tumors usually diagnosed post-mortem. Like other sarcomas located in the heart, it causes symptoms by obstruction or occlusion of cardiac cavities, local invasion, embolization or by systemic manifestations. We present an unusual case of a previously healthy young female patient who was accidentally diagnosed with a cardiac tumor of unknown origin during routine physical examination in May 1997. Until a few days prior to elective cardiac surgery for diagnostic purposes in June 1997, no clinical symptoms were present. To our surprise, a prim…

Neuron-specific enolase-producing epithelioid leiomyosarcoma with gross vascular invasion and intracardiac involvement.

We present an unusual case of epithelioid leiomyosarcoma in the right shoulder. A 10-cm tumor eroding the right scapula occurred in a 59-year-old man and grew into the subclavian vein with extension to the right atrium. An elevated serum level of neuron-specific enolase was also detected. Clinical presentation suggested small cell carcinoma of the lung (Pancoast tumor) with intravenous extension. Eventually, the patient died of respiratory insufficiency. Autopsy revealed a primary tumor in the right shoulder, numerous tumor emboli in the pulmonary arteries, and associated hemorrhagic infarctio…

Leiomyosarcoma of the inferior vena cava causing Budd-Chiari syndrome--a case report.

A forty-eight-year-old Japanese woman with leiomyosarcoma of the inferior vena cava presenting as Budd-Chiari syndrome is reported. A large tumor originating from the venous wall grew into the lumen, obstructing the hepatic vein, and extended up to the right atrium. Although this is a very rare tumor, care should be taken in order to make an early diagnosis and manage the condition effectively.

Postoperative radiotherapy-induced pulmonary leiomyosarcoma in breast cancer: a case report and literature review.

Radiation-induced sarcoma (RIS) is a rare and severe complication of radiotherapy following breast cancer surgery. It is characterized by a long latency period, high malignancy, limited clinical experience in diagnosis and treatment, and substantial individual variation in prognosis. This study reports a case of radiation-related pulmonary leiomyosarcoma that developed more than 20 years after radiotherapy for breast cancer. The tumor harbored a TP53 mutation and showed low PD-L1 expression. Imaging documented rapid tumor progression over a short interval, which rendered radical resection impo…

Rare Vascular Sarcoma Treated With Ex Vivo Liver Surgery: A Case of IVC Leiomyosarcoma.

We report a case of leiomyosarcoma of the inferior vena cava (IVC) treated with liver resection, autotransplantation, and IVC reconstruction using a Dacron graft. A 47-year-old female presented with mild to moderate lower extremity edema and weakness. Physical examination revealed no abdominal or pelvic abnormalities. However, further evaluation confirmed a tumor in the suprarenal IVC, extending to the hepatic veins, with thrombus formation in the suprahepatic IVC. During surgery, the tumor was confirmed to originate from the IVC. Given its proximity to the hepatic veins, an ex vivo liver rese…

Epigenetic Regulation of Uterine Smooth Muscle Tumors: Histone Modifications in Uterine Fibroids and Leiomyosarcoma.

Uterine smooth muscle tumors (USMTs) represent a diverse group of neoplasms arising from the myometrium, ranging from benign uterine fibroids (leiomyomas) to highly aggressive uterine leiomyosarcoma. While genetic alterations contribute to tumor development, growing evidence highlights the crucial role of epigenetic regulation in shaping tumor behavior. Among these mechanisms, histone modification has emerged as a key regulator of chromatin structure and gene expression. Histone modifications, including acetylation, methylation, phosphorylation, ubiquitination, ADP-ribosylation, and SUMOylatio…

Contrast-Enhanced Ultrasound Findings in Primary Hepatic Leiomyosarcoma: A Case Misdiagnosed as Liver Abscess.

Primary hepatic leiomyosarcoma (PHLS) is a rare malignant tumor of mesenchymal origin in the liver, with nonspecific clinical manifestations. High fever as the initial symptom is uncommon, making it easily misdiagnosed as a liver abscess. In this case, the tumor was initially mistaken for a liver abscess. Contrast-enhanced ultrasound (CEUS) corrected the diagnosis and helped avoid inappropriate treatment.

Renal Leiomyosarcoma With Regional Lymph Node Metastasis and Biopsy Tract Seeding: A Case Report.

Renal leiomyosarcoma is a rare and aggressive tumor that comprises a very small proportion of adult kidney malignancies. Its clinical and imaging characteristics can closely resemble those of other high-grade renal tumors, which complicates the diagnostic process. We report the case of a 66-year-old male patient who presented with a large left renal mass and was diagnosed with high-grade pleomorphic leiomyosarcoma. He underwent radical nephrectomy along with en bloc removal of a para-aortic lymph node, which was histologically confirmed to contain metastatic disease. In an unusual postoperativ…

Genomic characterization and histologic analysis of uterine leiomyosarcoma arising from leiomyoma with bizarre nuclei.

Leiomyoma with bizarre nuclei (LM-BN) is a rare variant of leiomyoma with a benign clinical course. In contrast, leiomyosarcoma (LMS) is a high-grade, malignant neoplasm characterized by high recurrence rates and poor survival. While LM-BN and LMS show distinct morphologies, they share similar immunoprofiles and molecular alterations, with both considered 'genomically unstable'. Rare cases of LM-BN associated with LMS have been reported; however, the histogenesis and molecular relationship between these two tumors remains unclear. In this study, we assessed 11 cases of LMS arising in conjuncti…

Conjunctival leiomyosarcoma in a patient with xeroderma pigmentosum: 5-year follow-up without recurrence.

Conjunctival leiomyosarcoma is a very rare soft tissue malignancy. Herein, we describe a conjunctival leiomyosarcoma case in a patient with another rare disease, xeroderma pigmentosum. The 27-year-old single-eyed xeroderma pigmentosum patient complained of exophytic mass covering the ocular surface in her left eye. A vascular, hemorrhagic mass covering the entire ocular surface of the left eye was identified on the examination. Thus, total mass excision surgery was performed. The pathological diagnosis was compatible with conjunctival leiomyosarcoma. Additional chemotherapy, radiotherapy, or s…

A rare localization of a common disease: Primary uterine Non-Hodgkin lymphoma mimicking leiomyosarcoma.

Genitourinary system lymphomas comprise a small part of extra-nodal lymphomas (ENLs). ENLs of uterine origin are extremely rare and are often confused with gynecological malignancies. We present an 80-year-old female patient diagnosed with diffuse large B-cell lymphoma (DLBCL) with a single focus of the uterus. The patient's only complaint was abnormal uterine bleeding. Magnetic resonance imaging revealed an intramural-subserous-submucous multiple mass lesion with minimal contrast enhancement mimicking leiomyosarcoma. Diffuse pathological 18F-fluorodeoxyglucose uptake was detected in the entir…
