Extracapsular cataract extraction and posterior-lip sclerectomy with viscoelastic.

A review of 15 cases suggests that posterior-lip sclerectomy can be performed safely with extracapsular cataract extraction (ECCE) and posterior chamber lens implantation. The anterior chamber was filled with viscoelastic at the end of each procedure; no case required reoperation for shallow chamber or hypotony. The mean intraocular pressure after 1 year was 12.1 mm Hg. The astigmatism induced by the triple procedure did not differ significantly from that caused by ECCE alone during the initial 2 postoperative years. A new mathematical model that describes the change over time of postoperative…

Neonatal cleft lip repair: the anesthesiologist's point of view.

Neonatal cleft lip repair is no longer an unusual procedure and several series have been published to date. We present our 3-year experience from the anesthesiologist's point of view. Preoperative evaluation, perioperative management, and postoperative problems are described. A thorough preoperative evaluation in order to exclude associated malformations (especially cardiac) and experience in neonatal anesthesia are essential for the sake of safety.

Incontinentia pigmenti associated with bilateral cleft lip and palate.

Incontinentia pigmenti (Bloch-Sulzberger syndrome) is an uncommon genodermatosis mainly affecting females. It consists of characteristic skin manifestations associated in 79.8% of patients with other systemic manifestations. Of these there have been five cases of palatal or lip abnormalities, none being bilateral cleft lip and palate. We describe a unique case of incontinentia pigmenti associated with a bilateral cleft lip and palate.

Morsicatio buccarum et labiorum (excessive cheek and lip biting).

In some individuals habitual cheek and lip biting becomes a fixed neurosis. Pieces of oral mucosa are actually torn free from the surface, producing a distinctive clinical appearance termed morsicatio buccarum et labiorum. Sometimes it may be confused with other dermatological disorders involving the oral mucosa, and can lead to misdiagnosis. Most patients with this condition are unaware of their habit and will not aid in the diagnosis. We suggest that the histopathological features of this condition are distinctive, if not pathognomonic, and that an accurate diagnosis should be rendered if a …

Towards better nutrition: lip service or a realistic fight?

Progress towards the objective of the World Food Conference of 1974 that "no child should go to bed hungry" is reviewed. The low market price of primary products keeps developing countries poor. Yet in these countries industry rather than agriculture has been supported by governments. All regions are increasing total food production but population growth threatens to offset this increase. In some areas there is decrease in food production per head of population. In many countries the social situation of women affects the nutrition of families. Other causes of malnutrition are discussed and fut

Repair of unilateral cleft lip: the rotation-advancement operation.

The most obvious deformity in a unilateral cleft of the lip is asymmetry of the lip and nose. Operation must repair the cleft, lengthen the lip, restore muscle continuity, and create an adequate labial sulcus. Simultaneous correction of the nasal deformity should be carried out to the greatest extent possible at the same time. The rotation-advancement repair has advantages over other repairs in scar placement, correction of the nasal deformity, and conservation of lip and nose tissue. If revision of the lip or nose is required, it can be accomplished more easily following the rotation-advancem…

Principles and techniques of bilateral complete cleft lip repair.

Important principles for repair of bilateral complete cleft lip are symmetry, primary orbicularis continuity, proper prolabial size and shape, median tubercle and mucocutaneous ridge formation from lateral lip tissue, and early construction of nasal tip and columella with anatomic placement of the alar cartilages. A two-stage repair employing techniques based on these concepts is described. At the initial procedure, the lateral crura are positioned and a tiny biconcave prolabium is shaped in anticipation of the changes with growth. The second stage (nasal correction) includes apposition of the…

Progressive addition lenses for deaf lip readers and signers.

Deaf patients depend on intermediate vision for interpersonal communication. Deaf presbyopes are at a particular disadvantage since they are usually corrected for near and far vision with bifocals without consideration of their intermediate vision. Progressive addition lenses were used in this study to provide an intermediate correction for a group of deaf presbyopes. The patients' communication ability was tested through Varilux II lenses and habitual bifocals on a series of standardized lip reading and sign language tests. The patients showed better visual communication skills with the progr…

Necrotizing sialometaplasia of the lip simulating squamous cell carcinoma.

A case of necrotizing sialometaplasia of the lip in an 68-year-old pipe smoker is described. Necrotizing sialometaplasia is a self-healing non-neoplastic disease probably of ischaemic nature. Thirty-nine cases of sialometaplasia are described in the literature up to early 1979. These cases appeared in the palate, nasal cavity, gingiva, lip, hypopharynx and maxillary sinus. Six cases have also been reported from major salivary glands. Histologically there is necrosis of mucous cells with partial replacement by squamous epithelium. This entity has often been mistaken for squamous or mucoepidermo…

Factors determining occurrence of cleft lip and cleft palate.

