Prevalence and dynamics of quinolone and macrolide resistance in hospital wastewater: insights from Kathmandu Valley, Nepal.

BACKGROUND: Hospital wastewater (HWW) serves as a critical reservoir for antibiotic-resistant bacteria (ARB) and antibiotic resistance genes (ARGs), especially those conferring resistance to quinolones, fluoroquinolones and macrolides. This study investigated the prevalence and seasonal dynamics of ARB and ARGs in HWW from the Kathmandu Valley, Nepal. METHODS: We collected and analysed 16 untreated HWW samples from eight hospitals during the summer and winter seasons (2022-2023). Physicochemical parameters, prevalence of ARB, selected ARGs (qnrS, aac(6')-Ib-cr, erm(B)) and class 1 integron (i…

Strevertene A, a Streptomyces-Derived Macrolide, Reveals Selective Activity against the Human Parasite Schistosoma mansoni.

Schistosomiasis, a neglected tropical disease caused by parasitic blood flukes, affects 250 million humans worldwide, leading to widespread chronic illness and eventually death. Praziquantel is the only widely used and broadly effective drug for schistosomiasis; however, reports of reduced efficacy raise concerns about future therapy. To identify alternative compounds with antischistosomal activity, almost 1000 bacterial and fungal extracts were screened against adult Schistosoma mansoni worm pairs. This effort revealed Streptomyces sp. ST501019 is a producer of strevertene A, a 28-membered pe

Microbiologic clearance in macrolide-resistant Bordetella pertussis: An infant treated with sulfamethoxazole-trimethoprim - A case timeline.

Amid a large-scale resurgence of pertussis in Japan in 2025, concern is growing about the spread of macrolide-resistant Bordetella pertussis (MRBP)-prevalent in China-posing a clinical challenge, particularly in infants. We describe a term 6-week-old unvaccinated infant with life-threatening MRBP. Diagnosis was confirmed by culture and multiplex PCR. The isolate harbored the 23S rRNA A2047G mutation. Daily microbiology (Gram stain, culture, quantitative PCR) was performed on nasopharyngeal swabs and lower-airway aspirates. The infant received empiric azithromycin and a 14-day course of sulfame

Does exposure timing of macrolide antibiotics affect the development of river periphyton? Insights into the structure and function.

Discharged sewage is the dominant source of urban river pollution. Macrolide antibiotics have emerged as prominent contaminants, which are frequently detected in sewage and rivers and pose a threat to aquatic microbial community. As a typical primary producer, periphyton is crucial for maintaining the biodiversity and functions of aquatic ecosystem. However, effects of antibiotic exposure time as well as the recovery process of periphyton remain undetermined. In the present study, five exposure scenarios of two typical macrolides, erythromycin (ERY) and roxithromycin (ROX) were investigated at…

Goniodomic Acid, a Transient Oxirane Intermediate in the Conversion of the Macrolide Algal Toxin Goniodomin A to Seco Acids.

The algal macrolide goniodomin A (GDA) undergoes ring-cleavage under unusually mild, alkaline conditions to form mixtures of stereoisomers of seco acids GDA-sa and iso-GDA-sa. In the primary fragmentation pathway, opening of the macrolide ring occurs by displacement of the carboxyl group by a base-catalyzed attack of the C32 hemiketal hydroxy group on C31, yielding an oxirane-carboxylic acid, named goniodomic acid. The oxirane ring is unstable, undergoing solvolytic opening to form mainly GDA-sa. Experimental support for this pathway obtained by carrying out the ring-opening reaction in H218O

The Mechanism of Action and Clinical Efficacy of Low-Dose Long-Term Macrolide Therapy in Chronic Rhinosinusitis.

Various chronic inflammatory airway diseases can be treated with low-dose, long-term (LDLT) macrolide therapy. LDLT macrolides can be one of the therapeutic options for chronic rhinosinusitis (CRS) due to their immunomodulatory and anti-inflammatory actions. Currently, various immunomodulatory mechanisms of the LDLT macrolide treatment have been reported, as well as their antimicrobial properties. Several mechanisms have already been identified in CRS, including reduced cytokines such as interleukin (IL)-8, IL-6, IL-1β, tumor necrosis factor-α, transforming growth factor-β, inhibition of neutr…

Clinical Characteristics of Macrolide-Refractory Mycoplasma pneumoniae Pneumonia in Korean Children: A Multicenter Retrospective Study.

Mycoplasma pneumoniae is a major causative pathogen of community-acquired pneumonia in children, and the treatment of choice is macrolides. There is an increasing trend in reports of refractory clinical responses despite macrolide treatment due to the emergence of macrolide-resistant M. pneumoniae. Early discrimination of macrolide-refractory M. pneumoniae pneumonia (MrMP) from macrolide-sensitive M. pneumoniae pneumonia (MSMP) is vital; however, testing for macrolide susceptibility at the time of admission is not feasible. This study aimed to identify the characteristics of MrMP in Korean chi…
