We report three patients with severe Guillain-Barré syndrome (GBS). On clinical examination, no sensory deficit was elicited. Electromyography (EMG) and motor nerve conduction velocity (NCV) studies were indicative of an axonal lesion. In two patients, we found total peripheral conduction block without volitional EMG activity. Sensory NCV and cortical median nerve evoked potentials remained normal. Sural nerve biopsy revealed unequivocal alterations of sensory nerve fibers, some in the form of primary demyelination. Therefore, these cases must bei classified as combined motor-sensory syndromes

Many side effects of steroid treatment have been reported. Even experienced neurologists may not know all important side effects to the nervous system. Autonomic nervous system dysfunction, psychosis and myopathy are more frequently encountered than dependency, reversible dementia, brain atrophy and benign intracranial hypertension. This review describes symptomatology, pathogenesis and treatment of these steroid-induced side effects.

Different forms of symptomatic dystonia have been reported in patients with multiple sclerosis, but the appearance of spasmodic torticollis is extremely rare. We present the case of a 24-year-old man with postural and intentional left hand tremor and leftward spasmodic torticollis. The duration of the disease is 6 years, without remissions. The cerebrospinal fluid examination revealed oligoclonal (two bands) hypergammaglobulinemia. Auditory and visual evoked potentials showed altered sensory afferentation. On magnetic resonance imaging brain scan, multiple T2 high signal lesions involving the

Essential tremor (ET) has been described as a monosymptomatic disorder. In reports describing large series of patients with ET, there are rare patients who exhibit a noticeable gait disorder. However, we have observed that patients with ET and normal gait often exhibit an abnormality of tandem gait. To investigate this observation, we examined whether a gait disorder was present in 36 consecutive patients (mean age 69) with ET. We employed a tremor rating scale that scored tremor amplitude, location, and disability. In all patients, gait and tandem gait were separately evaluated. Eighteen of 3

Mr A ..., 38 years old, follow up for five years for an obsessional nevrosis, hospitalized for a serious breakdown with experiences of derealization, discordance and apragmatism, was treated with haloperidol (15 mg), cyamemazine (200 mg), aenethol trithione (25 mg), associated with clorazepate (150 mg), fluvoxamine (300 mg), trimipramine (200 mg). One month later, the appearance of an extrapyramidal syndrome induced a treatment with biperidone (2 mg). The following day, he was in a state of sub-confusion with temporo-spatial disorientation and anxious perplexity. The extrapyramidal syndrome wa

There is often a high degree of uncertainty concerning the treatment of disc hernias. In cases with uncertain indication for operation only few hard data are available to base therapeutic decisions on. According to the few available controlled studies, which are discussed in detail, surgery yields better results only in the first year after surgery. There is no difference in the longer-term prognosis between surgery and physiotherapy. The need for further controlled trials using modern imaging techniques is obvious.

The term extrapyramidal system originally referred to the system concerned with motor function other than the pyramidal tract. This review covers papers that concern the basal ganglia and related structures except disorders relating to the cerebellum. Because the pathophysiologies of these disorders remain unknown, I review those papers that focused or implicated these points.

The authors review the literature describing the neural symptoms induced by tricyclic antidepressants, especially tremor, seizures, akathisia, myoclonus, dyskinesia and delirium. Sedation, modifications of sleep, memory and appetite are also described. Tremor and myoclonus are the most frequent drug-induced neural symptoms. Delirium is most often caused by high-dosage treatments. The pathophysiology of akathisia and dyskinesia raises important questions concerning the mode of action of antidepressants.

We report a 14-month-old male with infantile opsoclonus polymyoclonia syndrome. His clinical disabilities responded favorably to high-dose human immunoglobulin therapy. High dose immunoglobulin therapy is one of the therapies for patients with this syndrome.

In a survey of 47 women of all ages with Tourette syndrome, 26% had a premenstrual increase in tics (P less than .001) when various hormonal states were examined. An increase in tics at menarche predicted the premenstrual tic exacerbation (P less than .001). In contrast, other hormonally influenced conditions, such as premenstrual syndrome, oral contraceptive use, pregnancy, and menopause were not associated with a change in tic frequency. The finding of an increase in tic frequency during the estrogenic phase of the menstrual cycle provides additional insight into an important control mechani

Separate episodes of both catatonia and the so-called 'neuroleptic malignant syndrome' (NMS) occurred within the same patient. The only evidence for NMS in this case was prior administration of a neuroleptic and the presence of generalised muscular rigidity. It is suggested that it is misleading to view these conditions as separate diagnostic entities and that NMS is probably more correctly incorporated into the catatonic disorders.

Two patients who postoperatively developed extensive multiseptated hydrosyringomyelia following surgical repair of a lumbal meningomyelocele are reported. Since MRI has been available, an increasing number of reports showed that MRI is useful in the diagnosis of hydrosyringomyelia. Hydrosyringomyelia can be considered as a dysraphic lesion. Etiology and pathogenesis of hydrosyringomyelia are still not fully understood. Probably arachnoidal adhesions and cord tethering in both patients may be potential factors in producing cystic degeneration of the underlying structure secondary to ischemia.

A study is presented of 37 cases of chronic subdural hematomas. Initial manifestations: headache, epileptic seizures, mental disorders. Involvement of the III, V, VII cranial nerves, anisocoria almost in 50% on the contralateral side were noted. CSF xanthochromia is of certain diagnostic significance. Carotid angiography is the most informative method. Osteoplastic trephination with evacuation of the hematoma is the method of choice.

Patients diagnosed as suffering from common migraine according to the "Ad hoc committee" criteria of 1962 have been studied as for type of unilaterality of headache. Patients, with trauma to the face, head, and neck were excluded, together with patients with the faintest suspicion of aura and those with chronification of the headache. Common migraine criteria, as stipulated by the IHS and Vahlquist, were counted in every patient (n = 32), except the laterality which was a free variable (a total of 8 variables remaining). For comparison, as far as the number of migraine criteria is concerned, a

The etiology of Bell's palsy is still obscure, but the hypothesis that hypoxia and compression of the nerve induced by edema in the Fallopian canal are the main causes of Bell's palsy is widely accepted. Tojima (1988) reported that the motor nerve conduction velocity (MCV) gradually decreased as degeneration of the nerve fibers progressed in Bell's palsy. The majority of facial nerve fibers are myelinated, and the greater the fiber diameter, the faster the conduction velocity. For this reason, Tojima suggested that Wallerian degeneration in Bell's palsy would begin from the thicker myelinated

An hypothesis of increased endorphinergic activity has been proposed to account for the characteristic symptoms of Rett syndrome. Cerebrospinal fluid samples from eight girls with Rett syndrome were analysed for beta-endorphin (beta-EP) immunoactivity and compared with samples from a control group of 15 children with acute leukaemia in remission. Severity of symptoms was not found to be related to beta-EP level. A group of early-treated adolescents with phenylketonuria had beta-EP levels similar to the Rett syndrome patients, but no symptoms resembling theirs. Therefore it is unlikely that inc