The modern care of newborn infants with oral clefts renders their survival possible. Since they will eventually reproduce, a slight increase in the incidence may be expected and genetic counseling will be requested on occasion. The cleft lip with or without cleft palate has a different genetic inclination from isolated cleft palate and the risk of recurrence is different. Drugs, such as antiepileptics, salicylates, benzodiazepines and cortisone, have a role in causing oral clefts. When an oral cleft is a part of a syndrome, the genetics of the particular syndrome must be outlined and genetic c…

Secondary correction of nasal abnormalities associated with cleft lip.

Approximately 30% of patients will require a second operation for correction of nasal abnormalities associated with unilateral cleft lip. In a single second operation, the following steps seem to give the most satisfactory correction: repositioning the nasal septum in the midline; altering the contour of the alar cartilage on the cleft side; and rotation of the axis of the external naris on the cleft side. The usual result following this combination of procedures is moderate but not complete correction of the abnormality. About 50% of the patients will need a second operation for correction of…

Two-piece magnet-retained silicone lip prosthesis in a mandibulectomy patient with lower lip defect following oncologic resection: A clinical case report.

Lower lip defects after oncologic resection can severely affect oral competence, speech, esthetics, and psychosocial well-being. Surgical reconstruction alone may not fully restore dynamic perioral function, requiring adjunctive prosthetic rehabilitation. A 58-year-old male with right lower alveolar squamous cell carcinoma underwent mid-segmental mandibulectomy with immediate free fibula flap reconstruction and primary implant placement, followed by radiotherapy. One year later, he presented with partial lower lip deficiency, drooling of saliva, speech difficulty, and esthetic compromise. A tw…

Lip-split mandibulectomy versus lip-mandible preservation technique for oral and oropharyngeal cancer: a systematic review and meta-analysis of comparative studies.

BACKGROUND: Many studies have compared lip-splitting mandibulotomy (LSM) and lip-mandible preservation (LMP) techniques in oral and oropharyngeal cancer (OOPC) patients with inconsistent conclusions. Evidence-based recommendations for the optimal surgical approach for treating OOPC are lacking. METHODS: The Cochrane Library, Pubmed, Embase, Web of Science, WAN-FANG, CQVIP, and China National Knowledge Infrastructure were systematically searched to identify studies that compared LSM versus LMP for OOPC. An additional search of the gray literature was performed using Google Scholar, OpenGrey an…

Single-Stage Primary Palatal Closure and Bone Grafting: A Comparison Between Unilateral Cleft Lip and Alveolus and Unilateral Cleft Lip and Palate.

Background This study aimed to evaluate the outcomes of secondary alveolar bone grafting by assessing grafted bone resorption in the alveolar region and the anterior third of the hard palate in patients with unilateral cleft lip and palate (UCLP). A modified protocol involving simultaneous alveoloplasty and anterior palate closure was employed to optimize surgical outcomes and reduce the number of required procedures. Materials and methods A total of 55 patients were retrospectively evaluated: 16 with unilateral cleft lip and alveolus (UCLA) who underwent bone grafting limited to the alveolar …

Epidemiology of cleft lip and palate charity mission surgery at Bandung Cleft Lip and Palate Center, Indonesia: a 14-year institutional review.

BACKGROUND: The management of cleft lip and palate aims at improving the patient's aesthetic and functional outcomes. Delaying primary repair can disrupt the patient's functional status. Long-term follow-up is essential to evaluate the need for secondary repair or revision surgery. This article presents the epidemiology of cleft lip and palate, including comprehensive patient characteristics, the extent of delay, and secondary repair at our institutional center, the Bandung Cleft Lip and Palate Center, Faculty of Medicine, Universitas Padjadjaran, Bandung, Indonesia. METHODS: This retrospecti…

Seeing Cleft Lip from a New Angle: Crowdsourcing to Determine whether Scar Severity or Lip Angle Matters More to the General Public.

BACKGROUND: Modern cleft lip surgery aims to restore symmetry and create a level, normal-appearing Cupid's bow. However, families' concerns often center on the degree of scarring. The authors hypothesized that the lip angle would be less influential than scar severity in layperson ratings. METHODS: The authors received institutional review board approval for modifying patients' postoperative photographs to create systematic variations displaying different levels of scarring and lip angle. Each child's resulting composite images were presented in pairs to internet raters using Amazon Mechanica…
