South Africa: a legacy of family disruption.

This article draws together unusual characteristics of the legacy of apartheid in South Africa: the state-orchestrated destruction of family life, high rates of unemployment and a high prevalence of HIV/AIDS. The disruption of family life has resulted in a situation in which many women have to fulfil the role of both breadwinner and care giver in a context of high unemployment and very limited economic opportunities. The question that follows is: given this crisis of care, to what extent can or will social protection and employment-related social policies provide the support women and children

Elective ventilation for organ donation: law, policy and public ethics.

This paper examines questions concerning elective ventilation, contextualised within English law and policy. It presents the general debate with reference both to the Exeter Protocol on elective ventilation, and the considerable developments in legal principle since the time that that protocol was declared to be unlawful. I distinguish different aspects of what might be labelled elective ventilation policies under the following four headings: 'basic elective ventilation'; 'epistemically complex elective ventilation'; 'practically complex elective ventilation'; and 'epistemically and practicall

Specialist palliative and end-of-life care for patients with cancer and SARS-CoV-2 infection: a European perspective.

BACKGROUND: Specialist palliative care team (SPCT) involvement has been shown to improve symptom control and end-of-life care for patients with cancer, but little is known as to how these have been impacted by the COVID-19 pandemic. Here, we report SPCT involvement during the first wave of the pandemic and compare outcomes for patients with cancer who received and did not receive SPCT input from multiple European cancer centres. METHODS: From the OnCovid repository (N = 1318), we analysed cancer patients aged ⩾18 diagnosed with COVID-19 between 26 February and 22 June 2020 who had complete sp

Do Symptoms among Home Palliative Care Patients with Advanced Cancer Decide the Place of Death? Focusing on the Presence or Absence of Symptoms during Home Care.

OBJECTIVE: Our goal was to investigate the association between actual place of death among Japanese home end-of-life care patients with advanced cancer and individual, illness, and environmental factors, including the presence or absence of symptoms. METHODS: Using structured interviews, we asked 44 nurses from 19 home-visit nursing agencies about changes in status of all 123 of their home end-of-life care patients with advanced cancer between April and September 2013. The main outcome variable was actual place of death (Home/Hospital/Still surviving) and independent variables were the patien

Patients' and caregivers' experiences of familial and social support in resource-poor settings: A systematically constructed review and meta-synthesis.

BACKGROUND: Familial and social support for patients with life-limiting conditions is crucial, especially in resource-poor settings. However, limited knowledge exists about patients' and caregivers' experiences within these informal networks in such contexts. AIMS: This systematic review aimed to (i) synthesise the experiences of patients and caregivers regarding familial and social support in resource-poor settings, and (ii) understand the challenges they face in order to provide evidence for more compassionate, culturally congruent palliative care. DESIGN: Systematic review and meta-synthe

Neonatal extra-corporeal life support: indications and limitations.

Cardiac extra-corporeal life support is used more frequently in the current era of complex, high-risk neonatal heart surgery. Although outcome for neonates with complex heart disease has improved in the last decade, thanks to advances in surgery and intensive care, survival in the subset that require extra-corporeal support remains unchanged at below 40%. Neonatal cardiac extra-corporeal support is a technically challenging therapy that is applied in a range of contexts including: post-operative low cardiac output syndrome, cardiac arrest, high-risk interventional catheterisation or as a bridg

Integrating Paramedics into End-of-Life Care Increases Cancer Patients' Time Spent at Home instead of the Emergency Room: A Retrospective Cohort Study.

BACKGROUND: To enable patients with advanced cancer to spend their final days at home, palliative care support must be available at home around the clock. OBJECTIVES: To evaluate how the integration of paramedics into end-of-life care for cancer patients affects the utilization of the emergency department, the time spent at home, and places of death. DESIGN: Retrospective cohort study. SETTING/SUBJECTS: Patients who died of cancer in 2016-2017 in North Karelia, Finland. METHODS: Emergency department visits and time spent at home among patients with or without enrollment in the paramedics'

Palliative care or supportive care?

Palliative care is the active holistic (physical, psychological, social and spiritual) care of people who experience health-related suffering due to severe illness and those close to them, aiming to improve their quality of life. There are three levels of palliative care: palliative care approach, generalist palliative care, and specialist palliative care. Traditionally associated with end-of-life care, this article highlights the evolution of specialist palliative care, now deemed to be applicable throughout the course of an illness. Terminologies surrounding 'palliative care' are also consid

Experiences of Adults With Type 1 Diabetes Using Digital Health Technology for Diabetes Self-Care: Qualitative Study.

BACKGROUND: Type 1 diabetes is a constraining disease due to the burden of its management, and diabetes outcome largely depends on the effectiveness of diabetes self-care. Digital health technology (DHT), which includes continuous glucose monitoring, insulin delivery devices, and related mobile health apps, can support diabetes self-care and thereby improve diabetes outcomes. In literature, experiences with the use of DHT vary widely among people with diabetes and are a less studied area among adults with type 1 diabetes. OBJECTIVE: The study aimed to explore experiences of using DHT for diab

Understanding the effect of miliary tuberculosis through the experience of one young person.

In the UK, tuberculosis (TB) is on the increase. Miliary TB affects the lungs and can leave lasting damage, and affect a person's quality of life adversely. Healthcare professionals need to co-ordinate services efficiently, to ensure a smooth transition from hospital to home, and ensure there is appropriate support in the community for as long as necessary. In this article, a boy describes his journey through the illness: the associated bewilderment, deterioration, pain, fear and isolation; the emergencies; the prolonged but effective treatment; the care he received; and the support of his fam

A Reflexivity Lab as a methodological tool in end-of-life research.

BACKGROUND: Reflexivity is essential in end-of-life research due to its ethical sensitivity, emotional intensity, and relational complexity, yet it is often left to individual initiative and remains weakly institutionalized in palliative care research settings. OBJECTIVE: To describe the design, implementation, and initial outputs of a Reflexivity Lab developed as a structured, collective reflexive method embedded within a palliative care research service. METHODS: A Reflexivity Lab was implemented in 2023 within the palliative and supportive care service of the Lausanne University Hospital,

From PDTA to PIDTA: a new integrated care pathway to improve survival and quality of life in breast cancer patients.

The Percorso Diagnostico-Terapeutico Assistenziale (Diagnostic, Therapeutic and Care Pathway, PDTA), has long provided a structured framework for cancer care, supporting continuity and adherence to clinical standards. However, as improvements in cancer outcomes are accompanied by increasing complexity in patient needs, particularly in breast cancer, greater attention is being directed towards models of care that extend beyond survival to include quality-of-life outcomes and long-term survivorship needs. Within this context, interest is emerging in the PIDTA (Percorso Integrato Diagnostico-Tera

Understanding and addressing factors affecting carers' mental health during end-of-life caregiving: synopsis of meta synthesis of literature and stakeholder collaboration.

BACKGROUND: Family carers provide crucial support to patients nearing end of life. This can affect carers' own mental health negatively. It is important to understand what factors may affect carers' mental health and convey this information to stakeholders who can effect change. We conducted reviews of the qualitative, observational and intervention carer literature and worked with carer advisors and other stakeholders to make findings useful and accessible. AIM OF THE SYNOPSIS: To provide a synopsis of (1) project methods, (2) findings and implications from the evidence syntheses, (3) outcom

Family carers' experiences of serious illness conversations in haematological malignancies - a qualitative interview study.

BACKGROUND: Family carers of cancer patients commonly experience anxiety, as well as grief. To better manage their situation, they seek to understand the disease process and become involved in decision-making. Early integration of palliative care and serious illness conversations have been shown to improve quality of life, reduce symptom burden, and support both patients and their families. In haematological malignancies, patients often experience high disease burden, poor quality of life, and aggressive treatment near the end of life, with end-of-life discussions frequently occurring too late
